Radiocapitellar prosthetic arthroplasty in traumatic and post-traumatic complex lesions of the elbow.

Radiocapitellar prosthetic arthroplasty is a fascinating new option in the treatment of complex lesions of the elbow. The device has been developed in order to treat primitive and post-traumatic radiocapitellar degenerative arthritis, while preserving the ulnohumeral joint of the elbow. Compared to a radial head resection, it avoids valgus and axial instability of the elbow and forearm preventing ulnohumeral degeneration. Seven patients have been reviewed with a mean of follow-up of 40 months. Two of them presented with an acute trauma and five of them with a post-traumatic degenerative arthri

[Arthroscopy of the elbow: diagnostic and therapeutic approaches].

The elbow is one of the most complex joints of the human body. Bony, ligamentous and muscular constraints ensure elbow stability. During recent years elbow arthroscopy has become more and more popular resulting from technical and surgical innovations. The diagnostic and therapeutic elbow arthroscopy following traumatic elbow dislocation is the best example. Functional outcomes after elbow dislocation significantly depend on sufficient evaluation of elbow stability, possible accompanying soft tissue injuries and on the initiation of adequate therapy. Elbow arthroscopy after traumatic elbow disl

Radiographic assessment of medial elbow stability.

BACKGROUND: Diagnosing simple valgus instability of the elbow currently involves time- and cost-intensive imaging modalities such as magnetic resonance imaging or magnetic resonance arthrography. Previous studies have demonstrated that stress radiography represents an alternative diagnostic tool for such conditions. The aim of this study was to investigate whether standardized valgus stress radiography can identify soft-tissue lesions of the medial elbow. METHODS: A telos stress device (telos GAIII/E; telos Arzt- und Krankenhausbedarf GmbH, Woelfersheim-Bernstadt, Germany) was used to apply 5

[Diagnosis of upper arm, elbow, and proximal lower arm fractures].

The complex anatomy of the elbow joint enables a wide range of movement and complex functions in everyday and professional life. Typical injuries of this joint include a variety of different pathologies. Due to overlaying structures in plain radiographs, diagnosis of injuries to the elbow joint places high demands on the treating physicians and often needs further diagnostic imaging. The following article will give an overview of the most common of these injuries and their diagnosis.

Surgical treatment of rheumatoid arthritis of the elbow.

Rheumatoid arthritis (RA) is a chronic inflammatory condition that significantly impacts multiple joints, including the elbow, crucial for daily activities. Despite advancements in medical treatments like disease-modifying antirheumatic drugs that delay disease progression, surgical interventions are often necessary to improve the quality of life in RA patients. This review provides a comprehensive evaluation of the elbow, focusing on history taking, physical examination, and radiographic assessments, particularly magnetic resonance imaging findings. We detail the selection and counseling of p

Evaluation of accuracy of the Finnish elbow dysplasia screening protocol in Labrador retrievers.

OBJECTIVE: To determine whether the current Finnish screening method using a single flexed mediolateral view as scored by osteophyte is sufficient to diagnose mild elbow dysplasia in Labrador retrievers and to determine if an additional craniocaudal oblique projection would result in improvement in the screening protocol. MATERIALS AND METHODS: Thirteen dogs with one mildly affected elbow joint and one elbow joint without radiological evidence of osteophytes were studied. Radiographic and computed tomography studies were performed and the results compared with each other. RESULTS: Medial com…

A functional study of the rheumatoid elbow.

A clinical and functional study of rheumatoid elbows has been done to discover the magnitude and frequency of disabilities caused by elbow problems. Pain and weakness were found to affect all activities which were tested, with adjacent joint involvement, flexion deformity, and lack of supination causing significant disability. Instability was not a frequent problem. The frequency of functional disabilities, linked to severe pain, suggests that the development of an elbow joint replacement would be worthwhile.

Editorial Commentary: Safety in Anteromedial Elbow Portal Placement? Go North!

Since iatrogenic injury to surrounding structures is more likely in the elbow than in the other major joints, many studies have examined the relationship of elbow arthroscopy portals to the at-risk anatomy. In accessing the anterior compartment of the elbow from the medial side, the brachial artery and median, ulnar, and medial antebrachial cutaneous nerves are at risk. Factors that improve the safety of this approach include the use of a proximal versus distal anteromedial portal, a distended versus and nondistended joint, and a flexed versus extended elbow position, all of which result in an

Magnetic resonance imaging of primary and concomitant flexor enthesopathy in the canine elbow.

Flexor enthesopathy is a recently recognized elbow disorder in dogs and considered to be an important differential diagnosis for elbow lameness. Primary and concomitant forms of the disease have been previously described and treatments differ for the two forms. The goal of this prospective study was to compare magnetic resonance imaging (MRI) findings for dogs with primary flexor enthesopathy (n = 17), concomitant flexor enthesopathy (n = 23), elbow dysplasia alone (n = 13), and normal elbows (n = 7). Each elbow joint underwent MRI using the same low-field scanner. Sequences included transvers…

Treatment Outcomes of Simple Elbow Dislocations: A Systematic Review of 1,081 Cases.

BACKGROUND: The treatment of simple elbow dislocations (SEDs) has become more functional last decade with a tendency to shorter immobilization of the elbow, whereas simultaneously, surgical stabilization has been promoted by some authors. The primary aim of this study was to systematically review the literature and analyze the outcomes and complications of different treatment options for acute and persistent SEDs, including operative and nonoperative treatments with varying immobilization periods. METHODS: A literature search was performed based on the online medical databases MEDLINE, Embase

[Monteggia-like injuries : Pitfalls of surgical treatment].

The term Monteggia-like lesions or Monteggia equivalent injuries includes fractures of the proximal ulna with dislocation and dislocation fractures of the radial head from the proximal radio-ulnar joint. The complexity of the injury requires a dedicated understanding of the anatomical structures and their biomechanical properties. Particularly due to the rarity, the complication and revision rates are high. A conservative treatment is not usually effective. Three-dimensional imaging by computed tomography is part of the surgical preparation. The goal of surgical treatment is osteosynthetic rec

Transolecranon Exposure of Monteggia Variant Fracture-dislocations of the Elbow.

Posterior Monteggia fracture-dislocations are uncommon injuries that can result from a fall onto an outstretched hand. Often, these injuries are associated with coronoid and/or radial head fractures. When this is the case, direct fixation can require 3 separate fascial incisions: posterior to address the proximal ulnar fracture, lateral to address the radial head fracture, and medial to address the coronoid fracture. We illustrate a transolecranon surgical approach for fixation of a type IIA posterior Monteggia fracture-dislocation with associated radial head and coronoid fractures. In this ap

[Progress on prevention and treatment of heterotopic ossification].

Heterotopic ossification is the formation of pathological bone in non-skeletal tissues (including muscles, tendons or other soft tissues), and the pathogenesis is not completely clear. It is often caused by musculoskeletal trauma, postoperative bone and joint surgery, or damage of the nervous system, the clinical manifestations are joint swelling, pain, and movement disorders, which often occur around the hips, knees, and elbows. At present, the prevention of heterotopic ossification mainly includes drugs, radiotherapy, molecular biological mechanism intervention, and Chinese medicine-related

Radiographic/MR Imaging Correlation of the Elbow.

This article covers the use of radiographs and MR imaging for diagnosing elbow disorders. It concentrates on these techniques, but other imaging techniques including computed tomography, ultrasound, and nuclear medicine are discussed when clinically important. The article covers the use of imaging for specific symptoms including lateral, medial, anterior, posterior and generalized elbow pain, focal swelling, restriction of movement, locking, and neurologic symptoms. Radiographs have been the mainstay of initial imaging of the elbow, while ultrasound examination is often used for soft tissue di

[Recurrent dislocation of the elbow].

Recurrent dislocation of the elbow is uncommon compared to other recurrent dislocations. The most frequent etiology consists in lesions of the radial collateral ligament that cause instability of the elbow joint. Surgical repair using the Osborne and Cotterill technique seems to give the best results, both short and long term.
