Fifty women attending a clinic for sexually transmitted diseases with vulval condylomata acuminata were examined by cervical cytology and colposcopy for cervical infection by human papillomavirus (HPV) or epithelial abnormality indicating cervical intraepithelial neoplasia (CIN) or both. Collated results showed a high prevalence of both conditions in these 50 women; 25 (50%) had evidence of cervical infection by HPV and 18 (36%) epithelial abnormalities consistent with CIN 1 or 2.

Our review of the current use of the carbon dioxide laser in treating vulvar disease reveals that it holds significant promise in the treatment of vulvar intraepithelial carcinoma, condylomata acuminata, and focal vestibular adenitis. The long-range results of such treatment, however, are not yet known. Until sufficient data are accumulated to demonstrate the efficacy of the laser in treating the vulvar dystrophies and herpes genitalis, it should be used with caution, and the results should be carefully evaluated and reported.

A study of specific immune response in guinea-pigs after immunization with human papilloma viruses (HPV) purified from plantar warts, hand warts and condyloma acuminata is reported. Patients with condylomata acuminata of the vulva usually have increased difficulty to achieve complete response to treatment and also have a higher risk for disease recurrence. Treatment for this disease varies, including surgical excision, cryotherapy, electrocautery, CO2 laser therapy, topical therapy, and photodynamic therapy (PDT), but none of these alone provides a satisfactory outcome, especially for giant condyloma acuminatum (GCA). We reported two cases of GCA successfully cured with surgical removal, electrocautery and photodynamic therapy.

Morphological and epidemiological studies point to a close correlation between viral genital warts and genital cancer. Benign condylomata acuminata are commonly induced by HPV-6 and HPV-11. In precancerous bowenoid genital lesions, HPV-16 and HPV-18 could be cloned directly from cervical cancer tissue. Whereas the occurrence of HPV-6 and HPV-11 seems to be a low risk infection, HPV-16 and HPV-18 may represent a high risk for malignant conversion.

Based on histology and electron microscopy, a series of Papova virus acanthomas consisting of 300 common warts and Condylomata acuminata as well as primary efflorescences of 7 typical and 7 questionable cases of Epidermodysplasia verruciformis have been classified. Four cytological types expressing different cytopathogenic viral actions were established. Typ 4 ("basophilic foamy giant keratinocytes") seems to be specific for Epidermodysplasia verruciformis which thus can be differentiated histologically against ordinary warts (type 1-3).

Epidermolytic acanthomas (EA) are rare benign tumors of unclear etiology that present as flat, sometimes slightly keratotic, pale or whitish papules that are usually asymptomatic. Not uncommonly, their clinical appearance in the anogenital area might lead to misdiagnosis as other lesions that commonly develop at this site, such as condylomata acuminata. Though mainly asymptomatic, there are also reports of EA presenting with persistent genital pruritus. We describe the first reported case of pruritic scrotal EA successfully treated with topical pimecrolimus.

Giant condyloma is a variation of condylomata acuminata caused by a sexually transmitted virus infection. Although it is a benign disease, it carries a risk of malignant transformation. A 5-year study of 668 patients with genital lesions revealed that teenagers and those in the age group of 20 to 40 are most vunerable. The poor, the uneducated and unmarried persons are at high risk. Chancroid and syphilis are the majority of cases. Inguinal bubo, herpes pregenitalis, condylomata acuminata, erosive balanitis and traumatic ulcers were seen less frequently. Most of the genital lesions had the classic clinical features. The findings of this study suggest that the pattern of genital lesions is showing borderline changes.
