Time trends in the intracardiac potential recorded by pacemaker telemetry: comparison between steroid-eluting small area electrodes.

We assessed the time course of electrograms sensed both in the atrium and ventricle by two different steroid-eluting electrodes: Medtronic Capsure SP (with an area of 5.5 mm2) and Z (with an area of 1.2 mm2). We considered 68 unipolar electrodes: 31 atrial (19 Capsure SP 4523 and 12 Capsure Z 4533) and 37 ventricular (24 Capsure SP 4023 and 13 Capsure Z 4033) implanted in 47 consecutive patients (30 men and 17 women, with an age of 72 +/- 9.4 years). The pacemaker model was Medtronic Elite 7077-7086 (DDD-DDDR) in 25 patients and Medtronic Legend 8419-8424 (VVIR-AAIR) in 22 patients. The endoca

Prosthesis Control with an Implantable Multichannel Wireless Electromyography System for High-Level Amputees: A Large-Animal Study.

BACKGROUND: Myoelectric prostheses lack a strong human-machine interface, leading to high abandonment rates in upper limb amputees. Implantable wireless electromyography systems improve control by recording signals directly from muscle, compared with surface electromyography. These devices do not exist for high amputation levels. In this article, the authors present an implantable wireless electromyography system for these scenarios tested in Merino sheep for 4 months. METHODS: In a pilot trial, the electrodes were implanted in the hind limbs of 24 Sprague-Dawley rats. After 8 or 12 weeks, im

Chronic tissue response to untethered microelectrode implants in the rat brain and spinal cord.

OBJECTIVE: Microelectrodes implanted in the central nervous system (CNS) often fail in long term implants due to the immunological tissue response caused by tethering forces of the connecting wires. In addition to the tethering effect, there is a mechanical stress that occurs at the device-tissue interface simply because the microelectrode is a rigid body floating in soft tissue and it cannot reshape itself to comply with changes in the surrounding tissue. In the current study we evaluated the scar tissue formation to tetherless devices with two significantly different geometries in the rat br

Comparing Patient-Specific Variations in Intra-Cochlear Neural Health Estimated Using Psychophysical Thresholds and Panoramic Electrically Evoked Compound Action Potentials (PECAPs).

PURPOSE: Variations in neural survival along the cochlear implant electrode array leads to off-place listening, resulting in poorer speech understanding outcomes for recipients. Therefore, it is important to develop and compare clinically viable tests to identify these patient-specific intra-cochlear neural differences. METHODS: Nineteen experienced cochlear implant recipients (9 males and 10 females) were recruited for this study. We estimated the neural health along the electrode array for a group of experienced adult implant recipients using two methods: the difference between psychophysic

Double challenge: cochlear implantation in the only hearing ear with progressive hearing loss following meningitis and vestibular dysfunction after implantation.

OBJECTIVE: Vestibular dysfunction associated with cochlear implantation is rare. It is usually seen in patients with otosclerosis due to spread of electrical activity throughout the demineralized bone. A 17-year old female with progressive hearing loss 2 years after meningitis and vestibular dysfunction in the implanted ear is presented in this study. FINDINGS: The patient had mild hearing loss in the right ear and total hearing loss on the left side because of complete ossification of the cochlea following meningitis. She had to have cochlear implantation in the right ear because of progress

High beta activity in the subthalamic nucleus and freezing of gait in Parkinson's disease.

OBJECTIVE: Oscillatory activity in the beta band is increased in the subthalamic nucleus (STN) of Parkinson's disease (PD) patients. Rigidity and bradykinesia are associated with the low-beta component (13-20Hz) but the neurophysiological correlate of freezing of gait in PD has not been ascertained. METHODS: We evaluated the power and coherence of the low- and high-beta bands in the STN and cortex (EEG) of PD patients with (p-FOG) (n=14) or without freezing of gait (n-FOG) (n=8) in whom electrodes for chronic stimulation in the STN had been implanted for treatment with deep brain stimulation.

[Cochlear implants and total cochlear obstruction].

Total cochlea obstruction is not a contraindication for cochlear implant. We report our experience owing to the single channel cochlear implant Monosonic (9 cases), partial insertion of the electrodes-array (3 cases: 1 Digisonic and 2 minisystem) and, more recently the separate electrodes version of the Digisonic (6 cases). This new device is encouraging because it supplies 2/5 patients with telephone use.

[Study of the relationship of stapedial reflex thresholds induced during cochlear implant surgery and the highest hearing comfort of paediatric patients].

INTRODUCTION AND GOALS: We have studied the relationship between the minimum stapedial reflex thresholds induced by means of the intra-operative stimulation of the cochlear implant and the post-operative thresholds of highest auditory comfort level. METHODS: We have assessed the maximum auditory comfort at the end of the first and second quarters following activation of the cochlear implant, by which time the strategy for codification of the stimuli and the programming map are considered to be already stable. The study was carried out at our cochlear implants unit. All the patients in the stu

Clinical evaluation of the SmartNav system contribution in cochlear implantation surgery.

OBJECTIVE: To evaluate the contribution of electrode insertion live monitoring, offered by the Nucleus® SmartNav system, in cochlear implantation surgery since correct and precise electrode placement can assure better preservation of the cochlear structure. METHOD: A prospective cohort study was conducted on 66 patients, with normal cochlear anatomy, operated for unilateral cochlear implantation. Once the surgeon started inserting the electrodes under SmartNav monitoring, the progression of the electrode array and its position were assessed, and the angular insertion depth, placement time and

An Ultra-Flexible Neural Electrode with Bioelectromechanical Compatibility and Brain Micromotion Detection.

Neural electrodes, as core components of brain-computer interfaces(BCIs), face critical challenges in achieving stable mechanical coupling with brain tissue to ensure high-quality signal acquisition. Current flexible electrodes, including semi-invasive meningeal-attached types and implantable cantilever designs, exhibit significant mechanical mismatches (elastic modulus 5-6 orders higher than brain tissue) due to material/structural limitations, leading to interfacial slippage. While thread-like implants (e.g., Neuralink's electrodes) improve compliance via elongated structures, quantitative c

Chronic stability and selectivity of four-contact spiral nerve-cuff electrodes in stimulating the human femoral nerve.

This study describes the stability and selectivity of four-contact spiral nerve-cuff electrodes implanted bilaterally on distal branches of the femoral nerves of a human volunteer with spinal cord injury as part of a neuroprosthesis for standing and transfers. Stimulation charge threshold, the minimum charge required to elicit a visible muscle contraction, was consistent and low (mean threshold charge at 63 weeks post-implantation: 23.3 +/- 8.5 nC) for all nerve-cuff electrode contacts over 63 weeks after implantation, indicating a stable interface with the peripheral nervous system. Insular Role in Blood Pressure and Systemic Vascular Resistance Regulation.

OBJECTIVES: The insula is a brain area involved in the modulation of autonomic responses. Previous studies have focused mainly on its heart rate regulatory function, but its role in vascular control is not well defined. Ictal/postictal blood pressure (BP) fluctuations may have a role in the pathogenesis of sudden unexpected death in epilepsy. This study aims to characterize the insular influence on vascular regulation through direct high-frequency electrical stimulation (E-stim) of different insular regions during stereo-electroencephalographic studies. MATERIALS AND METHODS: An observational

Restoring motor control and sensory feedback in people with upper extremity amputations using arrays of 96 microelectrodes implanted in the median and ulnar nerves.

OBJECTIVE: An important goal of neuroprosthetic research is to establish bidirectional communication between the user and new prosthetic limbs that are capable of controlling >20 different movements. One strategy for achieving this goal is to interface the prosthetic limb directly with efferent and afferent fibres in the peripheral nervous system using an array of intrafascicular microelectrodes. This approach would provide access to a large number of independent neural pathways for controlling high degree-of-freedom prosthetic limbs, as well as evoking multiple-complex sensory percepts. Maximizing the fidelity of a photovoltaic subretinal prosthesis for human patients.

Objective.PRIMA subretinal implants provide prosthetic vision to patients blinded by age-related macular degeneration, with acuity closely matching the sampling limit of the pixel pitch: a single 100 μm pixel per line of a letter corresponds to 20/420 acuity. Decreasing the pixel size in the same flat geometry is difficult due to the constrained electric field, especially considering a 40 μm thick debris layer separating the implant from the target neurons. Here we optimize the electrode design to help overcome such limitations.Approach.An end-to-end modeling pipeline combines the retinal phot

Parametric characterization of the rat Hering-Breuer reflex evoked with implanted and non-invasive vagus nerve stimulation.

Vagus nerve stimulation (VNS) has rapidly gained interest as a treatment for a variety of disorders. A number of methods have been employed to stimulate the vagus nerve, but the most common relies on a cuff electrode implanted around the cervical branch of the nerve. Recently, two non-invasive methods have increased in popularity: transcutaneous cervical VNS (tcVNS) and transcutaneous auricular VNS (taVNS). Despite promising clinical results, there has been little direct comparison of these methods to stimulation delivered via an implanted device. In this study, we directly compared both non-i

[Cochlear implant in children: rational, indications and cost/efficacy].

A cochlear implant (CI) is a partially implanted electronic device that can help to provide a sense of sound and support speech to severely to profoundly hearing impaired patients. It is constituted by an external portion, that usually sits behind the ear and an internal portion surgically placed under the skin. The external components include a microphone connected to a speech processor that selects and arranges sounds pucked up by the microphone. This is connected to a transmitter coil, worn on the side of the head, which transmits data to an internal receiver coil placed under the skin. The
