Risk management in pregnancy termination.

This chapter reminds those who provide abortion services that crises are inevitable in the medical, counselling and administrative areas of … . A medical crisis must be treated as the life-threatening event that it is, regardless of personal ego damage, social disruptions and/or financial considerations. The more personnel trained in cardiopulmonary resuscitation, the better.

[Personality of patients with dermatologic artefacts].

35 female patients with dermatological artefacts being treated in the University Skin Clinic Basel have been investigated by a dermatologist, a psychiatrist, and a psychologist. The testpsychological examination with the Color Pyramid Test and the Rosenzweig Picture-Frustration Test yielded a striking uniform picture: The patients showed strong intrapsychic tension, severe depressivity, inhibition of aggression, affective inhibition, low frustration tolerance, weak ego-integration (with tendencies to break outs of impulses) and autoaggressive tendencies. These results point to a depressive and…

Neurasthenia, Generalized Anxiety Disorder, and the Medicalization of Worry in a Vietnamese Psychiatric Hospital.

This article examines two forms of the medicalization of worry in an outpatient psychiatric clinic in Ho Chi Minh City, Vietnam. Biomedical psychiatrists understand patients' symptoms as manifestations of the excessive worry associated with generalized anxiety disorder (GAD). Drawing on an ethnopsychology of emotion that reflects increasingly popular models of neoliberal selfhood, these psychiatrists encourage patients to frame psychic distress in terms of private feelings to address the conditions in their lives that lead to chronic anxiety. However, most patients attribute their symptoms to

Shifting self, shifting memory: testing the self-memory system model with hypnotic identity delusions.

According to Conway's self-memory system (SMS) model, autobiographical memories may be facilitated, inhibited, or misremembered to be consistent with current self. In 3 experiments, the authors tested this by hypnotically suggesting an identity delusion and indexing whether this shift in self produced a corresponding shift in autobiographical memory. High hypnotizable participants displayed a compelling identity delusion and elicited specific autobiographical events that they could justify when challenged. These memories were reinterpretations of previous experiences that supported the suggest

Disturbances of self-other distinction after stimulation of the extrastriate body area in the human brain.

In a recent experiment with functional magnetic-resonance imaging, we found that brain activity in the extrastriate body area (EBA) distinguished between observed self- and other-generated movements, being significantly higher during observation of someone else's movement. Here, we investigated further the role of EBA in self-other distinctions using low-frequency repetitive transcranial magnetic stimulation (rTMS). As compared with rTMS applied over a control site, rTMS applied over the EBA increased reaction times, without affecting accuracy, for the detection of other-generated movements. P

Pentheus rather than Oedipus: on perversion, survival and analytic 'presencing'.

Following an introductory review of the main developments in the psychoanalytic thinking on perversion, the author focuses on her own understanding of perversion and its treatment, based on the psychoanalytic treatment of patients with severe sexual perversions. This paper uses the term 'autotomy' (borrowed from the field of biology) to describe perversion formation as an 'autotomous' defence solution involving massive dissociative splitting in the service of psychic survival within a violent, traumatic early childhood situation; thus, a compulsively enacted 'desire for ritualised trauma' ensu

The obscure object of desire: 'nearly, but clearly not, like me': contingency preference in normal children versus children with autism.

The author describes the central role of contingency detection in early socioemotional development. It has been proposed (Gergely & Watson, 1999) that infants are innately equipped with a complex perceptual mechanism, the "contingency detection module," which functions to establish the primary representation of the bodily self as well as the later orientation toward reactive social objects. According to the "contingency switch" model, the target value of the module that is initially genetically set to preferentially explore perfectly response-contingent stimulation is "switched" at around 3 mo

'Listen, listen trust your own strange voice' (psychoanalytically informed conversations with a woman suffering serious illness).

This paper argues that the authentic voice of suffering can be denied to people with serious illness. Anguish and expressions of grief are sometimes replaced by joviality. Amid suffering, voices of anguish become strange inner voices that challenge defences of cheeriness and so are avoided, go unheard or are ignored. Bolstered by the everydayness of ward events and hospital life, suppression, repression and denial mean that people with serious illness can suffer without a voice. Listening to others requires the ability to listen to one's own inner voices and recognize how they might guide exch

Episodic memory and the self in a case of isolated retrograde amnesia.

Isolated retrograde amnesia is defined as impaired recollection of experiences pre-dating brain injury with relatively preserved anterograde learning and memory. We present findings from a patient (M.L.) with isolated retrograde amnesia following severe traumatic brain injury (TBI) that address hypotheses of the interrelationships of focal neuropathology, episodic memory and the self. M.L. is densely amnesic for experiences predating his injury, but shows normal anterograde memory performance on a variety of standard tests of recall and recognition. The cognitive processes underlying this perf

[The body as hostage of the self. Phenomenological aspects of anorexia nervosa].

There are several basic structures upon which anorexia nervosa could develop: hostility between mother and daughter, feministic protest, abandonnism, the ascetic structure, the reluctance against the being-thrown-on-the-world, asw. The phenomenological analysis reveals, through these different structures, a common distress. The patient feels the others, and also, the existence, as a violence. Yet, she feels help ass in her fight against this violence. So she takes her own body as hostage in order to delay her entry in the existence.

The use of color for the secondary elaboration of the dream.

Freud's term sekundäre Bearbeitung has been translated as both "secondary revision" and "secondary elaboration." In keeping with a distinction made by Silber (1973), the term secondary elaboration is used in this paper to indicate the process by which further dream modification occurs after dream recall in order to deepen the disguise of the manifest content for the analyst. Clinical cases are presented in an attempt to demonstrate the sole use of color for the alteration of dreams subsequent to their initial recall. Secondary elaboration in these cases is attributed to resistance to transfere

Relational patterns in four girls exposed to domestic violence: A case study.

BACKGROUND: Domestic violence has a significant impact on the psychological development of children, shaping their internal relational model … ure and hostile relational patterns, with parental figures perceived as threatening. Disruptions in ego functions such as impulse control, identity, and autonomy were observed. Defensive mechanisms including dissociation, repression, and fantasy were prominent, reflecting an overadaptive response to emotional distress. CONCLUSIONS: Relational dysfunc …

Resilience in Women who Experience Domestic Violence.

Violence in the family constitutes a serious social and psychological problem with harmful consequences leading, among others, to changes in the psychological functioning of the victim and, secondarily, also the perpetrator. The aim of this study was to examine resilience in women experiencing domestic violence. The "Ego Resiliency Scale" (ERS) was used to study the group of women suffering domestic violence. The study group included 52 women aged 30-65 years (mean age: 40.15) using assistance of the Crisis Intervention Centre due to experienced domestic violence. They most often reported suff…

Functional links of obsessive, dysmorphic, hypochondriac, and eating-disorders related mental intrusions.

Unwanted mental intrusions (UMIs) are the normal variants of obsessions in Obsessive-Compulsive Disorder (OCD), preoccupations about defects … re the most interfering, and EDs-related intrusions interfered the least. The four UMI were equally ego-dystonic. Women appraised OCD-related intrusions more dysfunctionally, but men appraised the four intrusive contents similarly. All UMI instigated the urge to "do something", to keep them under control and/or neutralizing them. Conclusions: Similari …
