Controversies in otoplasty.

In the last 30 years many surgeons, plastic as well as ENT surgeons, have been increasingly concerned with the correction of protruding ears. There are many controversial points of view regarding the preferable surgical approach. Each surgeon is claiming to find the best solution to this specific issue. To find an acceptable solution, various techniques tackling all the components of the deformity are screened. My preferred technique for the reconstruction of prominent ears, using the best of several procedures is presented: club-excision of the posterior skin, anterior scratching of the facul

Prominent ears.

The prominent ear deformity can be divided into two basic types, and combinations of the two. Two different operations are proposed to treat the cause of the deformity without foreign bodies or suture tension. The result should be an ideal shape. The ear should also feel normal and should not show any stigma of surgery.

Total Ossicular Chain Reconstruction: Is Autologous Material Better Than a TORP?

INTRODUCTION: Total ossicular chain reconstruction may be performed using autologous cartilage or bone, or with a total ossicular replacement prosthesis (TORP). It is unclear if one method offers better hearing outcomes. The goal of this study was to compare hearing outcomes following total ossicular chain reconstruction with autologous material or TORPs. METHODS: A single-institution prospective database was queried to identify patients who underwent total ossicular chain reconstruction and had available audiometric tests. Preoperative and postoperative air-bone gaps (ABG) at 0.5 to 4 kHz we

The acute surgical management of injury to the helix and antihelix in patients with large body surface area burns.

Deformity or loss of the ear may be caused by superficial dermal burns or deep burns. The depth of ear burns is progressive because the ear protrudes from head and is easily affected by external pressure. Therefore, burn wounds of the ear should be debrided as early as possible, before irreversible changes of the cartilage, and covered with healthy tissue. We describe a surgical procedure for treatment of the extensively burned ear. With this technique, the helix is covered by a postauricular advancement flap, and the antihelix is covered by a skin graft. Because the procedure is straightforwa

Ear reduction with or without setback otoplasty.

Ear reduction has been performed occasionally for aesthetic considerations. This series is comprised of 8 patients representing 7 bilateral reductions and 1 unilateral reduction for a total of 15 ears. Historically, surgery for macrotia consisted of resections of full-thickness wedges from the periphery of the ear. The surgical defect was repaired by directly approximating the wound edges. Additional removal of triangular or crescent-shaped segments from adjacent sides of the wound prevented the cupping of the reconstructed ear. Secondary deformities were not uncommon consequences. The most di

Chondritis of the burned ear: a preventable complication.

Chondritis may occur in up to 25 percent of burned ears and commonly follows a superficial partial-thickness injury. The onset is usually insidious and often delayed. Prevention is the key, as the treatment of an established infection frequently leads to disastrous consequences. Herein, we have reviewed 136 consecutive inpatients with burns of the ear. The burned ear was managed by careful, twice daily washing with minimal debridement, frequent application of mafenide acetate, and avoidance of pressure on the affected ear. Exposed cartilage was treated in the same manner, with surgical debride

Relapsing polychondritis: inflamed joints and ears.

BACKGROUND: Relapsing polychondritis (RP) is an episodic and progressive inflammatory disease of the cartilaginous structures, including elastic cartilage of the ear and nose, hyaline cartilage of the peripheral joints, fibrocartilage at axial sites, and cartilage of the tracheo-bronchial tree. The spectrum of its presentations may vary from intermittent mild episodes of chondritis to occasional organ involvement or even life-threatening manifestations. CASE REPORT: We presented a 64 year-old male patient with bilaterally knee arthritis and discoloration of pinna. CONCLUSION: There is lack o

The fissura antitragohelicina: an anatomic aid to the correction of prominent ears.

One of the important features of correction of prominent ears involves the creation of an antihelical fold in the ear cartilage. The precise and symmetrical location of this fold is crucial for the aesthetic result. This study investigated the use of the fissura antitragohelicina, a constant anatomic landmark, as a guide to the correct line for the new antihelix. In the first part of the study, 16 cadaveric ears were dissected. The fissura antitragohelicina was present in each specimen, and measurements of the distance between the fissura antitragohelicina and the helix and the antihelix were …

Regenerative Drug Discovery Using Ear Pinna Punch Wound Model in Mice.

The ear pinna is a complex tissue consisting of the dermis, cartilage, muscles, vessels, and nerves. Ear pinna healing is a model of regeneration in mammals. In some mammals, including rabbits, punch wounds in the ear pinna close spontaneously; in common-use laboratory mice, they remain for life. Agents inducing ear pinna healing are potential regenerative drugs. We tested the effects of selected bioactive agents on 2 mm ear pinna wound closure in BALB/c mice. Our previous research demonstrated that a DNA methyltransferase inhibitor, zebularine, remarkably induced ear pinna regeneration. Altho

Regeneration of rabbit ear tissue.

Holes punched in the ears of rabbits are repaired by regeneration of new tissues from blastemas found on the periphery of the wounds. The proportion whch successfully regenerated was greater in males than in females (75% vs 20%), in pregnancy than during etrus or lactation (75% vs 25%), and in ovariectomized does given testosterone than in those given oil carrier alone (88% vs 50%), but the speed of closure did not differ in these groups. The steroid hormone influence postulated by other workers is confirmed. Closure was faster in younger animals and for second holes punched on the same site w

A new method of total reconstruction of the auricle for microtia.

This new method for total reconstruction of the auricle is a two-stage operation in which the first stage involves fabrication and grafting of a three-dimensional costal cartilage framework and the second stage is the ear elevation operation. The three-dimensional frame was designed with emphasis on the fabrication of the helical crus, intertragic notch, and antitragus and by extending the crus helicis and fixing it to the undersurface of the base frame. The reconstructed auricle will have four different floors: the cymba and cavum conchae are the bottom (fourth) level, the crus helicis is the

Auricular ossification: A newly recognized feature of osteoprotegerin-deficiency juvenile Paget disease.

We report auricular ossification (AO) affecting the elastic cartilage of the ear as a newly recognized feature of osteoprotegerin (OPG)-deficiency juvenile Paget disease (JPD). AO and auricular calcification refer interchangeably to rigid pinnae, sparing the ear lobe, from various etiologies. JPD is a rare Mendelian disorder characterized by elevated serum alkaline phosphatase activity accompanied by skeletal pain and deformity from rapid bone turnover. Autosomal recessive transmission of loss-of-function mutations within TNFRSF11B encoding OPG accounts for most JPD (JPD1). JPD2 results from h

Tympanic membrane perforation repair with acellular porcine submucosa.

OBJECTIVES: To evaluate the efficacy of acellular porcine small intestine submucosa in the repair of chronic tympanic membrane perforations. Although tympanoplasty with autologous temporalis fascia and cartilage is common practice in the repair of chronic tympanic membrane perforations, these materials are associated with increased operative time and have variable availability and quality in individual patients. Recently, new materials for tympanoplasty have been explored, including acellular human dermis. Small intestine submucosa (Surgisis) is an inexpensive and readily available alternative
