Contraception and the etiology of pelvic inflammatory disease: new perspectives.

The dramatic increase in use of contraception worldwide makes it imperative to understand the effects of contraceptives on the health of women using them. In this article, we review the literature on the relationships of modern nonsurgical contraception with pelvic inflammatory disease. Subsequently, we identify areas where further research is needed to better define the risks and benefits of these contraceptive methods in various settings. From our review, two new conclusions emerge. First, our reanalysis of published data on the risk of PID associated with intrauterine device (IUD) use compa

Factors associated with willingness to use the contraceptive vaginal ring.

INTRODUCTION: The objective of this study was to examine factors associated with willingness to use the contraceptive vaginal ring among college students. MATERIAL AND METHODS: We electronically surveyed a random sample of female undergraduates. Factors associated with willingness to use the contraceptive vaginal ring were evaluated using a logistic regression model. RESULTS: Six hundred ninety-one women answered the survey. Willingness to use the contraceptive vaginal ring was positively associated with willingness to use the contraceptive patch, being employed at least 20 h/week, acceptabi

Pregnancy desire and dual method contraceptive use among people living with HIV attending clinical care in Kenya, Namibia and Tanzania.

AIM: To describe factors associated with pregnancy desire and dual method use among people living with HIV in clinical care in sub-Saharan Africa. DESIGN: Sexually active HIV-positive adults were enrolled in 18 HIV clinics in Kenya, Namibia and Tanzania. Demographic, clinical and reproductive health data were captured by interview and medical record abstraction. Correlates of desiring a pregnancy within the next 6 months, and dual method use [defined as consistent condom use together with a highly effective method of contraception (hormonal, intrauterine device (IUD), permanent)], among those

Epidemiological survey of vulvovaginal candidosis in Sfax, Tunisia.

Vulvovaginal candidosis (VVC) is a common infection of the female genital tract affecting 75% women at least once in their lifetime. The aim of this study was to determine the incidence and potential risk factors associated with VVC and recurrent vulvovaginal candidosis (RVVC). A prospective study of women with vaginitis symptoms was conducted over 2 years in the regional clinic of population and family education in Sfax. A discriminant analysis was used to evaluate the association between the incidence of Candida vaginitis and potential risk factors. Sporadic and recurrent VVC were documented

Contemporary hysteroscopic methods for female sterilization.

A permanent contraceptive method that avoids abdominal incisions and general anesthetic should be safer than sterilization by laparoscopy or laparotomy. In theory, the transcervical route ought to be ideal for female sterilization. However, past attempts have not seen widespread success, and contemporary efforts demonstrate that challenges to the creation of an ideal transcervical sterilization technique continue to exist. After 6 years of use, clinical data and real-world experience indicate that the Essure permanent birth control system is a viable option. Efficacy of 99.74% has been demonst

Integrating reproductive health services into HIV care: strategies for successful implementation in a low-resource HIV clinic in Lilongwe, Malawi.

BACKGROUND: Lighthouse Trust operates two public HIV testing, treatment and care clinics in Lilongwe, Malawi, caring for over 26 000 people living with HIV, 23 000 of whom are on antiretroviral treatment (ART). In August 2010, Lighthouse Trust piloted a step-wise integration of sexual and reproductive health (SRH) services into routine HIV care at its Lighthouse clinic site. The objectives were to increase uptake of family planning (FP), promote long-term reversible contraceptive methods, and increase access, screening and treatment for cervical cancer using visual inspection with acetic acid.

Risk factors for epithelial ovarian cancer in the female population of Belgrade, Serbia: a case-control study.

BACKGROUND/AIM: Ovarian cancer (OC) comprises 3% of all cancers, but it is the fifth most common cause of cancer death in women. The aim of this case-control study was to determine the risk factors for OC in the female population of Belgrade, Serbia. METHODS: A total of 80 consecutive patients were enrolled in the study between 2006 and 2008 in two national referral centers for OC in Serbia. The control subjects were recruited during the regular gynecological check-ups in the Public Health Center of the corresponding municipalities. All the study participants were interviewed during their vis

Hormonal intrauterine device use is related to mental rotations but not verbal memory skills.

There is perennial public and scientific interest in the neuroendocrine mechanisms underlying the large gender difference in three-dimensional (3D) mental rotation skills (favoring men) and small-to-moderate difference in verbal memory (favoring women). Past research has linked androgenic progestins in oral contraceptives to enhanced spatial skills, particularly 3D mental rotations, and estradiol to enhanced verbal memory skills. Hormonal intrauterine devices (IUDs) contain an androgenic progestin (i.e., levonorgestrel) without synthetic estradiol, but there is little-to-no systematic research

Contraceptive Progestogens and Incident Meningioma.

IMPORTANCE: Meningioma is a known adverse reaction of high-dose progestogens, but evidence regarding the risk associated with progestogens used as contraception is limited. OBJECTIVE: To examine whether different progestogens used as hormonal contraception are associated with increased risk of meningiomas. DESIGN, SETTING, AND PARTICIPANTS: This nested case-control study conducted over a 25-year study period from January 1, 2000, to December 31, 2024, is a Danish nationwide population-based register study, which included 3 million females aged 15 to 59 years with residence in Denmark. Sometimes You Do Get a Second Chance: Emergency Contraception for Adolescents.

Unplanned or unintended pregnancy remains a significant challenge for adolescents; many teens who plan ahead but opt not to choose long-acting reversible contraceptive methods have high failure rates with condom usage, oral contraceptives, and other less long-acting methods. Emergency contraception (EC) remains a necessity for those adolescents seeking a second chance to prevent the unintended consequences of unplanned sexual activity. At present, 5 postcoital methods remain available as EC globally: intrauterine devices, ulipristal acetate, a selective progesterone modulator, mifepristone; le

Migration of etonogestrel subcutaneous contraceptive implants: systematic review and recommendations for practice.

INTRODUCTION: Migration is a rare but serious complication of the etonogestrel contraceptive implant, and little is known about its extent. PURPOSE: To document and characterise cases of etonogestrel contraceptive implant migration in the scientific literature. METHODS: A systematic review of Medline, Embase and Global Health databases was carried out between January 2000 and January 2023 to identify articles presenting implant migrations. Narrative reviews, conference abstracts and articles not written in English or French were excluded. RESULTS: Forty-five articles, mostly published since

The Effect of Contraception on Genital Cytokines in Women Randomized to Copper Intrauterine Device, Depot Medroxyprogesterone Acetate, or Levonorgestrel Implant.

BACKGROUND: The ECHO trial randomized women to intramuscular depot medroxyprogesterone acetate (DMPA-IM), levonorgestrel implant (LNG-implant), or copper intrauterine device (Cu-IUD). In a substudy of the ECHO trial, we tested the hypothesis that contraceptives influence genital inflammation by comparing cervicovaginal cytokine changes following contraception initiation. In addition, we compared cytokine profiles in women who acquired HIV (cases) versus those remaining HIV negative (controls). METHODS: Women (n = 251) from South Africa and Kenya were included. Twenty-seven cervicovaginal cyto
