Improving irregular temporal modeling by integrating synthetic data to the electronic medical record using conditional GANs: a case study of fluid overload prediction in the intensive care unit.

OBJECTIVE: The challenge of irregular temporal data, which is particularly prominent for medication use in the critically ill, limits the performance of predictive models. The purpose of this evaluation was to pilot test integrating synthetic data within an existing dataset of complex medication data to improve machine learning model prediction of fluid overload. MATERIALS AND METHODS: This retrospective cohort study evaluated patients admitted to an ICU ≥ 72 hours. Four machine learning algorithms to predict fluid overload after 48-72 hours of ICU admission were developed using the original

An Integrated Influenza Surveillance Framework Based on National Influenza-Like Illness Incidence and Multiple Hospital Electronic Medical Records for Early Prediction of Influenza Epidemics: Design and Evaluation.

BACKGROUND: Influenza is a leading cause of death worldwide and contributes to heavy economic losses to individuals and communities. Therefore, the early prediction of and interventions against influenza epidemics are crucial to reduce mortality and morbidity because of this disease. Similar to other countries, the Taiwan Centers for Disease Control and Prevention (TWCDC) has implemented influenza surveillance and reporting systems, which primarily rely on influenza-like illness (ILI) data reported by health care providers, for the early prediction of influenza epidemics. However, these survei

Testing regular expression searches and machine learning models to determine housing instability and low income status from primary care electronic medical record data in Toronto, Ontario.

BACKGROUND: Housing and income are important social determinants of health (SDoH). Primary care providers often do not have information about these determinants, which could be used to support equitable health system planning and care delivery. The aim of this study was to use primary care electronic medical record (EMR) data to test two approaches (machine learning and regular expression searches) to obtain information about patients' housing instability and low income status. METHODS: We used de-identified EMR data from the St. Michael's Hospital Academic Family Health Team (Toronto, Ontari

Disease activity outcome measures are only available in half of the electronic medical files of patients with axial spondyloarthritis followed in an outpatient clinic: the results of an audit of a tertiary-care rheumatology department.

Current recommendations for management of patients with axial spondyloarthritis (axSpA) include regular collection of validated disease activity outcomes. This study aimed at evaluating the proportion of patients for whom validated outcome measures were available on their electronic medical reports (EMR), and the factors associated with the presence of such information on the EMR. We performed a cross-sectional monocentric observational study, including patients with an axSpA diagnosis who attended an outpatient visit between February, 2018 and February, 2019. Patients (demographics, disease c

Collecting and registering sexual health information in the context of HIV risk in the electronic medical record of general practitioners: a qualitative exploration of the preference of general practitioners in urban communities in Flanders (Belgium).

UNLABELLED: Background and aim Current health-care delivery requires increasingly proactive and inter-professional work. Therefore, collecting patient information and knowledge management is of paramount importance. General practitioners (GPs) are well placed to lead these evolving models of care delivery. However, it is unclear how they are handling these changes. To gain an insight into this matter, the HIV epidemic was chosen as a test case. METHODS: Data were collected and analysed from 13 semi-structured interviews with GPs, working in urban communities in Flanders. Findings GPs use vari

Investigating Strategies for Sustaining Modern Health Information Systems for Enhanced Care Delivery in Fragile Resource-Constrained Settings Through Electronic Medical Record Implementation in Haiti: Qualitative Study.

BACKGROUND: Robust and reliable health information systems (HISs) are foundational to equitable health care delivery in resource-constrained settings. Yet, HISs often exhibit significant fragmentation and complexity, which stem from many factors, including inadequate infrastructure, limited and unevenly allocated financial resources, expertise gaps, and a lack of integrated systems. At the same time, advances in modern HISs and digital technologies, such as electronic medical records (EMRs), present opportunities for addressing these limitations and supporting evidence-based health systems if

Use of an electronic medication management application to support Pharmacists Review to Optimise Medicines in Residential Aged Care (PROMPT-RC): a study protocol for a parallel cluster randomised controlled trial.

INTRODUCTION: Most older adults living in residential aged care facilities (RACFs) have at least one marker of potentially suboptimal prescribing. Pharmacists play a crucial role in medication management, with their effectiveness enhanced by using computerised decision support tools. The Pharmacists Review to Optimise Medicines in Residential Aged Care (PROMPT-RC) study aims to optimise medicine use by providing pharmacists in RACFs with an electronic medicine management app with integrated decision support (AusTAPER App/Pathway) to use as part of medication reviews they undertake. METHODS AN
