Stent graft repair of descending aortic dissection in patients with Marfan syndrome: an effective alternative to open reoperation?

OBJECTIVE: Aneurysms or dissections can involve multiple aortic segments in patients with Marfan syndrome, requiring staged replacement of the entire aorta. The optimal treatment of descending aortic dissection in these patients is a major challenge. We investigated the feasibility and outcomes of endovascular repair of the descending aorta in patients already submitted to open aortic root/arch surgery. METHODS: From March 1998 to July 2008, 12 patients (10 male and 2 female patients; mean age, 37.8 +/- 11.6 years) affected by Marfan syndrome underwent endovascular treatment for dissection of

[Endovascular treatment of descending aortic dissection (type B)].

Aortic dissection is one of the most serious aortic diseases by its potential for rupture, but also for other complications, such as cerebral or splanchnic ischemia, which may be fatal. If open surgery is the rule for lesions of the ascending aorta (type A), type B (not concerning the ascending aorta) is first a matter of medical treatment except when complications are present. In this case the placement of a stentgraft is a valuable alternative to open surgery. We report a patient presenting with a type B aortic dissection, characterized by rapid expansion and complicated by peri-aortic leaka

Endovascular therapy of ascending thoracic aorta.

We describe the endovascular treatment given to a patient who, after surgery on the ascending thoracic aorta due to acute type A dissection (Bentall's procedure), developed a pseudoaneurysm 12 cm in diameter of the ascending aorta at the brachiocephalic trunk. Firstly, an extra-anatomical carotid-carotid bypass was performed, which was followed by endovascular treatment, excluding the aortic arch from the origin of both coronary arteries to the origin of the left common carotid artery, occluding the brachiocephalic trunk. For accurate placement of the endoprosthesis, the device was released af

Biomechanical Analysis of Cadaveric Thoracic Aorta Zones: The Isthmus is the Weakest Region.

BACKGROUND: The thoracic aorta is a site of multiple pathological processes, such as aneurysms and dissections. When considering the development of endovascular devices, this vessel has been extensively manipulated because of aortic diseases, as well as to serve as a route for procedures involving the head and neck vessels. Therefore, the aim of the present study was to obtain biomechanical experimental information about the strength and deformability of this vessel. MATERIALS AND METHODS: Thirty-one thoracic aorta specimens were harvested during the autopsy procedure. They were carefully dis

Intravascular Ultrasound-Assisted Endovascular Treatment of Mesenteric Malperfusion in a Multichannel Aortic Dissection With Full True Lumen Collapse.

PURPOSE: To describe endovascular treatment of mesenteric malperfusion in a multichannel aortic dissection (MCAD) with full true lumen (TL) collapse following thoracic endovascular aortic repair (TEVAR). CASE REPORT: A 54-year-old man presented with chronic mesenteric ischemia and a previous TEVAR for MCAD complicated by superior mesenteric artery (SMA) malperfusion. Computed tomography angiography (CTA) demonstrated a 3-channel aortic dissection with a "false-true-false" configuration. The SMA was malperfused through the collapsed TL. CTA also showed a secondary entry tear, measuring 18 mm i

Multi-Branched Thoracoabdominal Endovascular Aortic Repair With Branch Access Through a Left Thoracotomy and Descending Thoracic Aortic Conduit: A Novel and Effective Alternative Access.

PURPOSE: To demonstrate an alternative access to perform directional branch catheterization during complex endovascular aortic repair. TECHNIQUE: Urgent endovascular aortic repair was indicated to treat a symptomatic post dissection thoracoabdominal aneurysm with large infrarenal dilatation with an off-the-shelf t-Branch endograft (Cook Medical, Bloomington, IN, USA). Traditional proximal arterial accesses were not suitable due to a previous aortic arch endograft. A novel approach was performed through a left postero-lateral thoracotomy, isolation of the descending thoracic aorta and anastomo

[Lower body ischemia due to bending of the stent after hybrid treatment for chronic stanford type B aortic dissection].

Lower body ischemia due to bending of a stented graft at the thoracic aorta was rare, particularly when it occurred in several days after surgery. We experienced this complication and performed the 3rd-time thoracic endovascular repair( TEVAR).A 49-year-old man with a chronic aortic dissection of Stanford type B underwent TEVER;however we failed to close the entry because of the tortuously bended distal arch of the aorta. Then it was decided the patient undergo a hybrid treatment with arch replacement and frozen elephant trunk. Seventeen days after the surgery, the blood pressure of the patien

Extensive replacement of the aorta from the ascending aorta to the supraceliac abdominal aorta during a one-stage operation.

A successful replacement of the aorta from the ascending aorta to the supraceliac abdominal aorta in one operation is herein reported. The patient was 66-year-old woman who had DeBakey type I chronic aortic dissection with a pre-disseminated intravascular coagulation (DIC) condition. The image diagnosis revealed a markedly enlarged false lumen with a narrow true lumen in the ascending aorta extending to the diaphragm level while below the diaphragm level, the aortic dilatation was mild. The operation was performed through both a thoracoabdominal incision and a median sternotomy to expose the w

Different clinical course of ascending aortic aneurysm in monozygotic twins.

A 45-year-old woman without medical history of cardiovascular disease came to the clinic. She was diagnosed as annuloectasia and aneurysm of ascending aorta and dissected aorta from above the aortic valve to renal artery level (type A). There are no laboratory abnormalities including CBC, biochemistry, and serology tests etc. She does not have any Marfanoid features. Surprisingly, when we reviewed her family history, we realized her monozygotic twin sister performed Bentall operation because of grade III/IV of aortic regurgitation due to annuloectasia and aneurysm of ascending aorta in our hos

Surgical repair of thoracic aortic aneurysms: results and complications.

Between January 1990 and February 2001 a total of 323 patients underwent following operations at our Institution: respectively 256 on the ascending aorta, 13 on the transverse arch and 54 on thoracic descending aorta. Sixteen patients with thoracic aortic aneurysms underwent endovascular stent graft implantation. The overall in-hospital mortality was respectively: 7% for ascending aortic aneurysms, 7.7% in the aortic arch aneurysms group and 5.5% for descending aortic aneurysms. The mortality was greater in case of emergency surgery and in the subgroup of patients with acute type A dissection.

Analysis of a streamlined pathway for aortic surveillance for Turner syndrome in a single centre.

BACKGROUND AND OBJECTIVE: The risk of aortic dissection (AoD) is increased in women with Turner syndrome (TS) but predicting those with this heightened risk is difficult. In response to this, we sought to create a pathway to monitor TS patients to improve efficiency and resource utilisation in our dedicated TS clinic, and to monitor more closely those women thought to be at increased risk of AoD. DESIGN AND PATIENTS: Our pathway was designed based on evidence derived from International Guidelines for the management of aortic disease in women with TS. Women were divided according to those with

An Asymptomatic Case of Type B Aortic Dissection Developing Type A Dissection During Interfacility Transfer: A Case Report.

During the transfer from a hospital to a tertiary hospital, patients with acute aortic dissection can deteriorate due to aortic rupture or extension of the dissection. Particularly when type B aortic dissection progresses to type A dissection, it can lead to serious complications. This case report that a 50-year-old man who was transferred to our institute for thoracic endovascular aortic repair for complicated acute type B aortic dissection developed retrograde type A aortic dissection and was successfully treated with total arch replacement using a frozen elephant trunk. The patient experien

Stenotic and obstructive lesions in acute dissecting thoracic aortic aneurysms.

The present study of 33 operatively treated patients, 88 per cent of whom survived the procedure, is concerned with an important problem associated with acute thoracic aortic dissection, the stenotic and obstructive lesions of the aorta and its branches. Their variety and nature are described, as are the additional operative procedures deemed necessary at the time of the operation, immediately thereafter, or later on. Much has been learned about these difficulties from clinical and autopsy observations and especially from careful arteriographic surveys. They seem to be generally well withstood

In Situ Laser Fenestration for Subclavian Preservation During Aortic Dissection Repair.

Laser fenestration is a novel technique to create an opening in an endograft to maintain perfusion to the arch vessels during thoracic endovascular aortic repair (TEVAR). A 40-year-old female who had undergone an emergent ascending aortic repair for acute dissection presented with aneurysmal degeneration of the descending thoracic aorta. She then underwent debranching of the left carotid and innominate arteries, followed by TEVAR. Herein, we describe our approach during repair with in situ laser fenestration of the endograft to preserve subclavian arterial flow.

Chronic dissecting aneurysm of ascending aorta with a large intramural thrombus and isolated aortic defects.

We present macroscopic and microscopic findings in a case of chronic dissecting aneurysm of ascending aorta and aortic arch associated with isolated tears of aortic wall without its rupture in a 71-year-old female presenting with minimal clinical symptomatology. Aneurysmal dilation of the aorta was caused not only by the bridging of the vascular wall based on the presence of an organizing intramural thrombus in the false lumen between the separated layers, but also by a wide flat defect in the aortic intima and media with the preservation of the aortic wall integrity due to fibrotical alterati

Dysphagia aortica: a rare cause of dysphagia.

Dysphagia is uncommon; however, it is considered a warning symptom, especially in the elderly population, and should prompt consideration of a neoplasm of the oesophagus. Dysphagia aortica is rare and often not considered in clinical practice. It is caused by any abnormality of the aorta that results in dysphagia. These include thoracic aortic aneurysm, aortic dissection and even a tortuous aorta. Our case is of an elderly woman, in her early 70s, with a history of treated colorectal carcinoma and hypertension, who presented with progressive dysphagia to solid foods, which, on investigations,

Small GTP-Binding Protein GDP Dissociation Stimulator Prevents Thoracic Aortic Aneurysm Formation and Rupture by Phenotypic Preservation of Aortic Smooth Muscle Cells.

BACKGROUND: Thoracic aortic aneurysm (TAA) and dissection are fatal diseases that cause aortic rupture and sudden death. The small GTP-binding protein GDP dissociation stimulator (SmgGDS) is a crucial mediator of the pleiotropic effects of statins. Previous studies revealed that reduced force generation in aortic smooth muscle cells (AoSMCs) causes TAA and thoracic aortic dissection. METHODS: To examine the role of SmgGDS in TAA formation, we used an angiotensin II (1000 ng·min-1·kg-1, 4 weeks)-induced TAA model. RESULTS: We found that 33% of Apoe-/- SmgGDS+/- mice died suddenly as a result

LOX Mutations Predispose to Thoracic Aortic Aneurysms and Dissections.

RATIONALE: Mutations in several genes have been identified that are responsible for 25% of families with familial thoracic aortic aneurysms and dissections. However, the causative gene remains unknown in 75% of families. OBJECTIVES: To identify the causative mutation in families with autosomal dominant inheritance of thoracic aortic aneurysms and dissections. METHODS AND RESULTS: Exome sequencing was used to identify the mutation responsible for a large family with thoracic aortic aneurysms and dissections. A heterozygous rare variant, c.839G>T (p.Ser280Arg), was identified in LOX, encoding
