Unusual Presentation of Confusion: Hyperammonaemic Hepatic Encephalopathy Due to Intrahepatic Portosystemic Venous Shunt.

PRESENTATION: A 75-year-old man presented to the emergency department with a 5-week history of confusion, daytime somnolence, and fatigue. His physical examination was unremarkable. DIAGNOSIS: Investigations initially showed deranged Liver function tests. Extensive investigations were done following that including CT brain, MRI brain, septic screen, Arterial blood gas, and Lumbar puncture, which were all inconclusive. An Electroencephalogram was suggestive of Encephalopathy. His Ammonia level was high (123). Triphasic CT liver, revealed Intrahepatic portosystemic venous shunt and was diagnose…

Bodily confusion: Lower differentiation of emotional and physiological states in student alcohol users.

BACKGROUND: Alexithymia, difficulty in recognising and naming emotions, is common among people who use alcohol. There is also emerging evide … k drinkers [F(1,1998) = 441.16; p < 0.001], confirming that higher alcohol use is related to higher confusion of emotional and non-emotional bodily feelings. CONCLUSIONS: Plausibly, this increased bodily confusion drives alcohol consumption: alcohol may serve as a means of dealing with undifferentiated changes in psychophysiological arousal accompanying em …

A confusion of pathways: Discerning cell death mechanisms in SARS-CoV-2 infection.

Upon SARS-CoV-2 infection, infected cells undergo necroptosis, whereas delayed apoptosis and pyroptosis occur in uninfected, bystander cells, thus providing a plausible explanation for the extensive injury among myriad uninfected cells.

Monte Carlo simulation with confusion matrix paradigm - A sample of internal consistency indices.

Monte Carlo simulation is a common method of providing empirical evidence to verify statistics used in psychological studies. A representative set of conditions should be included in simulation studies. However, several recently published Monte Carlo simulation studies have not included the conditions of the null distribution of the statistic in their evaluations or comparisons of statistics and, therefore, have drawn incorrect conclusions. This present study proposes a design based on a common statistic evaluation procedure in psychology and machine learning, using a confusion matrix with fou…

A bird's-eye view of pathologist over diagnostic confusion of oral cavity lesions.

INTRODUCTION: Oral cavity can be host to multitude of neoplastic, premalignant or non neoplastic pathological lesions. Diagnosis of lesions of oral cavity is always of interest to clinician and pathologist and rely on clinical appearance of lesions. There can be variation in diagnosis of clinical lesion with histopathology. Many oral carcinomas arise within the sites that previously had premalignant lesion. Incidence of oral cancers in population has increased among younger generations related to habits and lifestyle. These lesions during clinical presentation are misleading and create diagnos

Botulinum toxin injection, dilution confusion: The impact of toxin diffusion on clinical practice.

Botulinum toxins (BoNT) are a standard of care for spasticity management of children with a variety of neuromuscular (NM) conditions. BoNT relaxes skeletal muscles by inhibiting the release of acetylcholine from the neuromuscular junction (NMJ). As part of their training physiatrist become proficient in the targeted injections of BoNT into affected skeletal musculature. While the pharmacology and pharmacokinetics of BoNT are well characterized, there is limited literature on the clinical impact that varying the volume of diluent that a toxin is reconstituted within. In theory, injection of a l

Cryptococcal Anterior Glottic Web from Disseminated Infection Presenting as Confusion: A Case Report.

OBJECTIVES: To educate healthcare providers on Cryptococcus neoformans as a novel cause of glottic webs especially in an immunocompromised patient and discuss recurrence of the glottic web. This case also emphasizes the importance of a comprehensive patient evaluation in cases such as this, as laryngeal involvement in this case was only a portion of the patient's disease burden. METHODS: An extensive review of the patient's clinical course was assessed including patient presentation, diagnostic techniques, medical and surgical treatment, and complications. RESULTS: Prolonged Fluconazole ther

Frequent falls and confusion: recurrent hypoglycemia in a patient with tuberous sclerosis complex.

Recurrent hypoglycemia is common, but its presentation is often insidious resulting in delays in diagnosis and significant morbidity. We describe a case of an insulinoma presenting with falls and confusion in a patient with tuberous sclerosis, demonstrating the importance of early hypoglycemia identification and a potential shared molecular pathogenesis.

Steroid-responsive encephalopathy associated with autoimmune thyroiditis presenting as confusion, dysphasia, and myoclonus.

Steroid response encephalopathy associated with autoimmune thyroiditis (SREAT), or Hashimoto's encephalopathy, is a rare disorder believed to be immune-mediated. It is most often characterized by a subacute onset of confusion with altered level of consciousness, seizures, and myoclonus. We describe the case of a 48-year-old gentleman who presented with confusion and dysphasia. Specific clinical features and laboratory results led to a diagnosis of Hashimoto's encephalopathy. This case highlights the core features of this condition and the potential for complete response to steroid therapy.

Guidelines on interferon-γ release assays for tuberculosis infection: concordance, discordance or confusion?

Identification of latent tuberculosis (TB) infection and preventive therapy is important for TB control, especially in high-risk populations. Since the advent of interferon-γ release assays (IGRAs), many studies have evaluated their role in the diagnosis of active and latent TB. With the growing evidence base, many guidelines now include IGRAs. We surveyed the literature and contacted experts to identify 33 guidelines and position papers from 25 countries and two supranational organizations. The results show considerable diversity in the recommendations on IGRAs, with four approaches commonly

Can alcohol intoxication be excluded as the cause of confusion following head injury?

OBJECTIVE: To assess whether there is a breath alcohol level (BrAC) below which confusion in the head injured patient should not be attributed solely to the acute effects of alcohol. METHOD: Based in the Accident and Emergency Ward in Glasgow Royal Infirmary, a prospective observational study was carried out over a five month period. Patients admitted to the ward were recruited for the study if they had a primary diagnosis of head injury. The outcome measures recorded and analysed were sequential 2 hrly BrAC readings (mg/L) and Glasgow Coma Scale findings (Eye opening, motor and verbal respon…

[Dissociated horizontal deviations (DHD): nomenclature and etiology. An orientating attempt in terminological confusion].

PURPOSE: To classify dissociated horizontal deviations (DHD) based on the etiology of the vergence that causes the dissociation of the squint angle. METHODS: Dissociated strabismus can reliably be diagnosed by a change of the squint angle caused by an alternation in the fixation from one eye to the other, when all the other conditions (head posture, direction of gaze, fixation distance, accommodation) remain unchanged. The decisive diagnostic tool is the reversed fixation test: During monocular fixation with one eye, the squint angle of the other eye is neutralized using a synoptometer or a p

Panic disorder cases in Japanese-Brazilians in Japan: their ethnic and cultural confusion.

The comparatively high salaries made in Japan are attractive to many Japanese-Brazilians. The number of individuals from this ethnic group being treated in Japanese mental hospitals has increased. We hypothesized that Japanese-Brazilian patients with panic disorders adjusted better to Japanese society and culture than those with other mental disorders. The subjects in the present study are 40 Japanese-Brazilian patients undergoing treatment at the Department of Psychiatry at Jichi Medical School, Japan, from May 1990 to September 1998. Patients were divided into a panic disorder group, a schiz

Non-accidental injury: confusion with temporary brittle bone disease and mild osteogenesis imperfecta.

Accurate diagnosis of non-accidental injury (NAI) can be reached in the majority of cases by careful appraisal of the social and family history, combined with painstaking clinical roentgenographic and other imaging evaluations. Careful review of the scientific literature clearly indicates that collagen analysis to exclude mild forms of osteogenesis imperfecta, especially type IV, is recommended only in rare cases in which diagnosis of NAI remains in doubt even after thorough evaluation by experienced radiologists and/or other physicians. Until clinical research scientifically establishes the e
