Improving management of sexually transmitted infections in primary care: feasibility and acceptability of a new patient management tool for clinicians.

INTRODUCTION Routinely following an evidence-based clinical pathway of care for bacterial sexually transmitted infections (STIs) such as chlamydia or gonorrhoea is important to help reduce the spread of infections, prevent reinfections and avoid associated health complications. AIM To develop an easy-to-use tool for routine use by primary care clinicians to ensure best practice management of patients tested for and diagnosed with chlamydia or gonorrhoea. METHODS The tool (a MedTech Advanced Form) was developed in consultation with seven primary care clinicians and included different tabs for u

A microsimulation model projecting the health care costs for resistance to antibacterial drugs in Sweden.

BACKGROUND: Previous studies have shown that increasing antibacterial resistance (ABR) globally will cause extensive morbidity, deaths and escalated health care costs. METHODS: To project economic consequences of resistance to antibacterial drugs for the Swedish health care sector, we used an individual-based microsimulation model, SESIM. Health care consumption was represented as increased numbers of hospital days, outpatient visits and contact tracing for individuals getting clinical infections or becoming asymptomatic carriers. The risk of contracting a resistant bacterium was calculated u…

Public Health Response to Imported Mumps Cases-Fort Campbell, Kentucky, 2018.

Mumps is an acute viral disease caused by a paramyxovirus that presents with fever and swelling of one or more of the salivary glands. Although not generally considered a disease of military importance, mumps has been associated with outbreaks among young adults in close living quarters, potentially placing Soldiers at risk for transmission of mumps when living in congregated settings. This article reports a recent public health response to 3 imported mumps cases occurring at Fort Campbell, Kentucky, that resulted in a contact investigation for 109 close contacts across varied settings. Committee opinion no 632: Expedited partner therapy in the management of gonorrhea and chlamydial infection.

Sexually transmitted infections (STIs) disproportionately affect women and create a preventable threat to their fertility. One factor that contributes to young women's high rates of STIs is reinfection from an untreated sexual partner. One way to address this problem is through expedited partner therapy, the practice of treating the sexual partners of patients in whom STIs are diagnosed. Expedited partner therapy enables the obstetrician-gynecologist or other provider to give prescriptions or medications to patients to take to their partners without first examining these partners. Despite the

Response to importation of a case of Ebola virus disease--Ohio, October 2014.

On September 30, 2014, the Texas Department of State Health Services reported a case of Ebola virus disease (Ebola) diagnosed in Dallas, Texas, and confirmed by CDC, the first case of Ebola diagnosed in the United States. The patient (patient 1) had traveled from Liberia, a country which, along with Sierra Leone and Guinea, is currently experiencing the largest recorded Ebola outbreak. A nurse (patient 2) who provided hospital bedside care to patient 1 in Texas visited an emergency department (ED) with fever and was diagnosed with laboratory-confirmed Ebola on October 11, and a second nurse (p

Empirical evidence for synchrony in the evolution of TB cases and HIV+ contacts among the San Francisco homeless.

The re-emergence of tuberculosis (TB) in the mid-1980s in many parts of the world, including the United States, is often attributed to the e … nsmission among the San Francisco homeless. The findings of this study underscore the importance of contact tracing in detection of HIV+ individuals that may otherwise remain undetected, and thus highlights the ever-increasing need for HIV-related data and an integrative approach to monitoring high-risk populations with respect to HIV and TB transmission.

Tuberculosis in a shopping centre, Portugal, 2004-5.

Genotyping enables to confirm or exclude a tuberculosis (TB) cluster. Excluding the link between cases is particularly important in countries with intermediate/high incidence of TB where the emergence of several TB cases in a particular location in space or time (higher than the expected) could be explained by chance alone. During 2004 and 2005, five TB cases occurred in five shops of a Portuguese shopping centre which employed a total of about 1000 workers. After an epidemiological survey, 52 close contacts were identified and screened. Latent tuberculosis infection was diagnosed in 10 contac

A cost-effectiveness evaluation of a jail-based chlamydia screening program for men and its impact on their partners in the community.

BACKGROUND: Few cost-effectiveness evaluations of screening men in jails for chlamydia have been published, and none have evaluated the cost-effectiveness of providing partner notification services to the partners of chlamydia-infected inmates. GOAL: The goal of this study was to evaluate the cost-effectiveness of the chlamydia screening and partner notification programs for men conducted by a Massachusetts jail compared with 3 hypothetical alternatives. STUDY DESIGN: Using jail cost and testing data, we used decision analyses to compare the cost and effectiveness of universal screening, age

Investigation of patient contacts of heath care workers with infectious tuberculosis: 6 cases in the Paris area.

SETTING: From June to October 2003, six cases of infectious tuberculosis were reported in health care workers (HCWs) working in hospitals in the Paris area. OBJECTIVE: To describe a method of investigation to identify tuberculosis infection in patient contacts. DESIGN: To define the cohort of patient contacts, the risk of contamination was evaluated based on 1) the degree of infectiousness of the HCW, 2) patient characteristics, and 3) the proximity and length of exposure. RESULTS: The number of patient contacts selected was very variable. A small proportion was investigated in consultation

Ring vaccination and smallpox control.

We present a stochastic model for the spread of smallpox after a small number of index cases are introduced into a susceptible population. The model describes a branching process for the spread of the infection and the effects of intervention measures. We discuss scenarios in which ring vaccination of direct contacts of infected persons is sufficient to contain an epidemic. Ring vaccination can be successful if infectious cases are rapidly diagnosed. However, because of the inherent stochastic nature of epidemic outbreaks, both the size and duration of contained outbreaks are highly variable.

Surfing with spirochaetes: an ongoing syphilis outbreak in Brighton.

BACKGROUND/OBJECTIVES: There has been a recent shift in the epidemiology of early syphilis in the developed world with sporadic outbreaks on a historic low level of background disease. Here we describe an ongoing outbreak of syphilis in Brighton. METHODS: Data collected on all patients with a diagnosis of early infectious syphilis at Brighton GUM clinic. RESULTS: 30 cases of early syphilis were diagnosed over a 25 month period beginning in July 1999. 28 were homosexual or bisexual men, giving a rate of 134 cases per 100 000 homosexual men. The cases reported a median of three sexual contacts

Mass treatment of humans exposed to rabies--New Hampshire, 1994.

On October 22, 1994, the laboratory of the New Hampshire Division of Public Health Services (NHDPHS) diagnosed rabies in a kitten that had been purchased from a pet store in Concord, New Hampshire. On October 19, the animal had developed seizures, then died of unknown causes during the night of October 20-21. Approximately 665 persons received rabies postexposure prophylaxis because of exposure to this kitten and other cats from the same pet store. This report summarizes the epidemiologic investigation of the source of the infection and follow-up care of humans and animals potentially exposed

How home anterior self-collected nasal swab simplifies SARS-CoV-2 testing: new surveillance horizons in public health and beyond.

The sample collection procedure for SARS-CoV-2 has a strong impact on diagnostic capability, contact tracing approach, ultimately affecting the infection containment performance. This study demonstrates that self-collected nasal-swab has shown to be a valid and well tolerated procedure to SARS-CoV-2 surveillance in a healthcare system. More significantly, no performance adequacy difference was detected in self-administered swabs between healthcare worker (HCW) and non-HCW which allows to speculate that this procedure could be successfully extended to the entire population for mass screening.
