Platinum chemotherapy, radiotherapy and the inner ear: implications for "standard" radiation portals.

In this centre, platinum chemotherapy precedes radiotherapy in several primary malignancies arising in structures adjacent to the mastoid bone. The augmented ototoxicity of platinum and radiotherapy to fields encompassing the inner ear is exemplified by a case report. The order of administration may not be critical. This important toxicity, particularly for young patients, may be reduced by careful shielding considerations during planning of radiation portals, as exemplified in two further cases, and by carboplatin chemotherapy.

Noise-induced reduction of inner-ear microphonic response: dependence on body temperature.

The rate of reduction of chinchilla cochlear microphonic response with exposure to steady noise is less at lower body temperatures and greater at higher body temperatures. Before exposure to noise, this auditory response is invariant within the range of temperatures employed. The mechanism of reduction of cochlear response appears to involve processes sensitive to body temperature.

The development of inner ear gene therapy for DFNB9: From bench to bedside.

Hearing loss is a significant health problem facing the world's population today. Approximately 3 in 1000 children born in the US are found to have hereditary hearing loss. Currently, hearing aids and cochlear implants represent the main treatment options for these patients. The field of gene replacement therapy as a treatment for hereditary hearing loss has advanced rapidly. There are now five ongoing clinical trials using gene replacement therapy to target non-syndromic autosomal recessive hearing loss DFNB9. The preliminary results from these studies are truly exciting. In this review, we s

Interactive mixed reality anatomy learning of normal and malformed inner ears.

The objective of this study is to evaluate the effectiveness of interactive Mixed Reality (MR) learning and compare it to traditional methods for independently learning inner ear anatomy through two experiments. The evaluation was conducted using HoloInnerEar, our new interactive MR tool for inner ear anatomy learning. In the first controlled experiment (N = 32), we compared independent learning techniques for normal anatomy using the interactive 3D + 2D features of HoloInnerEar to the traditional method using a video course combined with a mobile medical imaging application. Results show that

Embryonic Development of the Inner Ear of the Catshark Scyliorhinus canicula.

INTRODUCTION: The inner ear is a complex three-dimensional structure responsible for the detection of sound, balance, and acceleration. Detailed knowledge about the development of the inner ear of gnathostomes (jawed vertebrates) comes from studies in bony fishes and tetrapods, but comparable information about this process in chondrichthyans, the oldest gnathostome radiation, is lacking. This study describes for the first time the embryonic development of the inner ear and its innervation in the catshark Scyliorhinus canicula. METHODS: By using molecular markers of proliferating cells, migrat

Development and Evaluation of a Novel Transcanal Catheter for Delivery of Hypothermia to the Inner Ear.

Mild therapeutic hypothermia (MTH) has demonstrated neuroprotective effects in the cochlea, particularly against noise-induced and electrode insertion trauma, by reducing inflammation and oxidative stress. Prior cadaveric studies have shown that localized cochlear cooling can be achieved using a probe placed on the promontory or a surface cooling device placed on the mastoid. While the effects of MTH on the vestibular system remain unstudied, its proximity and physiological similarity to the cochlea suggest potential benefits. We aimed to develop the first noninvasive, localized MTH device for

Interpreting the Evolutionary Echoes of a Protein Complex Essential for Inner-Ear Mechanosensation.

The sensory epithelium of the inner ear, found in all extant lineages of vertebrates, has been subjected to over 500 million years of evolution, resulting in the complex inner ear of modern vertebrates. Inner-ear adaptations are as diverse as the species in which they are found, and such unique anatomical variations have been well studied. However, the evolutionary details of the molecular machinery that is required for hearing are less well known. Two molecules that are essential for hearing in vertebrates are cadherin-23 and protocadherin-15, proteins whose interaction with one another acts

Overcoming barriers: a review on innovations in drug delivery to the middle and inner ear.

Despite significant advances in the development of therapeutics for hearing loss, drug delivery to the middle and inner ear remains a challenge. As conventional oral or intravascular administration are ineffective due to poor bioavailability and impermeability of the blood-labyrinth-barrier, localized delivery is becoming a preferable approach for certain drugs. Even then, localized delivery to the ear precludes continual drug delivery due to the invasive and potentially traumatic procedures required to access the middle and inner ear. To address this, the preclinical development of controlled

Radiological diagnosis of the inner ear malformations in children with sensorineural hearing loss.

Malformations in either the inner ear, vestibulocochlear nerve (VIIIth) or auditory cortex of the brain can lead to congenital sensorineural hearing loss (SNHL). In most cases the underlying disorders involve the membranous labyrinth at a microscopic level and therefore radiological examinations are entirely normal. In a significant proportion however (up to 20%), there are abnormalities visualized in the inner ear and/or the VIIIth nerve; the type of abnormality is relevant for the surgical planning of a cochlear implant. Imaging and the accurate radiological identification of the affected in

Gene Delivery into the Inner Ear and Its Clinical Implications for Hearing and Balance.

The inner ear contains many types of cell, including sensory hair cells and neurons. If these cells are damaged, they do not regenerate. Inner ear disorders have various etiologies. Some are related to aging or are idiopathic, as in sudden deafness. Others occur due to acoustic trauma, exposure to ototoxic drugs, viral infections, immune responses, or endolymphatic hydrops (Meniere's disease). For these disorders, inner ear regeneration therapy is expected to be a feasible alternative to cochlear implants for hearing recovery. Recently, the mechanisms underlying inner ear regeneration have bee

Evolution of Endolymph Secretion and Endolymphatic Potential Generation in the Vertebrate Inner Ear.

The ear of extant vertebrates reflects multiple independent evolutionary trajectories. Examples include the middle ear or the unique specializations of the mammalian cochlea. Another striking difference between vertebrate inner ears concerns the differences in the magnitude of the endolymphatic potential. This differs both between the vestibular and auditory part of the inner ear as well as between the auditory periphery in different vertebrates. Here we provide a comparison of the cellular and molecular mechanisms in different endorgans across vertebrates. We begin with the lateral line and v

Pharmacokinetic principles in the inner ear: Influence of drug properties on intratympanic applications.

Local drug delivery to the ear has gained wide clinical acceptance, with the choice of drug and application protocol in humans largely empirically-derived. Here, we review the pharmacokinetics underlying local therapy of the ear using the drugs commonly used in clinical practice as examples. Based on molecular properties and perilymph measurements interpreted through computer simulations we now better understand the principles underlying entry and distribution of these and other drugs in the ear. From our analysis, we have determined that dexamethasone-phosphate, a pro-drug widely-used clinica

Malformations of the middle and inner ear on CT imaging in 22q11 deletion syndrome.

The 22q11 deletion syndrome (22q11DS), the most frequent microdeletion syndrome in humans, presents with a large variety of abnormalities. A common abnormality is hearing impairment. The exact pathophysiological explanation of the observed hearing loss remains largely unknown. The aim of this study was to analyze the middle and inner ear malformations as seen on computer tomographic imaging in patients with 22q11DS. We retrospectively reviewed the charts of 11 22q11DS patients who had undergone a CT of the temporal bone in the past. Of the 22 examined ears, two showed an abnormal malleus and i

Division of the stapedial tendon results in noise-induced damage to the inner ear.

BACKGROUND: The effect of division of the stapedial tendon on susceptibility to noise-induced inner ear damage has not been previously studied. This study aimed to evaluate the effects of noise exposure following division of the stapedial tendon in guinea pigs. MATERIAL AND METHODS: Ten adult albino guinea pigs were used. The stapedial tendon of each right ear was cut. The stapedial tendon in each left ear was left intact and these ears served as a control group. DPOAEs and ABR tests were performed before and 10 days after noise exposure. The animals were exposed to a 110-dB noise stimulus fo

Recurrent meningitis in an adult secondary to an inner ear malformation: imaging demonstration.

Congenital labyrinthine dysplasia with a translabyrinthine cerebrospinal fluid (CSF) fistula may be an anatomic cause for recurrent meningitis. This condition is usually seen in children aged 5 to 10 years who present with sensorineural hearing loss (SNHL) and CSF discharge through the nose or ear, with or without recurrent meningitis. Multidetector-row computed tomography (MDCT) and high-resolution T2-weighted magnetic resonance imaging (MRI) of the petrous portion of the temporal bone can help to diagnose this abnormality. We report a case of translabyrinthine CSF fistula in an adult-a 30-ye
