Histomorphometric and biomechanical analyses of the osseointegration of loaded orthodontic microscrews inserted at different cortical bone thickness sites.

OBJECTIVES: The aim of this study was to evaluate the osseointegration of loaded orthodontic microscrews at different cortical bone thickness (CBT) sites. STUDY DESIGN: Forty-eight microscrews were inserted bilaterally in the tibias of 12 beagles, and divided into thick and thin CBT group. After 1, 3, 5, and 7 weeks, the dogs were killed and bone-screw specimens prepared for polyfluorochrome sequential labeling, microscopic computerized tomography (μCT) analysis, and biomechanical pull-out testing. RESULTS: All μCT parameters were increased in the thin CBT group, and decreased in the thick C…

Thickness of buccal bone in the mandible and its clinical significance in mono-cortical screws placement. A CBCT analysis.

The aim of this study was to determine the safest length of monocortical screws that can be inserted for the treatment of mandibular fractures following Champy's technique. Fifty cone-beam computed tomography (CBCT) scans of hemi-mandibles were studied. Linear measurements were taken from the buccal cortical plate to the tooth apex, from the canine to the second premolar, and from the buccal cortical plate to the tooth apex and the inferior alveolar canal in the molar area. The minimum values of the horizontal distances both at the level of the apex and the inferior alveolar canal at the secon

Effects of lifetime loading history on cortical bone density and its distribution in middle-aged and older men.

We have reported previously that long-term participation of weight-bearing exercise is associated with increased QCT-derived cortical bone size and strength in middle-aged and older men, but not whole bone cortical volumetric BMD. However, since bone remodeling and the distribution of loading-induced strains within cortical bone are non-uniform, the aim of this study was to examine the effects of lifetime loading history on cortical bone mass distribution and bone shape in healthy community dwelling middle-aged and older men. We used QCT to assess mid-femur and mid-tibia angular bone mass dist…

Effect of autogenous cortical bone particulate in conjunction with enamel matrix derivative in the treatment of periodontal intraosseous defects.

BACKGROUND: The aim of the present study was to assess the additional clinical benefit of autogenous cortical bone particulate (ACBP) when added to enamel matrix derivative (EMD), compared to EMD alone, in the treatment of deep periodontal intraosseous defects. METHODS: A total of 28 intraosseous lesions in 27 patients with advanced periodontitis were included in this controlled clinical trial and randomly assigned to the EMD group (14 defects) or to the EMD + ACBP group (14 defects). Immediately before surgery (baseline) and after 6 and 12 months, probing depth (PD), clinical attachment leve…

[Experimental and clinical substantiation of allogenic cortical bone matrix use for treatment of patients with habitual mandibular dislocation].

Cortical bone matrix (CBM) was fixed under the zygomatic arch of 20 experimental rabbits by the titanium wire. CBM transformed into the organotypic bone tissue during 90-180 days, and the transformation processes were more significant if the cortical layer of the inferior border of the zygomatic arch had been removed and CBM had been miotisized. Taking into account good results of the experiment, CBM was applied in clinic for treatment of habitual mandibular dislocation (HMD). The new operative approach to the TMJ and method for HMD using CBM was developed. It is easy to perform, less traumati…

Exploration of the covariation signal between cortical bone and dentine volumes across the upper limb bones and anterior teeth in modern humans and relevance to evolutionary anthropology.

Cortical bone and dentine are two mineralized tissues sharing a common embryological origin, developmental, and genetic background, distinct from those of enamel. Understanding their relationship is crucial to decipher the factors acting on their postnatal development, and shedding light on the evolutionary patterns of tissue proportions. Here, we investigate the coordinated variation between cortical bone and dentine volumes measured from arm and forearm bones (humeri, ulnae, radii) and upper anterior teeth (central incisors, lateral incisors, canines) of modern humans. Given the shared chara…

Modeling-based bone formation transforms trabeculae to cortical bone in the sclerotic areas in Buschke-Ollendorff syndrome. A case study of two females with LEMD3 variants.

Buschke-Ollendorff syndrome is a rare autosomal dominant condition caused by pathogenic variants in LEMD3 and characterized by connective tissue nevi and sclerotic bone abnormalities known as osteopoikilosis. The bone phenotype in Buschke-Ollendorff syndrome including osteopoikilosis remains unclear. We investigated bone turnover markers, pelvis and crura X-rays; lumbar spine and femoral neck DXA; bone activity by NaF-PET/CT, bone structure by μCT and dynamic histomorphometry in adults with Buschke-Ollendorff syndrome. Two women aged 25 and 47 years with a BMI of 30 and 32 kg/m2, respectively,

Mitochondrial Point Mutation m.3243A>G Associates With Lower Bone Mineral Density, Thinner Cortices, and Reduced Bone Strength: A Case-Control Study.

Mitochondrial dysfunction is associated with several clinical manifestations including diabetes mellitus (DM), neurological disorders, renal and hepatic diseases, and myopathy. Although mitochondrial dysfunction is associated with increased bone resorption and decreased bone formation in mouse models, effects of alterations in mitochondrial function on bone remodeling and mass have not been investigated in humans. We recruited 45 carriers (29 females, 16 males) with the m.3243A>G mutation and healthy controls matched for gender, age, height, and menopausal status. DXA and HRpQCT scans were per

Primary cortical neurons precipitate and extrude large mitochondria-associated calcium-phosphate sheets with a bone-precursor-like ultrastructure.

Calcium-phosphate (CaP) is a ubiquitous inorganic compound that plays an important structural role in healthy bone and teeth formation, but its pathologic buildup can occur in dyshomeostatic calcium disorders like Alzheimer's disease and Leigh syndrome. The nexus of pathologic extracellular CaP in the nervous system is not well understood, but prior evidence suggests mitochondria could be a source. We have observed mitochondria-sized sheet-like CaP aggregates within functional wild type cortical neuron cultures at 1 and 20 days in vitro. Neurons were extracted from embryonic day 18 (E18) rat e

Biomechanical impact of cortical bone vs. traditional pedicle screw trajectories: a finite element study on lumbar spinal instrumentation.

BACKGROUND: Pedicle screw fixation using the cortical bone trajectory (CBT) enhances stability by engaging cortical bone, offering a valuable alternative to the traditional pedicle screw trajectory (TT). This study used finite element analysis to compare L4-5 instrumentation with CBT and TT screws, investigating whether the increased cortical bone engagement in CBT improves stability but makes it more susceptible to fatigue failure. METHODS: A L3-sacrum model was generated using anonymized CT patient data, validated against existing studies, showing consistent ROM (range of motion) values. A …

Enabling Technology with Minimally Invasive Strategies: Robot-Assisted Cortical Bone Trajectory Screw Fixation in Lateral Transpsoas Interbody Fusion.

OBJECTIVE: Robot-assisted cortical bone trajectory (CBT) screw placement is safer than the traditional fluoroscopy-assisted approach. This technical note reports a novel technique of robot-assisted CBT screw placement using a subfascial transmuscular approach. METHODS: This technique involves two stages: first, the positioning of a lumbar interbody cage through a lateral transpsoas approach, and then, the robot-assisted placement of CBT screws. The second step is performed with the patient in prone position with navigation frames (ICT, DRB, and surveillance-marker) registered to the robotic s…

PDGF-BB improves cortical bone quality through restoring the osteogenic microenvironment in the steroid-associated osteonecrosis of rabbits.

OBJECTIVE: Steroid-associated osteonecrosis of the femoral head (SONFH) is a refractory disease characterized by progressive bone destruction. Clinical evidence suggests that SONFH may extend beyond the intra-capital region to the femoral neck, metaphysis, and even diaphysis, increasing the risk of subtrochanteric fractures and implant loosening post-surgery. While our previous study demonstrated that platelet-derived growth factor-BB (PDGF-BB) promotes reparative osteogenesis in the femoral head, its effects on cortical bone quality in the extended diaphyseal regions under steroid-associated …
