Global reemergence of tuberculosis: are host defense peptides an option to ameliorate disease burden?

Tuberculosis is the most relevant infectious disease worldwide according to the estimates of the World Health Organization, and despite being a curable disease, it requires a 6-9-month therapy with multiple antibiotics. Intermittent drug therapy due to noncompliance or poor delivery of therapy promotes the emergence of bacterial strains showing resistance to multiple drugs and the rise of extremely drug-resistant strains. Moreover, increased antibiotic resistance has been observed for several microorganisms, including extremely drug-resistant tuberculosis, vancomycin-resistant Enterococcus fae

Genetic and genomic architecture of the evolution of resistance to antifungal drug combinations.

The evolution of drug resistance in fungal pathogens compromises the efficacy of the limited number of antifungal drugs. Drug combinations have emerged as a powerful strategy to enhance antifungal efficacy and abrogate drug resistance, but the impact on the evolution of drug resistance remains largely unexplored. Targeting the molecular chaperone Hsp90 or its downstream effector, the protein phosphatase calcineurin, abrogates resistance to the most widely deployed antifungals, the azoles, which inhibit ergosterol biosynthesis. Here, we evolved experimental populations of the model yeast Saccha

Unusual accumulation of a wide array of antimicrobial resistance mechanisms in a patient with cytomegalovirus-associated hemophagocytic lymphohistiocytosis: a case report.

BACKGROUND: Infections with multidrug-resistant organisms (MDRO) pose a serious threat to patients with dysregulated immunity such as in hemophagocytic lymphohistiocytosis (HLH), but such infections have rarely been comprehensively characterized. Here, we present a fatal case of HLH secondary to cytomegalovirus (CMV) infection complicated by both anti-viral drug resistance and sepsis from multiple MDROs including pandrug-resistant superbug bacteria. CASE PRESENTATION: A previously healthy, six-year-old boy presented with a 45-day history of fever prior to a diagnosis of hemophagocytic lymphoh

Mutation-driven alterations in the AcrB efflux pump modulate antibiotic resistance in Klebsiella pneumoniae.

OBJECTIVES: To investigate the global prevalence of the acrB efflux transporter among antibiotic resistance genes (ARGs) in Klebsiella pneumoniae and to determine how mutations within AcrB drug-binding pockets influence structural stability and antibiotic interactions. METHODS: A global genomic analysis of 42 ARGs reported in K. pneumoniae isolates worldwide was performed to identify the prominence of acrB. A total of 92 AcrB protein sequences were assessed for drug-binding pocket mutations relative to the wild-type strain NCTC9637. De novo protein structures were generated using AlphaFold2 a

Advances in carbohydrate polymer-based stimuli-responsive nanocarriers: precision strategies for overcoming multidrug resistance in targeted cancer therapy.

Multidrug resistance (MDR) remains a formidable challenge in cancer therapy, often rendering conventional chemotherapeutics ineffective. Recent advances in carbohydrate polymer-based stimuli-responsive nanocarriers have opened new avenues for overcoming MDR while achieving targeted drug delivery. This narrative review highlights the unique properties of carbohydrate polymers, such as biocompatibility, biodegradability, and functional versatility, that make them ideal candidates for designing smart nanocarriers. We discuss molecular engineering strategies to develop stimuli-responsive systems c

Efflux Pump Inhibitors: Revisiting an Old Strategy in the Era of Antimicrobial Resistance.

Antimicrobial resistance (AMR) is a growing global threat that undermines the effectiveness of existing antibiotics and complicates the treatment of infectious diseases. Among the multiple mechanisms driving resistance, bacterial efflux pumps play a critical but often underappreciated role by actively expelling antibiotics from the cell and reducing intracellular drug concentrations. These pumps are widely distributed across gram-positive and gram-negative bacteria and contribute to multidrug resistance against several major antibiotic classes. Importantly, efflux-mediated resistance is freque

Plasmid-mediated antimicrobial resistance in non-typhoidal Salmonella: serotype-specific mechanisms and ecological implications.

Non-typhoidal Salmonella (NTS) is a leading cause of foodborne illness, with multidrug-resistant (MDR) strains challenging treatment and food safety. Serotype-specific plasmid associations underlie distinct antimicrobial resistance (AMR) risks: IncHI2 plasmids in S. Typhimurium, virulence-plasmid exclusion in S. Enteritidis, pESI megaplasmids in S. Infantis, and multi-plasmid carriage in S. Indiana. These profiles shape persistence in livestock, processing, and retail settings, raising the likelihood of resistance spread along the farm-to-fork continuum. Plasmid interactions, including helper-

Combinations of colistin and bacteriocins prevent the selection of colistin resistance in Acinetobacter baumannii.

Acinetobacter baumannii is a multidrug-resistant nosocomial pathogen responsible for infections that are often difficult to treat. Here, we show that exposure of A. baumannii to the last-resort antibiotic colistin, which disrupts the outer membrane of Gram-negative bacteria, results in inner membrane permeabilization and depolarization, ultimately inhibiting ATP synthesis. Nevertheless, under these conditions, colistin-resistant mutants are rapidly and frequently selected. In addition, A. baumannii is able to tolerate colistin, most likely due membrane depolarization and ATP depletion, which a

An update on Escalating Drug Resistance among Uropathogenic Bacteria: Challenges in Treatment and Scope for Maneuver.

Uropathogens are among the most common infectious agents in humans worldwide. The problem is further exacerbated by the escalating emergence and dissemination of antibiotic resistance genes (ARGs) among bacteria causing urinary tract infections (UTIs). The World Health Organization (WHO) has identified antibiotic resistance as the most serious public health threat of this century, demanding global action. The most common bacteria contributing approximately 85% of UTIs include Escherichia coli, Klebsiella pneumoniae, and Enterococcus spp. Reports of intrinsic and/ or acquired resistance mechani

The emerging pathogen Candida auris: host interactions and disease drivers.

Candida auris has emerged over the past fifteen years as a multidrug-resistant human fungal pathogen responsible for hospital-acquired infections associated with high mortality rates. Clinical isolates frequently exhibit resistance to one or more first-line antifungal drugs, and management is further complicated by persistent colonization, biofilm formation, and historical misidentification in diagnostic laboratories. Rapid species-level identification and accurate antifungal susceptibility testing are critical for effective patient care and infection control. In this article, we summarize the

Comprehensive Review of Candidozyma (Candida) auris Management: Insights From the Society of Infectious Diseases Pharmacists.

Candidozyma (Candida) auris has emerged over the past two decades as a formidable global health threat due to its multidrug resistance, persistence in healthcare environments, and rapid nosocomial spread. Recently reclassified into the genus Candidozyma based on phylogenomic analysis, C. auris poses major challenges for both clinical management and infection control. Its ability to tolerate heat, salinity, and disinfectants supports long-term survival on surfaces and medical devices, facilitating transmission. Biofilm formation further enhances virulence and resistance to antifungal therapy. C

From pumps to networks: ABC transporters in leukemia resistance and microenvironmental adaptation.

ATP-binding cassette (ABC) transporters have long been recognized as critical mediators of multidrug resistance (MDR) in leukemia; however, emerging evidence reframes them as adaptive regulators within an integrated resistance network rather than static efflux pumps. This review consolidates recent mechanistic, functional, and translational insights into the role of key transporters, ABCB1 (P-gp), ABCC1 (MRP1), and ABCG2 (BCRP), in drug resistance and microenvironmental adaptation. We describe how stress-responsive transcriptional programs (ATF4/JUN), stromal kinase signaling (PKC-NF-κB), and

A review of the human microRNA and the Mycobacterium tuberculosis epigenetic effects on the emergence drug resistance.

AIMS: Mycobacterium tuberculosis (MTB) uses epigenetics to avoid the hostile host immune defence systems and also mount resistance to chemotherapy when exposed to antibiotic stress. MTB's epigenetic survival tool-kit includes genomic DNA histone acetylation/deacetylation, methylation, phosphorylation, ubiquitylation, etc. The non-coding host microRNAs (miRNAs) as genomic products of epigenetic control of drug extrusion processes, drug permeability barrier formation or metabolism, and target alteration are hijacked by MTB to mount multi-drug resistance. The miRNAs involved and the mechanisms us

Phage therapy: A novel approach to combat drug-resistant pathogens.

Antibiotic-resistant infections, such as those caused by the overuse of antibiotics, have greatly strained healthcare systems. Among them, drug-resistant bacteria ESKAPE (Enterococcus faecium, Staphylococcus aureus, Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa, and Enterobacter species) are typical and common. Enterococcus faecalis and Escherichia coli are of equal concern. These pathogens often have higher pathogenicity than the same strains, and resistance has reduced treatment options, so new treatment options are needed to address these pathogens. This review anal

Molecular mechanisms of heavy metal resistance and cross-/co-resistance to antibiotics in Pseudomonas aeruginosa.

Heavy metal pollution is a growing environmental and public health concern, particularly due to its impact on microbial communities. Pseudomonas aeruginosa, a highly adaptable bacterium, has developed resistance to heavy metals (HMs), which is closely linked to antibiotic resistance through shared genetic and regulatory pathways. This co-resistance poses significant challenges for environmental health and antimicrobial management. Additionally, microplastics act as carriers for HMs and antibiotics, creating a compounded pollution stressor that further influences bacterial resistance patterns.

Recent insights into actinobacteria research in antimicrobial resistance: a review.

Antimicrobial resistance (AMR) has emerged as a global health crisis, taking 4.71 million lives in the year 2021 and posing significant challenges to healthcare systems. Actinobacteria, particularly Streptomyces sp., are a well-established source of bioactive secondary metabolites, including antibiotics such as polyketides, aminoglycosides, and macrolides with activity against multidrug-resistant (MDR) bacteria. However, only 10% of the antibiotic genes are expressed, and others are silent in cryptic biosynthetic gene clusters (BGCs) that remain inactive under standard laboratory conditions. A

Andrographolide reverts multidrug resistance in KBChR 8-5 cells through AKT signaling pathway.

Multidrug resistance (MDR) is a major obstacle in cancer chemotherapy. P-glycoprotein (P-gp) one of the ATP-binding cassette (ABC) transporters plays an important role in MDR. In this study, we examined the sensitizing property of andrographolide (Andro) to reverse MDR in the drug-resistant KBChR 8-5 cells. Andro exhibited increased cytotoxicity in a concentration-dependent manner in the P-gp overexpressing KBChR 8-5 cells. Furthermore, Andro showed synergistic interactions with PTX and DOX in this drug-resistant cells. Andro co-administration enhanced PTX- and DOX-induced cytotoxicity and red
