Management of acute distal humeral fractures in patients with rheumatoid arthritis. A case series.

BACKGROUND: The best surgical treatment for a patient with rheumatoid arthritis and an acute distal humeral fracture is not well established. Because of the distorted anatomy of the arthritic elbow joint and the adjacent osteoporotic bone, total elbow arthroplasty may be favored over open reduction and internal fixation in these patients. We retrospectively analyzed a series of patients with rheumatoid arthritis in whom an acute distal humeral fracture had been treated with either open reduction and internal fixation or total elbow arthroplasty; our purpose was to evaluate their outcomes and t

An Unreported Variant of Convergent Dislocation of Elbow.

INTRODUCTION: Pediatric traumatic elbow dislocation is an uncommon injury accounting for 3-% of all pediatric elbow injuries. It can present as an isolated injury or in association with fractures of radius, ulna, or humerus. Posterolateral type is the most common accounting for 70% of all pediatric elbow dislocations. Other less common types are anterior, medial, lateral, convergent, and divergent dislocations. We describe an unreported variant of convergent elbow dislocation. CASE REPORT: This is the case report of a convergent elbow dislocation with a radial neck fracture and proximal ulna

WITHDRAWN: Interventions for treating wrist fractures in children.

BACKGROUND: Approximately a third of all fractures in children occur at the wrist, usually from falling onto an outstretched hand. OBJECTIVES: We aimed to evaluate removable splintage versus plaster casts (requiring removal by a specialist) for undisplaced compression (buckle) fractures; cast length and position; and the role of surgical fixation for displaced wrist fractures in children. SEARCH METHODS: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (October 2007), the Cochrane Central Register of Controlled Trials (The Cochrane Library 2007, Issue 4), MED

Triangular Fixation Technique for Bicolumn Restoration in Treatment of Distal Humerus Intercondylar Fracture.

BACKGROUND: Distal humerus intercondylar fractures are intra-articular and comminuted fractures involving soft tissue injury. As distal humerus is triangle-shaped, parallel plating coupled with articular fixation would be suitable for bicolumn restoration in treatment of distal humerus intercondylar fracture. METHODS: This study included 38 patients (15 males and 23 females) who underwent olecranon osteotomy, open reduction and internal fixation with the triangle-shaped cannulated screw and parallel locking plates (triangular fixation technique). Functional results were assessed with the visu

Outcomes following distal humeral fracture fixation with an extensor mechanism-on approach.

BACKGROUND: Distal humeral fractures have traditionally been managed with surgical approaches that disrupt the extensor mechanism. We hypothesized that an extensor mechanism-on approach for operative fixation of distal humeral fractures with parallel or orthogonal plate constructs would allow excellent healing, a motion arc of the elbow exceeding 100°, and maintenance of extensor mechanism strength. METHODS: Distal humeral open reduction and internal fixation (ORIF) was performed with either orthogonal or parallel plate constructs in seventy-nine elbows. Thirty-seven elbows were fixed via an

Radial head fractures.

The radial head is the most commonly fractured bone of the elbow, with most fractures occurring in women older than 50 years. The radial head is an important stabilizer for valgus, axial, and posterolateral rotational forces. Loss of articular segments of the radial head negatively affects elbow kinematics and stability. Most fractures are treated nonsurgically. Indications for nonsurgical treatment include nondisplaced and isolated displaced fractures without a block to motion. Fragment excision is indicated when a mechanical block from a small, displaced fragment impedes elbow motion. If tec

Acute fractures of the scaphoid bone: Systematic review and meta-analysis.

BACKGROUND: The scaphoid fractures account for 50%-80% of all carpal bone fractures in young individuals. Non-union of the fracture occurs in approximately 5%-10% of undisplaced scaphoid fractures. Current management varies significantly among different places and surgeons. OBJECTIVES: The purpose of this review is to investigate the evidence of the effectiveness and safety of various treatments of acute scaphoid fractures. METHODOLOGY: Systematic review and metanalysis of all the randomised and quasi-randomised trials comparing different treatments of acute scaphoid fractures. RESULTS: Thi

Hahn-Steinthal fracture: report of two cases.

Isolated capitellar fractures are rare injuries accounting for only 1% of all elbow fractures. Type I or Hahn-Steinthal fractures require internal fixation to restore the anatomic integrity of the elbow joint. Many different fixation methods have been described and, independently of the method chosen, precise reduction is mandatory to maximize articular congruency and to diminish the potential for secondary osteoarthritis. The treatment and the outcome of two type I capitellar fractures are described in this study. One patient (April 1999) underwent open reduction and Kirschner wire fixation w…

Problems With Medium-Sized Joints: Elbow Conditions.

For patients with elbow pain, a comprehensive history and physical examination can identify the mechanism of injury and specific tests can help determine the underlying pathology. When imaging is indicated, x-ray typically is the initial modality. Indications for ultrasonography include the need for static, dynamic, and stress visualization of elbow cartilage, tendons, ligaments, and osseous structures. Magnetic resonance imaging study is preferred for assessment of chronic elbow pain because of its ability to detect bone marrow edema, tendinopathy, nerve entrapment, and joint effusion. In chi

An original internal fixation technique by tension band wiring with steel wire in fractures of the coronoid process.

Fractures of the coronoid process, which is a key element in anterior elbow joint stability, represent 14% of proximal ulnar fractures. Optimal treatment should stabilize all fractures associated with elbow instability. Different techniques have been described: suture repair, screws, plates… We propose a series of 5 patients who were treated with an original, easy, tension band wiring fixation technique using steel wire with easy hardware removal.

Coaching of patients with an isolated minimally displaced fracture of the radial head immediately increases range of motion.

STUDY DESIGN: Prospective cohort. INTRODUCTION: Elbow stiffness is the most common adverse event after isolated radial head fractures. PURPOSE OF THE STUDY: To assess the effect of coaching on elbow motion during the same office visit in patients with such fractures. METHODS: We enrolled 49 adult patients with minimally displaced radial head fractures, within 14 days of injury. After diagnosis, we measured demographics, catastrophic thinking, health anxiety, symptoms of depression, upper extremity-specific symptoms and disability, pain, and elbow and wrist motion. The patient was taught to

Intra-articular injuries of the elbow: pitfalls of diagnosis and treatment.

Poor results in treating fractures and dislocations about the elbow may be avoided if the surgeon is aware of the possible injuries, examines good radiographs of both elbows, and treats the injury promptly and appropriately. A displaced fracture of the lateral or medial condyle of the humerus should be suspected if there is a flake fracture of the adjoining metaphysis; open reduction and internal fixation give better results than closed reduction. A shear fracture of the capitulum humeri can only be seen on a lateral radiograph; excision of the fragment, followed by mobilization, is sufficient

Fractures of the radial head.

Most fractures of the radial head are stable undisplaced or minimally displaced partial fractures without an associated fracture of the elbow or forearm or ligament injury, where stiffness following non-operative management is the primary concern. Displaced unstable fractures of the radial head are usually associated with other fractures or ligament injuries, and restoration of radiocapitellar contact by reconstruction or prosthetic replacement of the fractured head is necessary to prevent subluxation or dislocation of the elbow and forearm. In fractures with three or fewer fragments (two arti

Good functional outcomes after open reduction and internal fixation for AO/OTA type 13-C2 and -C3 acute distal humeral fractures in patients aged over 45 years.

BACKGROUND: The purpose of this study was to report the functional outcomes and complications after open reduction and internal fixation (ORIF) for Arbeitsgemeinschaft für Osteosynthesefragen (AO)/Orthopaedic Trauma Association (OTA) type 13-C2 and -C3 acute distal humeral fractures with minimum 2-year follow-up. Our hypothesis was that ORIF would provide functional outcomes that are comparable to those of total elbow arthroplasty and elbow hemiarthroplasty reported in the literature. MATERIALS AND METHODS: During a 6-year period, 23 patients aged > 45 years were treated with double plating f

Osteochondral flap fracture of the coronoid in pediatric humeral lateral condyle: A report of 3 cases.

RATIONALE: Osteochondral flap fractures of the elbow are rare in children. To the best of our knowledge, only 12 cases are reported in the literature. Only 1 case was accompanied with lateral condyle fracture classified as Milch I, which was nondisplaced. The mechanism of these injuries is not explained in detail, and the treatment methods are not discussed. Here, we present 3 cases of osteochondral flap fracture of the ulnar coronoid process with accompanying obvious displacement of the lateral condyle fracture. PATIENT CONCERNS: All patients fell into one of their outstretched arms, which c
