Respiratory Emergencies and Management of Mining Accidents.

The rapid detection of the reasons for mining accidents that lead to emergency situations is vital for search and rescue work. The control of fire and gas leakage provides an immediate approach for rescue works for deaths or injuries and the detection of who needs resuscitation outside of the mine. The evacuation and recovery operations should be directed by continuous monitoring of the mine environment due to fire and explosion risks. The main toxic gases in mines are carbon monoxide (CO) and carbon dioxide (CO2); the flammable gases are methane (CH4), CO, and hydrogen (H2); the suffocating g

[Paediatric dermatology emergencies in a tertiary hospital].

INTRODUCTION: In recent years, the increasing demand for dermatological consultations in the Emergency department has resulted in the publication of a variety of studies on this subject. However, most of them deal with the general population, without taking into account the changes in frequencies found in young children (ages 0-14). OBJECTIVES: To determine the frequency of various dermatological diagnoses made by the on-call paediatrician in the Emergency Department, and after referral to Paediatric Dermatology. MATERIAL AND METHODS: Firstly, a descriptive retrospective study was performed

Bedside ultrasonography for obstetric and gynecologic emergencies.

There has been an increase in the availability and use of bedside ultrasonography in the acute care setting. The approach to the female patient with a pelvic complaint (including pelvic pain, vaginal bleeding, vaginal bleeding in pregnancy, or vaginal discharge) has been transformed by the use of bedside ultrasonography. Providers familiar with the transabdominal and transvaginal (endocavitary) ultrasonographic examination can obtain more accurate information faster, thereby improving time to consultation or discharge and achieving an increase in patient satisfaction. This article reviews the

Acute aortic emergencies--part 1: aortic aneurysms.

This article is a 2-part series about patients with aortic emergencies. Patients with acute aortic disease who present to the emergency department represent some of the highest acuity patients that emergency clinicians will ever encounter in acute care settings. Part 1 focuses on aortic aneurysms in the thorax or abdomen. An aortic aneurysm involves transverse dilatation, leading to rupture and hemorrhage. Diagnosis is largely based on clinical presentation and carefully selected imaging studies. Emergency interventions are guided by whether or not the patient is hypertensive or in shock. For …

Antidotes for toxicological emergencies: a practical review.

PURPOSE: Appropriate therapies for commonly encountered poisonings, medication overdoses, and other toxicological emergencies are reviewed, with discussion of pharmacists' role in ensuring their ready availability and proper use. SUMMARY: Poisoning is the second leading cause of injury-related morbidity and mortality in the United States, with more than 2.4 million toxic exposures reported each year. Recently published national consensus guidelines recommend that hospitals providing emergency care routinely stock 24 antidotes for a wide range of toxicities, including toxic-alcohol poisoning, …

Emergencies of the liver, gallbladder, and pancreas.

Abdominal pain is a common presenting complaint in today's emergency department (ED). Disorders related to the liver, gallbladder, and pancreas are responsible for many of these presentations. With the increasing prevalence of gallstones, as well as alcohol use and abuse, the numbers of cases are likely to increase. This article examines hepatic emergencies including alcoholic hepatitis, spontaneous bacterial peritonitis, hepatorenal syndrome, and hepatic encephalopathy. In addition, the authors review the presentation, evaluation, and management of acute biliary tract disorders with some emph…

Mortuary provision in emergencies causing mass fatalities.

Emergencies that cause the deaths of large numbers of people can result from major accidents, natural disasters or acts of hostility. While what has happened cannot be undone, other people may be protected from similar events by careful investigation of the causes, the collection of evidence and the prosecution of any criminal acts. Within the wider investigation, responding to the deaths will focus on the respectful treatment and accurate identification of the physical remains, and where possible their return to the next of kin. A large number of deaths or complexity of circumstances, such as…

[Vascular emergencies of the head and neck].

The clinical manifestations of vascular lesions of the head and neck may be variable (hemorrhagic, ischemic, compressive). Diagnosis often is made at the time of acute presentation, but delayed manifestations, sometimes long after the initial presentation, should not be overlooked. Hemorrhagic manifestations are characterized by epistaxis, corresponding mainly to lesions of the nasal cavities but involvement of the internal carotid artery should be excluded (life threatening). In addition, some vascular malformations may lead to severe hemorrhage. Ischemic manifestations typically result from

Bacteriological aspects implicated in abdominal surgical emergencies.

UNLABELLED: The purpose of the present study was to establish the microbial etiology of abdominal surgical emergencies as well as the relationship between the bacterial etiology and the virulence factors produced by the respective isolated strains. 110 bacterial strains were isolated from 100 randomized clinical cases, operated during 2009-2010 in the First Surgical Clinic of the University Hospital of Bucharest. The clinical cases (sex ratio 52 M/48F aged between 22-85 years old) were classified into three risk groups, as related to their severity. The isolated strains were characterized by c…

[Hypertensive emergencies in adults: a practical review].

Hypertensive emergencies must be distinguished from severe blood pressure elevations without acute target organ damage. Clinical examination (chest pain, dyspnoea, neurological disorders, ECG, retinal examination) and laboratory tests (blood and urine tests, cerebral imaging in case of neurological disorders) have to be immediately performed. Immediate referral to an intensive care unit is indicated, and an intravenous antihypertensive therapy has to be implemented. Blood pressure objectives depend on the associated acute pathology (myocardial infarction, pulmonary oedema, aortic dissection, s…

Adrenal insufficiency and other adrenal oncologic emergencies.

Normal function of the adrenal gland can be disrupted not only by metastases of nonadrenal cancers but also by their treatment. In addition, tumors of the adrenal gland itself can cause disease by hypersecretion of a variety of hormones, adrenal gland destruction with inadequate production of cortisol, and by metastasis to other sites. Although rare, abnormal adrenal function should be considered in the appropriate clinical settings as failure to recognize and treat can result in significant morbidity and mortality. The adrenal "incidentaloma" is a frequent finding of abdominal radiologic stud

[Neuropediatric emergencies--epileptic seizures and acute hemiplegia].

Status epilepticus and stroke are life-threatening neurological emergencies and immediate recognition and medical management are imperativ. There is a serious risk of neurological sequelae. To limit secondary brain damage a prolonged seizure has to be treated without delay. After a short history and clinical evaluation (principles of resuscitation) a benzodiazepine (usually diazepam or lorazepam) has to be administered and in case of persistence of seizure-activity phenytoin or phenobarbital. Blood- and urine-sample must be collected in the acute phase to perform further metabolic or toxic exa…

[Emergencies in adults with sickle cell disease].

Sickle cell disease is an inherited disease characterized by the presence of an abnormal haemoglobin. It is the more prevalent genetic disease at birth in Ile-de France area. Sickle cell disease can be complicated by acute vaso-occlusive crisis, chronic visceral involvement related to ischemic process, and infectious complications. In adults, acute vaso-occlusive crisis is the major clinical problem prompting admission to hospital and the main cause of death. It mainly manifests by osteoarticular pain and acute chest syndrome and can be complicated by multiorgan failure. Acute vaso-occlusive c

[Small intestine tumors: our experience in emergencies].

Small bowel tumours are relatively rare neoplasms; unusual occurrence associated with nonspecific symptoms, and low-sensitivity tests availability, are responsible for diagnostic delay. A retrospective study was performed on 42 cases with acute presentation, from 1972 to 2001; median age was 52 years (range 14-79) and there was a slight female prevalence (57.1% vs 42.9%). The most common acute presentation was occlusion (57.1%), followed by gastrointestinal (GI) bleeding (23.8%), perforation (14.3%) and occlusion/perforation (4.8%).
