BACKGROUND AND PURPOSE: Patients with acute disseminated encephalomyelitis (ADEM) and multiple sclerosis (MS) have a similar pattern of abnormalities on conventional MR images. A case of severe acute encephalomyelitis associated with a serological diagnosis of both Mycoplasma pneumoniae and Legionella pneumophila infection is reported. Serological co-positivity between these two pathogens has been reported previously, and has, in general, been attributed to cross-reactivity. This is the first documented case of co-infection using more sensitive and specific serological techniques. The potential significance of these findings is discussed in the context of the considerable problems in the interpretation of serological data.

Mice, guinea pigs, and duck embryo cell cultures were inoculated with known subtypes of Venezuelan equine encephalomyelitis (VEE) virus and the attenuated (TC-83) strain of VEE. With the exception of TC-83, all strains were highly pathogenic for suckling mice by either intracranial or intraperitoneal routes of inoculation used. Virulence for older mice and guinea pigs provided a means to distinguish strains. In addition, virulence or lack of virulence for adult mice or guinea pigs provides a rapid method for separating epizootic subtype IB from TC-83 VEE virus isolates.

Major variables which determine the induction and severity of adoptive autoimmune encephalomyelitis are the age and strain of the animal, and the amount of killed mycobacteria in the adjuvant. Control of these factors results in consistent production of this disease in high incidence and in severe form. The pathologic changes in the central nervous system can be correlated with the clinical disease. Maturity of the target tissues in the central nervous system of the newborn appears to be an important factor which distinguishes the response of the guinea pig from that of other species.
