Analysis of risk factors for conversion in laparoscopic adrenalectomy: a single-institution series of 256 patients.

INTRODUCTION: Although introduction of the laparoscopic approach for adrenal gland surgery revolutionized the strategy in management of adrenal gland tumors, open surgery is still a method of choice in some clinical scenarios. Moreover, laparoscopy may have limitations resulting in conversion. AIM: To assess risk factors predisposing for conversion based on our own material, including 256 laparoscopic adrenalectomies performed in 2009-2016. MATERIAL AND METHODS: A retrospective analysis of patients who underwent adrenalectomies between 2009 and 2016 was conducted. Patients were analyzed for

Factors Affecting Early Cholecystectomy for Acute Cholecystitis in Older People-A Population-Based Study.

OBJECTIVES: Acute cholecystitis is one of the most common surgical presentations in Australia and increases with age. Guidelines recommend early laparoscopic cholecystectomy (within 7 days), as it results in shorter length of stay, reduced costs and readmission rates. Despite this, there is a perception that early cholecystectomy may result in higher morbidity and conversion to open surgery in older patients. Our objective is to report the proportion of early versus delayed cholecystectomy in older patients in New South Wales (NSW), Australia, and to compare health outcomes and factors influen…

Acute cholecystitis: early versus delayed cholecystectomy, a multicenter randomized trial (ACDC study, NCT00447304).

OBJECTIVE: Acute cholecystitis is a common disease, and laparoscopic surgery is the standard of care. BACKGROUND: Optimal timing of surgery for acute cholecystitis remains controversial: either early surgery shortly after hospital admission or delayed elective surgery after a conservative treatment with antibiotics. METHODS: The ACDC ("Acute Cholecystitis-early laparoscopic surgery versus antibiotic therapy and Delayed elective Cholecystectomy") study is a randomized, prospective, open-label, parallel group trial. Patients were randomly assigned to receive immediate surgery within 24 hours o

Could laparoscopic colon and rectal surgery become the standard of care? A review and experience with 750 procedures.

INTRODUCTION: The benefits of the laparoscopic approach to colon and rectal surgery do not seem as great as for other laparoscopic procedure … ocedures performed. Intraoperative complications occurred in 8.3%, with 29.0% of these resulting in conversion to open surgery. The overall rate of conversion to open surgery was 10.8%, most commonly for oncologic concerns. Median operating time was 175 minutes for all procedures. Postoperative complications occurred in 27.5% of procedures completed laparoscopically but wer …

Prior Hip Arthroscopy Increases Risk for Perioperative Total Hip Arthroplasty Complications: A Matched-Controlled Study.

BACKGROUND: Arthroscopic hip surgery is becoming increasingly popular for the treatment of femoroacetabular impingement and labral tears. Reports of outcomes of hip arthroscopy converted to total hip arthroplasty (THA) have been limited by small sample sizes. The purpose of this study was to investigate the impact of prior hip arthroscopy on THA complications. METHODS: We queried our institutional database from January 2005 and December 2017 and identified 95 hip arthroscopy conversion THAs. A control cohort of 95 primary THA patients was matched by age, gender, and American Society of Anesth

Uptake of robot-assisted colon cancer surgery in the Netherlands.

BACKGROUND: The robot-assisted approach is now often used for rectal cancer surgery, but its use in colon cancer surgery is less well defined. This study aims to compare the outcomes of robotic-assisted colon cancer surgery to conventional laparoscopy in the Netherlands. METHODS: Data on all patients who underwent surgery for colon cancer from 2018 to 2020 were collected from the Dutch Colorectal Audit. All complications, readmissions, and deaths within 90 days after surgery were recorded along with conversion rate, margin and harvested nodes. Groups were stratified according to the robot-ass

A risk-based framework to support conversion decision-making during minimally invasive left pancreatectomy: results from a multicentre registry.

Minimally invasive left pancreatectomy (MILP) is increasingly adopted worldwide, but conversion to open surgery still occurs and is associated with poorer postoperative outcomes. Tools to identify patients at increased risk of conversion are limited. This multicentre registry-based study included patients undergoing laparoscopic (LLP) or robot-assisted (RLP) MILP within the IGOMIPS registry. Predictors of conversion were analysed using multivariable logistic regression. A simple preoperative risk score, the modified Conversion Risk Score (mCRS), was derived from independent predictors of conve…

Short-term perioperative safety and long-term follow-up after robotic-assisted resection of non-renal, non-adrenal retroperitoneal tumors: a 10-year retrospective cohort study.

BACKGROUND: Open surgery remains the conventional approach for large retroperitoneal tumors because of limited operative space, complex vascular anatomy, potential bleeding risk, and oncological concerns. However, evidence regarding robotic-assisted laparoscopic resection for retroperitoneal tumors larger than 10 cm remains limited. This study aimed to evaluate the feasibility, safety, perioperative outcomes, and follow-up results of robotic-assisted laparoscopic resection for non-renal and non-adrenal retroperitoneal tumors, with particular focus on tumors larger than 10 cm. METHODS: We retr

Laparoscopic Hartmann reversal after multiple open surgeries: surgical technique.

Hartmann's reversal following prior multiple laparotomies represents on of the most technically challenging procedures in colorectal surgery. Dense adhesions, altered anatomy, and impaired tissue planes increase the risks of enterotomies, anastomotic complications, and conversion to open surgery. Although minimally invasive techniques offer improved postoperative recovery, their role in complex reoperative settings remains limited, prefering an open approach. This article presents a structured laparoscopic approach to enhance safety and reproducibility. A standardized laparoscopic Hartmann rev…

[Lower mid-line skin incision and minimal sternotomy in pediatric open heart surgery--an alternative cosmetic approach].

Full median sternotomy has been the standard approach for open heart surgery. However, it leaves an unsightly surgical scar. We therefore designed a lower mid-line skin incision and minimal sternotomy approach as a cosmetic alternative. Since February 1993, 78 pediatric patients with congenital heart disease have undergone this open heart surgery procedure. Their ages ranged from 1 month to 13 years (median: 2 years and 7 months) and body weights ranged from 2.4 to 43 kg (median: 11.5 kg). Thirty-one patients had atrial septal defect, 34 perimembranous ventricular septal defect, 9 subpulmonary

The utilization of fluorescent cholangiography during robotic cholecystectomy at an inner-city academic medical center.

In recent years, fluorescent cholangiography using Indocyanine green (ICG) dye has been used to aid identification of structures during robotic cholecystectomy. We sought to compare cholecystectomy with ICG dye versus laparoscopic cholecystectomy at an inner-city academic medical center. Between January 2013 and July 2016, we identified 287 patients of which 191 patients underwent laparoscopic cholecystectomy and 96 patients underwent robotic cholecystectomy with ICG dye. Preoperative risk variables of interest included age, sex, race, body mass index (BMI), and acute cholecystitis. Primary ou

Preoperative high C-reactive protein level is associated with an increased likelihood for conversion from laparoscopic to open appendectomy in patients with acute appendicitis.

Background: Laparoscopic appendectomy (LA) has been increasingly adopted for its advantages over the open appendectomy, but there are possibilities of conversion from laparoscopic to open appendectomy (CA) if the patients had complicated appendicitis concurrently, or when the extent of inflammation prohibits successful procedure. In this retrospective study, we aimed to clarify the preoperative predictors for CA. Patients and methods: From January 2010 to April 2016, medical records of 93 consecutive patients who underwent LA for suspected appendicitis were reviewed retrospectively. Factors ev
