An epidemic of enterovirus 71 infection in Taiwan. Taiwan Enterovirus Epidemic Working Group.

BACKGROUND: Enteroviruses can cause outbreaks of hand-foot-and-mouth disease (characterized by vesicular lesions on the hands, feet, and oral mucosa) or herpangina, usually without life-threatening manifestations. In 1998 an epidemic of enterovirus 71 infection caused hand-foot-and-mouth disease and herpangina in thousands of people in Taiwan, some of whom died. METHODS: We assessed the epidemiologic aspects of this outbreak. Cases of hand-foot-and-mouth disease or herpangina in ambulatory patients were reported to the Taiwan Department of Health by a mean of 818 sentinel physicians. Severe c

[The use of Bayes' theorem in controlling the coming epidemic peak of epidemic meningitis in the 1980s].

To control the coming epidemic peak of epidemic cerebrospinal meningitis (ECM) in the 1980s in the regions with 100 million population in China, the mathematical models based upon Bayes' theorem (BT) were established and used respectively in provincial, regional and county's level. Reports of ECM from each ten-day's period or each month during the meningitis season were analysed to create forecast models. Records of ECM vaccinating rate in previous years were fully taken into account to modify the theoretical values. Calibration, split-sample, random-sample selection, as well as actual forecas

Toxic epidemic syndrome, Spain, 1981. Toxic Epidemic Syndrome Study Group.

The initial symptoms of a new multisystem disease probably caused by ingestion of denatured rapeseed oil were acute pleuropneumopathy, fever, headache, exanthems, myalgia, and eosinophilia. Later features were severe myalgia and thromboembolic accidents. The present phase of the illness is typified by scleroderma-like involvement, pulmonary arterial hypertension, and a neuromuscular syndrome. Nearly all the patients with these late changes are women. The pathological lesion is a peculiar form of non-necrotising vasculitis which primarily affects the intima.

[Determination of influenza epidemic intensity thresholds for Tianjin using the moving epidemic method].

Objective: To determine the threshold for influenza epidemic intensity during the 2024-2025 season in Tianjin using the moving epidemic method (MEM). Methods: Collect weekly data on the percentage of influenza-like illness cases (ILI%), positive detection rate of influenza virus (PR), and PR×ILI% from sentinel hospitals in Tianjin for 2017-2025. Establish an influenza early warning model using MEM, select the optimal value of parameter δ, compare model fitting performance, evaluate the reliability of the three types of data, and determine the threshold for influenza epidemic intensity in Tianj

John Cross, epidemic theory, and mathematically modeling the Norwich smallpox epidemic of 1819.

In this paper, we reintroduce Dr. John Cross' neglected and unusually complete historical data set describing a smallpox epidemic occurring in Norwich, England in 1819. We analyze this epidemic data in the context of early models of epidemic spread including the Farr-Evans-Brownlee Normal law, the Kermack-McKendrick square Hyperbolic Secant and SIR laws, along with the modern Volz-Miller random-network law. We show that Cross' hypothesis of susceptible pool limitation is sufficient to explain the data under the SIR law, but requires parameter estimates differing from the modern understanding o

Systematic Comparison of Epidemic and Non-Epidemic Carbapenem Resistant Klebsiella pneumoniae Strains.

Over the past few decades, extensively drug resistant (XDR) resistant Klebsiella pneumoniae has become a notable burden to healthcare all over the world. Especially carbapenemase-producing strains are problematic due to their capability to withstand even last resort antibiotics. Some sequence types (STs) of K. pneumoniae are significantly more prevalent in hospital settings in comparison to other equally resistant strains. This provokes the question whether or not there are phenotypic characteristics that may render certain K. pneumoniae more suitable for epidemic dispersal between patients, h

A novel sub-epidemic modeling framework for short-term forecasting epidemic waves.

BACKGROUND: Simple phenomenological growth models can be useful for estimating transmission parameters and forecasting epidemic trajectories. However, most existing phenomenological growth models only support single-peak outbreak dynamics whereas real epidemics often display more complex transmission trajectories. METHODS: We develop and apply a novel sub-epidemic modeling framework that supports a diversity of epidemic trajectories including stable incidence patterns with sustained or damped oscillations to better understand and forecast epidemic outbreaks. We describe how to forecast an epi…

Comparative analysis of protein evolution in the genome of pre-epidemic and epidemic Zika virus.

Zika virus (ZIKV) causes microcephaly in congenital infection, neurological disorders, and poor pregnancy outcome and no vaccine is available for use in humans or approved. Although ZIKV was first discovered in 1947, the exact mechanism of virus replication and pathogenesis remains unknown. Recent outbreaks of Zika virus in the Americas clearly suggest a human-mosquito cycle or urban cycle of transmission. Understanding the conserved and adaptive features in the evolution of ZIKV genome will provide a hint on the mechanism of ZIKV adaptation to a new cycle of transmission. Here, we show compre

Concurrency can drive an HIV epidemic by moving R0 across the epidemic threshold.

OBJECTIVE: The objective of this study is to investigate whether concurrency can drive an HIV epidemic by moving R0 across the epidemic threshold. DESIGN AND METHODS: We use a mathematical framework for a dynamic partnership network and the spread of a one-stage infection to study how concurrency is related to the basic reproduction number R0. Two concurrency indices were used to measure the level of concurrency. The model allows varying the level of concurrency in the population, while other key network properties such as partnership duration and lifetime number of partners are kept fixed. I

Disease metaphors in new epidemics: the UK media framing of the 2003 SARS epidemic.

Since the emergence of HIV/AIDS in the 1980s, social scientists and sociologists of health and illness have been exploring the metaphorical framing of this infectious disease in its social context. Many have focused on the militaristic language used to report and explain this illness, a type of language that has permeated discourses of immunology, bacteriology and infection for at least a century. In this article, we examine how language and metaphor were used in the UK media's coverage of another previously unknown and severe infectious disease: Severe Acute Respiratory Syndrome (SARS). SARS

Forecasting of infectious disease epidemics in sexual networks using attenuated renewal processes.

Forecasting emerging infectious disease outbreaks is particularly challenging for sexual networks of men who have sex with men (MSM), where … assessed using the continuous ranked probability score (CRPS). Across both simulated and empirical epidemics in heterogeneous networks, renewal models attenuated by a reverse-sigmoid curve, to account for early depletion of highly-connected nodes, consistently outperformed alternatives, achieving more balanced and reliable forecasts. Our findings suggest a …

The public health burden of diabetes mellitus and thyroid disease: twin epidemics.

Thyroid dysfunction and diabetes mellitus frequently occur together, with evidence showing higher rates of each condition in individuals affected by the other. Beyond their statistical association, emerging research indicates shared mechanisms involving autoimmunity, obesity, ageing and insulin resistance, which might explain their bidirectional relationship. This Review synthesizes current evidence on these links, with a particular focus on how thyroid dysfunction influences glycaemic control and how antidiabetic therapies affect thyroid function. Unlike earlier reviews, we frame thyroid diso

The Intertwined Epidemics of Obesity and Obstructive Sleep Apnea: A Comprehensive Analysis.

The escalating global prevalence of both obesity and obstructive sleep apnea (OSA) represents a major public health challenge due to their combined impact on individual health outcomes and healthcare infrastructure. This review provides an analytical evaluation of the complex, bidirectional relationship between these conditions. Evidence demonstrates that OSA prevalence rises markedly across progressive body mass index (BMI) categories, reaching up to 90% in individuals with Class III obesity. This association is driven by mechanical mechanisms-such as upper airway pharyngeal fat accumulation

Centuries of Potato Late Blight: Tracking Global Epidemics and Managing Future Outbreaks.

Phytophthora infestans killed the potato crop in Ireland in 1845, leading to widespread famine and the death of more than one million people. Historic herbarium specimens from the famine era were used to understand the pathogen's biology and track its global spread, providing a valuable resource for research. Historic outbreaks in the United States and Europe were caused by the FAM-1 lineage, whereas the US-1 lineage spread later. The famine lineage was basal in the phylogeny and ancestral to modern US-1, Mexican, and globally aggressive lineages. An admixture between the famine lineage and th

Urban contact patterns shape respiratory syncytial virus epidemics with implications for vaccination.

Urban environments may alter the landscape of disease transmission with implications for control. Yet, it is unclear whether urban-rural differences exist in the dynamics of childhood respiratory diseases, given specific mixing patterns in younger age groups. Here, we leverage county-level data on respiratory syncytial virus (RSV) from the United States to reveal an urban-rural gradient in both the intensity and age structure of the RSV epidemic, where urban locations experience more prolonged epidemics with higher burden in infants (under 1 year of age). We develop a mechanistic epidemiologic…

Tuberculosis and Impact of COVID-19 on Spread of Epidemics in Kazakhstan.

This study examines the epidemiological situation of tuberculosis (TB) in the regions of the Republic of Kazakhstan over the past seven years (2018-2024), which cover the before-, during- and after-COVID-19 periods, with a focus on the risks of its emergence and spread. The analysis revealed that while TB incidence is declining, mortality remains high in the before- and during-COVID-19 periods, indicating a general decline in population health. The concentration of TB incidence in relation to geographic location was mainly in the northern, western and southern regions. Before COVID-19, TB inci

The burden of HIV-1 and HIV-2 epidemics in Ivory Coast.

Simian immunodeficiency viruses (SIV) infecting chimpanzees (SIVcpz) and sooty mangabeys (SIVsm) are, respectively, the biological precursors of human immunodeficiency viruses (HIV) Types 1 and 2. Former French colonies in West Africa are the regions where retroviruses first jumped from primates to humans. Ivory Coast is nowadays a country of over 29 million people, being 2% (580,000) persons living with HIV (PLWH). However, one-quarter remains undiagnosed. Heterosexual transmission is by far the most frequent mechanism of HIV acquisition and women exhibit higher rates of infection than men. D

Seven insights from Albert Camus's Plague about epidemics, public health and morality.

For Albert Camus, plague was both a fact of life and a powerful metaphor for the human condition. Camus engaged most explicitly and extensively with the subject of plague in his 1947 novel, The Plague (La peste), which chronicles an outbreak of what is presumably cholera in the French-Algerian city of Oran. I often thought of this novel-and what it might teach us-during the recent COVID-19 pandemic. In this article, I discuss seven important insights from The Plague about epidemics, public health and morality.
