Engaging community to co-design a multilevel intervention to reduce lung cancer disparities in persistent poverty tracts in California through group model building and simulation.

OBJECTIVES: This study examined social determinants and structural barriers to lung cancer outcomes in Kern and Fresno counties, California, and co-developed a multilevel intervention strategy informed by community perspectives. DESIGN: Engaging stakeholders through group model building (GMB) to elicit their knowledge to build a system dynamics (SD) simulation model for intervention strategy design. SETTING AND PARTICIPANTS: We identified and trained four community members from two community-based organisations in Central Valley, California, to help recruit GMB participants. 14 community mem

Homelessness and the response to emerging infectious disease outbreaks: lessons from SARS.

During the 2003 severe acute respiratory syndrome (SARS) outbreak in Toronto, the potential introduction of SARS into the homeless population was a serious concern. Although no homeless individual in Toronto contracted SARS, the outbreak highlighted the need to develop an outbreak preparedness plan that accounts for unique issues related to homeless people. We conducted key informant interviews with homeless service providers and public health officials (n = 17) and identified challenges specific to the homeless population in the areas of communication, infection control, isolation and quarant

Contraceptive use and maternal mortality in Indonesia: a community-level ecological analysis.

BACKGROUND: Prior studies have shown that contraceptive use reduces maternal mortality independently of other maternal health services. The present study took advantage of geographically detailed Indonesian data to study the interplay between contraceptive use and other risk and protective factors for maternal mortality at the community level, a level of analysis where the protective effects of family planning can be best understood. METHODS: Data from the 2015 Intercensal Population Survey (SUPAS) and the 2014 Village Potential Survey (PODES) were used to construct a series of census block-l

[Cross-Sector and Inter-Agency Collaboration in Intensive Psychiatric Home Treatment: Qualitative Process Evaluation].

OBJECTIVE: Intensive psychiatric home treatment (IHT) in Germany provides, for the first time, a legal framework for delegating hospital psychiatric services to third-party providers. This study examines implementation models, incentives, prerequisites, challenges, and effects. METHODS: Interviews with clinic representatives (n=13) and delegation partners (n=8) were analyzed using content analysis. RESULTS: Three models emerge: cross-sectoral concepts, overcoming geographic barriers, and personnel substitution. Incentives include reimbursement and service expansion. Cooperation requires visi

Chronic oedema: a prevalent health care problem for UK health services.

Chronic oedema (CO) is a major clinical problem worldwide, which has many important secondary consequences for health, activity and participation. Effective treatment planning and organisation of services is dependent on an understanding of the condition and its epidemiology. This cross-sectional study was designed to estimate the point prevalence of CO within the health services of one UK urban population and to determine the proportions that have concurrent leg ulceration. Patients with CO in all anatomic sites were ascertained by health care professionals in one acute and one community hosp

Adolescent contraception: survey of attitudes and practice of health professionals.

In Nigeria, there is a persistent reluctance to provide adolescents with contraceptives and this is based largely on the premise that the culture does not support pre-marital sexual activity. However, several studies have revealed that a large number of adolescents are experiencing early sexual debut, some as early as 13 years. Furthermore, mortality resulting from termination of unwanted pregnancies is on the increase among Nigerian adolescents. The aim of this study is to assess the attitudes and practice of health workers in Ibadan regarding adolescent contraception. A structured questionna

An integrated community health worker intervention in rural Nepal: a type 2 hybrid effectiveness-implementation study protocol.

BACKGROUND: Evidence-based medicines, technologies, and protocols exist to prevent many of the annual 300,000 maternal, 2.7 million neonatal, and 9 million child deaths, but they are not being effectively implemented and utilized in rural areas. Nepal, one of South Asia's poorest countries with over 80% of its population living in rural areas, exemplifies this challenge. Community health workers are an important cadre in low-income countries where human resources for health and health care infrastructure are limited. As local women, they are uniquely positioned to understand and successfully n

Evaluation of the Effectiveness of a Bilingual Nutrition Education Program in Partnership with a Mobile Health Unit.

There are limited reports of community-based nutrition education with culinary instruction that measure biomarkers, particularly in low-income and underrepresented minority populations. Teaching kitchens have been proposed as a strategy to address social determinants of health, combining nutrition education, culinary demonstration, and skill building. The purpose of this paper is to report on the development, implementation, and evaluation of Journey to Health, a program designed for community implementation using the RE-AIM planning and evaluation framework. Reach and effectiveness were the p

A couple-focused, integrated unplanned pregnancy and HIV prevention program in urban and rural Zambia.

BACKGROUND: Zambia's total fertility rate (5 births per woman) and adult HIV prevalence (11.5%) are among the highest in the world, with heterosexual couples being the most affected group. Jointly counseling and testing couples for HIV has reduced up to 58% of new HIV infections in Zambian clinics. Married women using contraceptives in Zambia have a high (20%) unmet need for family planning and low (8.6%) uptake of cost-effective long-acting reversible contraceptives. We present an integrated counseling, testing, and family-planning program to prevent HIV and unplanned pregnancy in Zambia. OB

Health inequalities and deprivation: a policy lens on community nursing.

With the government's 10-year health plan for England high on the agenda, there has never been a more poignant time for community services to drive forward their position in tackling the health inequalities in their local populations. Through the lens of recent policy and guidance, the author focuses on the potential role that community nurses will have in designing and delivering services as care is moved from hospital to local neighbourhood health centres. The unique perspective that community nurses bring, combining experience, knowledge and an insight into areas of deprivation, is widely r

Evidence base for primary prevention of obesity in children and adolescents.

Effective interventions to prevent obesity in children have never been more necessary. There have been over 30 published reviews and meta-analyses on such interventions (randomized and controlled trials) since 2008. In summary, interventions which involve the whole community (community-based) in complex interventions (promoting healthy eating, reduction in sedentary behaviours and increase in physical activity) that target environments and upstream determinants appear to be more effective. In this article the strengths and weaknesses of community-based complex interventions which aim to preven

A multisector community-engaged collaborative for mental health integration in primary care and housing developments: Protocol for a stepped-wedge randomized controlled trial (the Harlem Strong Program).

BACKGROUND: Addressing mental health disparities following COVID-19 requires adaptive, multi-sectoral, equity-focused, and community-based approaches. Mental health task-sharing in gateway settings has been found to address mental health care gaps in low- and middle-income countries, but is not a common practice in the U.S., especially in non-medical settings, such as low-income housing developments (LIH). This research study will evaluate the effectiveness of a multisectoral community-engaged collaborative for task-sharing mental health care on consumer, provider, and implementation outcomes,

Contraceptive Practice in Sub-Saharan Africa.

Forty eight of the African continent's 54 sovereign states are located in the Sub-Saharan Africa (SSA) region, with the government of each defining and shaping its own health services and delivery systems. This paper reviews the trends and patterns of contraceptive practice in the region. Using survey data available from the Demographic and Health Surveys and Performance Monitoring and Accountability 2020, the study finds modern contraceptive practice to be on the rise overall but with much geographic variation. The contraceptive methods most frequently used are injectables and, more recently,

A model to improve preparedness and strategically enhance resiliency at a community level: The Garden State approach.

This paper focuses on community resiliency planning in high-density areas, concentrating on balancing critical infrastructure recovery needs with real-life limitations in funding, and knowledge for how emergency management is done. A case study examines how a New Jersey government agency and state university were able to pilot a community resiliency concept in order to rapidly inject emergency management knowledge into a community that was affected by Superstorm Sandy. The lessons learned are intuitively understood by emergency planning professionals: preparing continuity of operations plans,

Infectious Disease Physicians' Perceptions About Ebola Preparedness Early in the US Response: A Qualitative Analysis and Lessons for the Future.

On September 30, 2014, the first US patient with Ebola virus disease was diagnosed. Hospitals and healthcare systems identified many complex issues that needed to be addressed to prepare for possible future outbreaks. Here we summarize themes identified in free text responses from a query of infectious disease physicians from the Infectious Disease Society of America's (IDSA) Emerging Infections Network (EIN) early in the domestic Ebola response and place them into the context of biopreparedness for possible future events. We queried infectious disease physician members of the EIN from October

Operationalizing diagnostic criteria for Alzheimer's disease and other age-related cognitive impairment-Part 2.

This article focuses on the effects of operational differences in case ascertainment on estimates of prevalence and incidence of cognitive impairment and/or dementia of the Alzheimer type. Experience and insights are discussed by investigators from the Framingham Heart Study, the East Boston Senior Health Project, the Chicago Health and Aging Project, the Mayo Clinic Study of Aging, the Baltimore Longitudinal Study of Aging, and the Aging, Demographics, and Memory Study. There is a general consensus that the single most important factor determining prevalence estimates of Alzheimer's disease (

Comprehensive care in Huntington's disease: a physician's perspective.

Huntington's disease is a slowly progressive neurodegenerative disorder with wide-ranging effects on affected individuals and their families. Until a cure is found for the disease, patients and their families will continue to need care over years, even generations. The ideal care for HD is provided by a team, led by a physician, with input from rehabilitation therapists, nurses, psychologists, genetic counselors, social workers, and other health care providers. The goals of care are to maximize the quality of life at all points through the course of the disease, in part by anticipating problem

Private midwives: a new approach to family planning service delivery in Ghana.

Data on 134 private sector midwives who were educated and trained in family planning service delivery by the Ghana Registered Midwives' Association (GRMA) showed that, for most, the course was the first they had received since qualifying as a midwife. Four out of five of the new family planning clients served by these midwives were first time contraceptive users. Approximately 9% of the clients were men, most of whom requested condoms. While delivering women is still the main function of the maternity homes, it is clear that family planning services have come to represent an important element
