One hundred fifty-eight primary Souter elbow arthroplasties were done on 134 patients (121 women) with severe joint destruction (Larsen Grade 5) or large bone defects or both. Joint replacement operations were done at our institution from 1985-1997. The study group comprised 156 joints in 132 patients with rheumatoid arthritis or other variants of chronic inflammatory joint disease, one in a patient with osteoarthritis, and one patient with posttraumatic arthrosis. The mean age of the patients at the time of surgery was 57 years (range, 26-81 years) and the mean disease duration was 27 years (…

Musculoskeletal complications following chickenpox are rare among immunologically normal children. Septic arthritis after varicella is caused by group A Streptococcus and affects the knee most frequently. We present a case of septic arthritis of the elbow caused by Staphylococcus aureus. We review the English language literature on septic arthritis complicating chickenpox. We conclude that diagnosis and treatment should be carried out according to guidelines on de novo septic arthritis including the early use of magnetic resonance imaging.

Although limited literature exists on medial epicondylitis of the elbow, this disorder is an injury affecting many athletes at every level, especially throwing athletes. Care must be taken in diagnosing medial epicondylitis to distinguish it from other possible pathologies of the medial elbow, which may exist concurrently. The large majority of patients diagnosed with medial epicondylitis will respond to a well-structured, nonsurgical program; however, patients with persistent or recurring symptoms can be treated surgically, which yields high patient satisfaction and ultimately a reliable retu…

Elbow injuries in athletes who perform overhead throwing motions often present diagnostic challenges because of the undue stresses and often chronic, repetitive patterns of injury. Accurate and efficient assessment of the injured elbow is essential to maximize functional recovery and expedite return to play. Radiographic evaluation should be tailored to the specific injury suspected and requires a thorough understanding of normal anatomic relationships as well as familiarity with common injuries affecting these athletes.

Entrapment of the ulnar nerve at the elbow is the second most common focal peripheral neuropathy. Recent advances have facilitated the electrodiagnosis of this common nerve entrapment. The goals of electrodiagnosis are to localize ulnar nerve dysfunction, confirm that the disturbance is confined to the ulnar nerve, and assess the severity of ulnar nerve dysfunction. The goal of this review is to highlight the important advances in anatomy, neurophysiology and methodology that impact upon the electrodiagnosis of entrapment of the ulnar nerve at the elbow, illustrate the limits of electrodiagnos…

In this study we will discuss entrapment of the median, ulnair, radial and lateral antebrachial nerves of the elbow and the forearm. Compression of the nerves may occur when they traverse a tunnel and an incompatibility exists between the diameter of the tunnel and its contents (e.g. nerves, tendons,...). However, at the elbow and the forearm the nerves are also exposed to particularly dynamic compressions. This is due to anatomical relationships changing between the nerve and its surrounding muscles, tendons and aponevroses during the motion of flexion-extension of the elbow and the prono-sup…

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association2001eng
PURPOSE: The purpose of this study was to show the diagnostic and therapeutic benefits of elbow arthroscopy in athletically active pediatric patients. We describe our techniques for elbow arthroscopy in young patients.
TYPE OF STUDY: Case series of 47 patients.
METHODS: We reviewed 49 cases of elbow arthroscopies performed in 47 pediatric and adolescent patients retrospectively over a 16-year period. Charts were reviewed to identify the age of patients at the time of surgery, side involved, hand dominance, diagnosis, complications, the primary sport involved, and the average volume of loose …

The surgical treatment of elbow ectopic ossification associated with elbow stiffness has progressed significantly in the past decade. Although previous reports describe inconsistent results and high complication rates, numerous recent reports document not only good results, but also lower complication rates. The current study outlines the authors' treatment of patients with ectopic bone about the elbow. Various modalities have been used for prophylaxis against elbow ectopic ossification in the patient with elbow trauma. However, despite these prophylaxis efforts, periarticular ossification may…

Increased valgus stresses on the medial aspect of the elbow can result in a number of bony and soft-tissue abnormalities. The most common abnormalities are ulnar neuropathy and valgus instability secondary to injury of the medial collateral ligament. Diagnosis and treatment of these disorders are discussed in this article.

The clinical presentation, diagnosis, radiographic features, mechanism, pathologic changes, and treatment of elbow instability are understood better now. Elbow instability can be classified according to five criteria: (1) the timing (acute, chronic or recurrent); (2) the articulation(s) involved (elbow versus radial head); (3) the direction of displacement (valgus, varus, anterior, posterolateral rotatory); (4) the degree of displacement (subluxation or dislocation); and (5) the presence or absence of associated fractures. Posterolateral rotatory instability is the most common pattern of elbow…

Using any imaging modality, the elbow is a complex joint to evaluate. The use of scintigraphy in the evaluation of the effects of acute or chronic trauma has largely been confined to epicondylitis. We developed a skyline view of the elbow that minimized the effect of overlap and assessed its incremental value in the scintigraphic assessment of several pathological conditions. Thirty-four abnormalities were evaluated in 20 patients with a range of pathologies, including epicondylitis, ligamentous avulsion injury, articular injury, fractures and synovitis. The incremental value of addition of th…

Competitive and recreational athletes sustain a wide variety of soft tissue, bone, ligament, tendon and nerve damage to their upper extremities. Most such injuries are related to direct trauma or repetitive stress, and account for a significant amount of 'down time' for athletes participating in a wide range of sports, particularly those in which the arm is utilised for throwing, catching or swinging. Overuse injuries to the elbow include musculotendinous injuries, ulnar nerve injuries and ligamentous injuries. Osteochondrol lesions of the capitellum and posterior impingement injuries in the j…

Tendon injuries are among the more common sources of elbow pain, resulting from occupational and sports activities as well as the activities of daily living. The exquisite soft-tissue contrast resolution and multiplanar capability of MR imaging have proven ideal for evaluating, diagnosing, and managing tendon injuries. Familiarity with normal anatomy, the spectrum of tendon derangement from tendinosis to complete tears, and the typical injury mechanisms involved in tendinopathy about the elbow are all invaluable for appropriate MR imaging examination and interpretation.

PURPOSE: To correlate radiographic and clinical findings of elbow fractures.
MATERIAL AND METHODS: A retrospective study was made of 110 consecutive adult elbow fractures of various types involving the humerus, the ulna, and the radius.
RESULTS: Twenty-seven fractures with radiographically conspicuous distortions healed. Of these only 15 had clinical sequelae. Five had clinical sequelae without showing conspicuous radiographic distortions after healing.
CONCLUSION: For radiographic evaluation and classification purposes, it would seem to be important to note the status of the capitellum, th…
