Bedwetting--not always what it seems: a sign of sleep-disordered breathing in children.

ISSUES AND PURPOSE: Nocturnal enuresis (bedwetting) can linger long into childhood. Sleep research has documented that nocturia and bedwetting are symptoms of obstructive sleep apnea (OSA) in adults and that bedwetting is predictive of OSA in children. CONCLUSIONS: Nocturnal polyuria is a cardiovascular response to negative pressure breathing (inspiration against a closed glottis), which is characteristic of OSA. PRACTICE IMPLICATIONS: Evidence of nocturnal polyuria and sleep-disordered breathing are important signs of OSA, a serious but treatable condition.

Urinary tract infections in children.

A five-year review of pediatric patients with urinary tract infections shows that absolute colony counts are not satisfactory as the sole criterion for urinary tract infection. Prior to culture results, patients can be treated empirically if the urine is nitrite-positive or if there is more than an occasional organism per high power field on unspun urine. In this series, no diagnostic or therapeutic benefit resulted from localizing studies. Patients with enuresis and infection should undergo radiologic evaluation for an anatomic abnormality.

Allergy testing, psychological assessment and dietary treatment of the hyperactive child syndrome.

Thirty-one children with behavioural problems and learning difficulties were allergy tested for sensitivity to salicylates, artificial colours and flavours, eighteen children had a positive response, and 15 of these were given the Australian Version of the Feingold K.P. diet. Ninety-three per cent responded with improved behaviour in the areas of overactivity, distractability, impulsiveness and excitability. Sleep and enuresis problems were resolved partially or completely. This study demonstrates that the aforementioned elimination diet significantly affects behaviour.

Daytime Bladder Control Status in Toddlerhood Is Associated With Subsequent Bedwetting in Preschool Years: A Nationwide Cohort Study of Over 30 000 Japanese Children.

OBJECTIVES: Nocturnal enuresis is common in early childhood. While daytime bladder control typically precedes nighttime continence, the temporal relationship between early daytime bladder control and subsequent bedwetting remains unclear. We investigated whether daytime bladder control status at age 2.5 years-as indicated by diaper use-is associated with bedwetting at age 4.5 years in a Japanese nationwide cohort. METHODS: We analyzed data from the Japanese Longitudinal Survey of Newborns in the 21st Century (2010 cohort). Daytime bladder control was assessed at age 2.5 years through caregive…

Incontinence in children with autism spectrum disorder.

INTRODUCTION: Autism spectrum disorder (ASD) and incontinence (nocturnal enuresis (NE), daytime urinary incontinence (DUI), fecal incontinence (FI)) are relevant disorders in childhood. In general, children with special needs such as intellectual disability (ID) or ASD are more often affected by incontinence than typically developing children. OBJECTIVE: In the few studies conducted on children with ASD, gastrointestinal (GI) symptoms have received more attention than NE, DUI and lower urinary tract symptoms (LUTS). The aim of the present study was to assess the prevalence of incontinence, LU…

Prevalence of incontinence, attention deficit/hyperactivity disorder and oppositional defiant disorder in preschool children.

Externalizing disorders as attention deficit/hyperactivity disorder (ADHD) and oppositional defiant disorder (ODD) are common in children with nocturnal enuresis (NE), daytime urinary incontinence (DUI) and faecal incontinence (FI). We examined the prevalence rates of ADHD, ODD and incontinence in a defined geographical area and analysed the association between externalizing disorders and subtypes of incontinence. 1,676 parents of children who were presented at the mandatory school-entry medical examination completed a questionnaire with all DSM-IV items of ADHD, ODD and six questions regardin…

Penile strangulation injuries.

Penile tourniquet injury is probably not a rare entity. Various objects can cause penile strangulation, which may lead to severe injury. Three penile strangulation injuries are presented, one due to thread and two to hair. There were urethral fistulas in two cases. All patients were circumcised and had enuresis nocturna. They were treated successfully. In enuretic patients, it should be kept in mind that there may be a strangulation that can cause penile edema or ulceration and even a urethral fistula.

[The characteristics of the bladder-sphincter reflex in children].

Based on the examination of 89 children (71 patients with various bladder diseases and 18 healthy subjects), the authors developed a new procedure for recording the vesicosphincteral reflex by the use of urethral profilometry. The procedure involved comparison of background and stimulation examination findings of urethral profilograms. The optimum parameters were defined for detection of vesicosphincteral reflex and the conditions were developed for its performance. The procedure may be useful in the diagnosis of neurogenic bladder, sphincter-detrusor dyssynergia, stress-induced enuresis, etc.

Rectal diameter assessment in enuretic children-exploring the association between constipation and bladder function.

OBJECTIVES: Detrusor overactivity and constipation often co-exist in children with enuresis. Constipation is known to be linked to detrusor overactivity. The voiding chart is the best non-invasive way to investigate bladder function, whereas the ultrasonographical detection of rectal dilatation is the best way to objectify constipation. We wanted to investigate a possible relationship between the rectal diameter and voiding chart data in enuretic children. METHODS: Children with therapy-resistant enuresis were retrospectively evaluated. All had completed a voiding chart for at least 48 h. The…

Basis and therapeutical rationale of the urinary concentrating mechanism.

The urine-concentrating mechanism performs one of the most essential functions in water and electrolyte metabolism and serves primarily to m … ed a precise differential diagnosis. These include different forms of diabetes insipidus, nocturnal enuresis and nocturia in the elderly. Diabetes insipidus represents a dramatic but rare disturbance of water balance caused by deficient AVP secretion (neurogenic), reduced renal sensitivity to AVP (nephrogenic), an abnormally high fluid intake (primary poly …

Prevalence of primary tethered cord syndrome associated with occult spinal dysraphism in primary school children in Turkey.

The prevalence and associated factors of primary tethered cord syndrome (PTCS) in primary school children were investigated. A cross-sectional study was performed in four demographically different primary schools in Turkey. Demographic, familial and physical data were collected from 5,499 children based on enuresis as a predominant symptom and dermatologic and orthopedic signs as clues of occult spinal dysraphism. Statistical analysis and input of the data were carried out with the SPSS package program 10.00, and logistic regression analysis was used to identify discriminating factors between …
