The Clinical and Electroencephalographic Spectrum of Tilt-Induced Syncope and "Near Syncope" in Youth.

BACKGROUND: The aim of the study was to characterize the clinical and electroencephalographic (EEG) patterns associated with tilt-induced reflex syncope and delayed orthostatic hypotension without syncope in youth. METHODS: We conducted a prospective observational study of 95 patients referred to a pediatric neurology clinic for head-upright tilt testing. Clinical signs, symptoms, video EEG, and continuous blood pressure and heart rate were monitored. RESULTS: Eighty patients had reflex syncope, and 15 had delayed-onset hypotension without syncope. The mean age was 15.3 (standard deviation ±

When diabetic ketoacidosis becomes a neurological emergency: cerebral edema in a child with new-onset type 1 diabetes mellitus - a case report and narrative review.

BACKGROUND: Diabetic ketoacidosis (DKA) is a life-threatening complication of type 1 diabetes mellitus (T1D), potentially leading to severe outcomes, including cerebral edema (CE). The case describes an eight-year-old child with previously undiagnosed T1D complicated by DKA, and subsequent development of CE. It is followed by a short literature review including other pediatric cases of clinically overt CE. CASE PRESENTATION: A boy was admitted to the outpatient clinic with a several-day history of vomiting, abdominal pain and headache, leading to a presumptive diagnosis of gastrointestinal in

Deconstructing the heterogeneity of adolescent social anxiety: A cognitive diagnostic approach from a sex perspective.

BACKGROUND: The cognitive vulnerability patterns of adolescent social anxiety are highly sex-specific. However, traditional data-driven subtyping approaches often overlook these sex-specific patterns due to a lack of direct theoretical mapping. This study introduces Cognitive Diagnosis Models (CDMs) to identify sex-specific cognitive subtypes, exploring how these cognitive patterns manifest within high-risk comorbid contexts. METHODS: A sample of 2005 Chinese adolescents (Mage = 12.9 years; 54.5% male) was assessed on four core cognitive attributes: post-event rumination, negative social inte

Analysis of the nervous system and gastrointestinal adverse events associated with solifenacin in older adults using the US FDA adverse event reporting system.

BACKGROUND: Antimuscarinics are the backbone of the pharmacological management of overactive bladder. Still, concerns have been raised over the nervous system (NS) adverse drug events (AEs) due to their dissimilarities to muscarinic receptor-subtype affinities. OBJECTIVE: This study aimed to identify the nervous system and gastrointestinal adverse drug events (ADEs) associated with solifenacin use in older adults (≥65 years). METHODS: A case/non-case analysis was performed on the reports submitted to the FDA Adverse Event Reporting System (FAERS) between 01/01/2004 and 30/06/2020. Cases were

Amitraz poisoning: A case report of an unusual pesticide poisoning in Sri Lanka and literature review.

BACKGROUND: Amitraz is a pesticide used worldwide on animals and in agriculture. It contains triazapentadiene, which is a centrally acting alpha-2 adrenergic agonist. Amitraz poisoning is fairly uncommon in humans and occurs via oral, dermal or inhalational routes. Only a limited number of case reports of human intoxication have been published and most of them are of accidental ingestion by children. CASE PRESENTATION: A twenty-year-old Sri Lankan female presented following self-ingestion of 20 ml of amitraz resulting in 37.8 mg/ kg of amitraz poisoning. She lost consciousness after 20 min of

Cerebral oxygenation in patients undergoing shoulder surgery in beach chair position: comparing general to regional anesthesia and the impact on neurobehavioral outcome.

BACKGROUND: Ischemic brain damage has been reported in healthy patients after beach chair position for surgery due to cerebral hypoperfusion. Near-infrared spectroscopy has been described as a non-invasive, continuous method to monitor cerebral oxygen saturation. However, its impact on neurobehavioral outcome comparing different anesthesia regimens has been poorly described. METHODS: In this prospective, assessor-blinded study, 90 patients undergoing shoulder surgery in beach chair position following general (G-group, n=45) or regional anesthesia (R-group; n=45) were enrolled to assess the pr

Delayed Emergence Due to Severe Respiratory Acidosis Following Prolonged Spine Surgery: A Case Report.

Delayed emergence from anesthesia is a frequent challenge in surgical settings. Common etiologies include residual drug effects, metabolic derangements, or neurological insults. We report a case of a 55-year-old woman who underwent an elective six-hour lumbar laminectomy and cervical implantation surgery. Despite an uneventful intraoperative course and routine neuromuscular blockade reversal, the patient exhibited somnolence and ventilatory failure after reversal and extubation. Arterial blood gas (ABG) analysis revealed severe respiratory acidosis. The patient required re-intubation and contr

Initial Dose Tarlatamab-Associated Tumour Lysis Syndrome in Small Cell Lung Cancer: A Case Report.

Tumour lysis syndrome (TLS) in solid tumours is rare but may occur in small-cell lung cancer (SCLC) with a high tumour burden. We report a case of fatal TLS following tarlatamab treatment for relapsed SCLC. A 61-year-old woman with limited-stage SCLC developed recurrence with extensive liver metastases after chemoradiotherapy and durvalumab consolidation therapy. Owing to rapid disease progression, an initial 1-mg step-up dose of tarlatamab was administered as second-line treatment. On Day 2, the patient developed mild cytokine release syndrome, which improved with dexamethasone. On Day 6, she

Prolonged intermittent hemodialysis using a standard dialysate flow rate for severe overdose of sustained-release valproic acid: A case report.

BACKGROUND: Valproic acid poisoning can be life threatening and may require urgent extracorporeal elimination. In particular, sustained-release formulations pose a challenge, as conventional short-duration intermittent hemodialysis may fail to remove the drug sufficiently because of delayed and prolonged drug absorption. While prolonged intermittent hemodialysis is a rational alternative, its clinical effectiveness and safety in cases of severe sustained-release sodium valproate overdose have not been reported. CASE: A woman in her 20 s developed coma after ingesting 45 g (approximately 1100

Should LifeVest Be Worn During Shower: A Case Report.

The wearable cardioverter-defibrillator (WCD), or LifeVest (ZOLL Medical Corporation, an Asahi Kasei company, Tokyo, Japan), is designed for continuous monitoring and immediate treatment of patients at high risk of sudden cardiac arrest (SCA). Due to a lack of water resistance and inability to operate safely in wet conditions, current guidelines recommend wearing the device at all times except for short periods, such as during showering. We present the case of an 88-year-old male with complex coronary artery disease, ischemic cardiomyopathy, and permanent atrial fibrillation who experienced ca

Case report of atypical re-sedation after general anesthesia using remimazolam.

BACKGROUND: Remimazolam, an ultra-short-acting anesthetic with flumazenil as a reversal agent, typically facilitates patient awakening postoperatively. However, our case reveals an unusual occurrence: despite flumazenil initially restoring consciousness, re-sedation due to remimazolam ensued six hours later. CASE: A 65-year-old woman underwent total intravenous general anesthesia with remimazolam and remifentanil during the 140-min surgery. Despite an initially smooth recovery, she progressively became drowsy upon transfer to the general ward, eventually reaching a stuporous state. Multiple i

Hypoglycemic unawareness: challenges, triggers, and recommendations in patients with hypoglycemic unawareness: a case report.

BACKGROUND: Hypoglycemia is a fairly common complication in diabetic patients, particularly in those on insulin therapy. Hypoglycemia symptoms are classified into two types: autonomic and neuroglycopenic symptoms. If a person develops neuroglycopenic symptoms before the appearance of autonomic symptoms or is asymptomatic until blood sugar levels are very low, the patient will develop hypoglycemic unawareness (HU). CASE PRESENTATION: A 25-year-old Iranian woman with HU presented with a severe hypoglycemic episode. This episode was characterized by loss of consciousness and focal neural deficit

A case of dyskinesia after levetiracetam administration.

BACKGROUND: Antiepileptic drug (AED) induced dyskinesia is an unusual manifestation in the medical field. In the previous case reports describing first generation-AED related involuntary movements, the authors suggested that a plausible cause is pharmacokinetic interactions between two or more AEDs. To date, development of dyskinesia after levetiracetam (LEV) has not been reported. CASE PRESENTATION: A 28-year-old woman with a history of brain metastasis from spinal cord glioblastoma presented with several generalized tonic-clonic seizures without restored consciousness. LEV was administered
