Impact of Column Support Stiffness on the Mechanical Performance of Flat Frame Structural Systems Supporting Thin-Walled Folded Roofs.

This article presents a new parametric method for shaping flat transverse frame structural systems supporting thin-walled roofs made of flat sheets folded unidirectionally and transformed elastically to various shell forms. The parameterization was limited to one independent variable, that is the stiffness of the support joints. For different discrete values of simulated stiffness, the surface areas of the cross sections of the tensile and compressed elements and the section modulus of the bending elements were calculated so as to obtain the optimized work of the frame and its elements in the

Development of a Machine Learning Algorithm to Correlate Lumbar Disc Height on X-rays with Disc Bulging or Herniation.

Lumbar disc bulging or herniation (LDBH) is one of the major causes of spinal stenosis and related nerve compression, and its severity is the major determinant for spine surgery. MRI of the spine is the most important diagnostic tool for evaluating the need for surgical intervention in patients with LDBH. However, MRI utilization is limited by its low accessibility. Spinal X-rays can rapidly provide information on the bony structure of the patient. Our study aimed to identify the factors associated with LDBH, including disc height, and establish a clinical diagnostic tool to support its diagno

Hypogonadism in men with prolactinoma: Diagnosis, treatment, and management of persistent hypogonadism.

Prolactin-secreting adenomas comprise approximately 50 % of all clinically relevant pituitary adenomas. Most men with prolactinomas present with large and invasive tumors. Despite effective medical therapy with dopamine agonists and prolactin normalization, over 20 % of men with prolactinomas will remain with hypogonadism. There are two suggested mechanisms for hypogonadism: central suppression of the hypothalamic-pituitary-gonadal axis caused by elevated prolactin levels leading to inhibition of the kisspeptin neurons in the hypothalamus and loss of pulsatile luteinizing hormone secretion, an

Intense pulsed light treatment in meibomian gland dysfunction: Past, present, and future.

Dry eye disease is a highly prevalent condition, which can substantially impair quality of life, work productivity, and vision. It is considered an inflammatory disease and the most common cause is meibomian gland dysfunction. Despite many treatment alternatives being available, including artificial tears, warm compresses, antibiotics, and anti-inflammatory therapy, lasting treatment effects are rare. Over the last two decades, intense pulsed light therapy, after being well established in dermatology, has been gradually introduced to the field of ophthalmology to treat meibomian gland dysfunct

Image reconstruction based on compressed sensing for sparse-data endoscopic photoacoustic tomography.

Endoscopic photoacoustic tomography (EPAT) is an interventional application of photoacoustic tomography (PAT) to visualize anatomical features and functional components of biological cavity structures such as nasal cavity, digestive tract or coronary arterial vessels. One of the main challenges in clinical applicability of EPAT is the incomplete acoustic measurements due to the limited detectors or the limited-view acoustic detection enclosed in the cavity. In this case, conventional image reconstruction methodologies suffer from significantly degraded image quality. This work introduces a com

Cerebral Edema and Elevated Intracranial Pressure.

PURPOSE OF REVIEW: This article reviews the management of cerebral edema, elevated intracranial pressure (ICP), and cerebral herniation syndromes in neurocritical care. RECENT FINDINGS: While corticosteroids may be effective in reducing vasogenic edema around brain tumors, they are contraindicated in traumatic cerebral edema. Mannitol and hypertonic saline use should be tailored to patient characteristics including intravascular volume status. In patients with traumatic brain injury who are comatose, elevated ICP should be managed with an algorithmic, multitiered treatment protocol to maintai

A study of persistent unilateral middle ear effusion caused by occult skull base lesions.

The goal of this article is to review a series of patients with persistent unilateral middle ear effusion (MEE) and to suggest a more contemporary diagnostic algorithm. The author conducted a retrospective chart review of adults with persistent unilateral MEE and normal findings on physical and nasopharyngoscopic examinations whose MEE was eventually found to be caused by a variety of occult skull base lesions. The study population was made up of 79 patients-52 women and 27 men, aged 21 to 83 (mean: 54.8) at presentation-who had been referred to an academic tertiary care medical center between

Concept of gunshot wound spine.

Gunshot wound (GSW) to the spine which was earlier common in the military population is now being increasingly noted in civilians due to easy availability of firearms of low velocity either licensed or illegal combined with an increased rate of violence in the society. Contributing to 13% to 17% of all spinal injuries, the management of complex injury to the spine produced by a GSW remains controversial. Surgery for spinal cord injuries resulting from low velocity GSWs is reserved for patients with progressive neurologic deterioration, persistent cerebrospinal fluid fistulae, and sometimes for

A theoretically based index of consciousness independent of sensory processing and behavior.

One challenging aspect of the clinical assessment of brain-injured, unresponsive patients is the lack of an objective measure of consciousness that is independent of the subject's ability to interact with the external environment. Theoretical considerations suggest that consciousness depends on the brain's ability to support complex activity patterns that are, at once, distributed among interacting cortical areas (integrated) and differentiated in space and time (information-rich). We introduce and test a theory-driven index of the level of consciousness called the perturbational complexity in

Biomechanics of the weakened mandible: use of image correlation analysis.

Uninterrupted resection of mandibular bone is often necessary during maxillofacial operations for cancer. This weakens the mandible, and increases the risk of fracture. To our knowledge no biomechanical analysis has been made of deformations and strains that occur during chewing if this happens, so we have made such an analysis of the weakened mandible using a new technique: image correlation. Five fresh explanted human mandibles were prepared with black and white lacquer, and placed in a loading device that allowed replication of a physiological biting exercise. Calibrated pieces of bone were

Inadvertent arterial catheterization complicating femoral venous access for haemodialysis.

OBJECTIVE: Large-bore catheters for temporary haemodialysis are often placed via the internal jugular or femoral vein, guided by external landmarks or ultrasound techniques. Inadvertent femoral artery catheterization may occur during attempted placement of the dialysis catheter in the femoral vein. MATERIAL AND METHODS: This investigation was carried out in Skåne University Hospital, Malmö, Sweden. Between 2008 and 2011, patients referred for consultation by a vascular specialist owing to inadvertent arterial catheterization after attempted placement of a dialysis catheter in the femoral vein

Reduced hands-off-time and time to first shock in CPR according to the ERC Guidelines 2005.

BACKGROUND AND AIM: Chest compressions and early defibrillation are crucial in cardiopulmonary resuscitation (CPR). The Guidelines 2005 brought major changes to the basic life support and automated external defibrillator (BLS-AED) algorithm. We compared the European Resuscitation Council's Guidelines 2000 (group '00) and 2005 (group '05) on hands-off-time (HOT) and time to first shock (TTFS) in an experimental model. METHODS: In a randomised, cross-over design, volunteers were assessed in performing BLS-AED over a period of 5min on a manikin in a simulated ventricular fibrillation cardiac arr

Imaging of tinnitus: a review.

Tinnitus, a buzzing or ringing in the ear, may be pulsatile or continuous (nonpulsatile). The distinction, with a detailed clinical evaluation, determines the most appropriate imaging study. Pulsatile tinnitus suggests a vascular neoplasm, vascular anomaly, or vascular malformation. Most of the neoplasms are glomus tympanicum and glomus jugulare tumors. Vascular anomalies may cause pulsatile tinnitus, but the mechanism is unknown, and another (treatable) cause should be sought. Most neoplasms and anomalies are best seen on bone algorithm computed tomographic (CT) studies. Dural vascular malfor

Changes of median nerve conduction, cross-sectional area and mobility by radioulnar wrist compression intervention in patients with carpal tunnel syndrome.

BACKGROUND: Owing to the compressive nature of the neuropathy, patients with carpal tunnel syndrome (CTS) have prolonged distal motor latency (DML), sensory nerve latency (SNL), median nerve swelling and restricted median nerve mobility. The purpose of this study was to noninvasively augment carpal tunnel space using radioulnar wrist compression (RWC) and evaluate its effects on median nerve pathological properties in patients with CTS. It was hypothesized that the RWC intervention would reduce the median nerve DML, SNL and cross-sectional area (CSA) and enhance longitudinal median nerve mobil

Management of immediate and delayed postpartum hemorrhage with cesarean delivery.

The management of immediate and delayed postpartum hemorrhage with cesarean delivery presents specific challenges in diagnosis and treatment. Real-time monitoring and quantitation of intraoperative and postoperative blood loss facilitate rapid recognition of immediate or delayed abnormal bleeding, enabling timely intervention. Effective response to ongoing postpartum hemorrhage depends on clear communication and collaboration among the nursing, obstetric, and anesthesiology teams. Medical management involves multiple courses of uterotonics and associated resuscitative measures, which should be

Speech Perception in Noise: A Narrative Review of Cognitive Contribution and Hearing-Aid Signal Processing.

BACKGROUND: Environmental noise is one of the most pervasive ecological health issues in modern urban settings, contributing to the onset of hearing damage but also to the development of annoyance, sleep disturbance, cardiovascular diseases, and impaired communication. For people with hearing impairment of any age, the main challenge posed by noisy environments is not merely detecting sound but understanding speech against a competing background-a task heavily related to cognition. METHODS: Narrative review; the literature has been identified through searches of PubMed, Scopus, and Google Sch

Treatment of male stress urinary incontinence at time of inflatable penile prosthesis placement a review of contemporary literature.

BACKGROUND AND OBJECTIVE: Male stress urinary incontinence (SUI) and erectile dysfunction (ED) are well established diagnoses within Men's Health, often more specifically within the prostate cancer survivorship cohort. Taken individually, well defined treatment algorithms exist with which many surgeons are comfortable; however, treatment of both in a single setting or staged fashion introduces complexity. Emerging treatment options also exist, and there is immature or minimal data when these are combined with inflatable penile prosthesis (IPP) insertion, radiation history, and/or variable degr
