Transendoscopic contact neodymium:yttrium aluminum garnet laser correction of epiglottic entrapment in standing horses.

Fifty-seven Standardbred and 44 Thoroughbred racehorses and 1 Thoroughbred polo mare with primary clinical signs of exercise intolerance or respiratory tract noise or combined exercise intolerance and respiratory tract noise were referred for laser correction of epiglottic entrapment. Significantly (P less than 0.001) more Standardbred than Thoroughbred racehorses were affected, compared with the observed hospital population during the same period. At referral, 14 horses did not have evident epiglottic entrapment and were returned to exercise without development of entrapment after treatment,

Risk of acute epiglottitis in patients with preexisting diabetes mellitus: A population-based case-control study.

OBJECTIVE: Studies have revealed that 3.5%-26.6% of patients with epiglottitis have comorbid diabetes mellitus (DM). However, whether preexisting DM is a risk factor for acute epiglottitis remains unclear. In this study, our aim was to explore the relationship between preexisting DM and acute epiglottitis in different age and sex groups by using population-based data in Taiwan. METHODS: We analyzed data between January 2000 and December 2013 obtained from the Taiwan National Health Insurance Research Database. The case group consisted of 2,393 patients with acute epiglottitis. The control gro…

Acute epiglottitis--aetiology, epidemiology and outcome in a population before large-scale Haemophilus influenzae type b vaccination.

Over a period of 18 years 219 consecutive cases of acute epiglottitis were diagnosed and subsequently investigated in order to elucidate the aetiology, epidemiology and outcome of this disease in a well-defined population in Sweden before general vaccination against Haemophilus influenzae type b infection was introduced. Compared with the results from other parts of the industrialized world, high incidence rates were found in both children (14/100,000/year) and adults (2.3/100,000/year). The annual trend showed a significant decline in incidence among children, whereas in adults it remained un…

Is CPAP the best therapy for all phenotypes of OSA? A case of complete epiglottic collapse treated with CPAP.

INTRODUCTION: Different studies estimate the prevalence of epiglottis complete or near-complete collapse among patients with Obstructive Sleep Apnea (OSA) between 11.5 to 26.6%. (1)(2) Treating OSA with Positive Airway Pressure (PAP) therapy in patients with complete or near-complete epiglottis collapse could worsen the severity of disease and cause further suffering for the patient. CASE SUMMARY: 73 years old man with medical history of PTSD and severe OSA who is intolerant to CPAP. Multiple attempts for adjusting pressure settings were unsuccessful in improving his tolerance. Possibility of

Aspiration as the first-choice procedure for airway management in an infant with large epiglottic cysts: A case report.

BACKGROUND: Epiglottic cysts is a rare but potentially lethal supraglottic airway pathology in infants due to the high risk of cannot intubation or cannot ventilation. Awake fiberoptic intubation appeared to be the safest technique, but it is very challenging in infants with large epiglottic cysts. Even it has the risk of airway loss. We report that cyst aspiration is an effective treatment as the first-choice procedure for airway management in an infant with large epiglottic cysts. CASE SUMMARY: A 46-day-old male infant weighing 2.3 kg presented to the emergency room with difficulty feeding,

Ramsay Hunt syndrome affecting the vagus nerve with epiglottic ulcers as the first manifestation: a case report.

Ramsay Hunt syndrome involving the vagus nerve is very rare. We herein describe a 53-year-old man who developed severe pharyngeal pain after alcohol intoxication. Antibiotic treatment was ineffective. Laryngoscopy showed an ulcer on the right side of the epiglottis. As the condition progressed, the patient developed hoarseness. He then gradually developed multiple herpes lesions on the right side of the head, face, and neck along with right peripheral facial paralysis. Corticosteroid, analgesic, antiviral, and nutritional nerve therapy resulted in disappearance of the herpes lesions, epiglotti

Supra-Epiglottic Upper Airway Volume in Elderly Patients with Obstructive Sleep Apnea Hypopnea Syndrome.

OBJECTIVE: Small upper airway measurements areas and high body mass index are recognized risk factors for obstructive sleep apnea syndrome (OSAS) in non-elderly populations; however, there is limited information regarding elderly patients. We evaluated whether upper airway volume is associated with OSAS and OSAS treated with continuous positive airway pressure (CPAP) treatment and whether BMI is correlated with upper airway volume and measurements in elderly subjects. METHODS: In 60 volunteers aged 75.58±0.9 years: 20 OSAS, 20 OSAS chronically treated with CPAP, and 20 controls, semi-automati

[Combined use of the Airway Scope and gum elastic bougie for tracheal intubation in a patient with an epiglottic cyst].

A 71-year-old man with sigmoid colon cancer was scheduled for the sigmoidectomy under general anesthesia. In preoperative examination, an epiglottic cyst was found accidentally. He had no subjective symptom. Rapid induction with remifentanil, propofol, rocuronium and ephedrine was performed, and the mask ventilation was easy. The Airway Scope (AWS) was inserted at first, but it was difficult to intubate because of the epiglottic cyst. Then a gum elastic bougie (GEB) was inserted under observation by AWS monitor. GEB was passed into the trachea, and the tube was passed over the GEB into the tra

Shikani™ Seeing Optical Stylet-aided tracheal intubation in patients with a large epiglottic cyst.

Large epiglottic cysts can block the glottis, leading to serious consequences. This condition presents a challenge in terms of airway management for anesthesiologists during induction of anesthesia. We report the use of a Shikani™ Seeing Optical Stylet combined with a Macintosh laryngoscope to aid tracheal intubation in seven patients with large epiglottic cysts. Use of this technique can avoid cyst rupture and allow smooth, safe intubation.

Incidentally Detected Asymptomatic Epiglottic Capillary Hemangioma in a 66-Year-Old Female: A Case Report.

Laryngeal hemangiomas are benign vascular tumors that occur predominantly in children. Occurrence in adults is rare, particularly when developing in the epiglottis. This report describes a rare case of a pedunculated capillary hemangioma of the epiglottis in a 66-year-old female. The lesion was incidentally discovered during general anesthesia and was completely asymptomatic. Laryngeal endoscopy revealed a bluish-red, pedunculated mass with prominent vascularity, and MRI findings were consistent with a superficial vascular lesion without evidence of infiltration. Due to the potential risk of a

Barriers to Adoption of the Pre-Epiglottic Baton Plate (Tübingen Palatal Plate) for Infants With Robin Sequence.

ObjectiveDespite evidence of effective management of upper airway obstruction (UAO) in infants with Robin sequence (RS), adoption of the pre-epiglottic baton plate (PEBP) has been slow in North America. The objectives of this study were to identify barriers and elucidate opportunities to overcome them.DesignAn email list of providers in craniofacial units involved in management of RS was compiled using published data form the American Cleft Palate-Craniofacial Association and institutional websites. A custom-designed survey was distributed to the compiled listserv. Responses were aggregated an

Case Reports of Low-Temperature Plasma Radiofrequency Treatment for Congenital Epiglottic Cysts in Neonates and Infants.

An uncommon benign condition in neonates, known as congenital epiglottic cyst, can lead to symptoms such as neonatal dyspnea, laryngeal stridor, and, in severe cases, even pose a potential threat of asphyxia-related mortality. A congenital epiglottic cyst is one of the neonatal emergencies. Fibrolaryngoscopy is the predominant method of diagnosis, and early identification, diagnosis, as well as treatment are the key points. Successful endoscopic ablation of congenital epiglottic cysts in 3 newborns was achieved through the utilization of a low-temperature plasma radiofrequency ablation system.

Effects of Head and Neck Alignment and Pharyngeal Anatomy on Epiglottic Inversion During Swallowing in Dysphagic Patients.

The effects of head and neck alignment and pharyngeal anatomy on epiglottic inversion remain unclear. This study investigated the factors involved in epiglottic inversion, including head and neck alignment and pharyngeal anatomy, in patients with dysphagia. Patients with a chief complaint of dysphagia and who underwent videofluoroscopic swallowing study at our hospital from January to July 2022 were enrolled. They were divided into three groups based on the degree of epiglottic inversion as the complete-inversion (CI), partial-inversion (PI), and non-inversion group (NI) groups. Data were comp

Exaggerated ventilatory drive estimates from epiglottic and esophageal pressure deflections in the presence of airway occlusion.

Esophageal and epiglottic pressure deflections are widely used to quantify ventilatory effort during sleep in patients with obstructive sleep apnea (OSA). However, changes in upper airway patency will fundamentally alter pressure gradients across the respiratory system with different airflow and volume-dependent effects on esophageal versus epiglottic pressure. The magnitude of these obstruction effects on ventilatory effort assessed from pressure deflections has not been systematically investigated. This study sought to quantify the direct effect of airway occlusion on esophageal and epiglott

End-stage renal disease as a risk factor for epiglottitis: a population-based cohort study in Taiwan.

OBJECTIVES: Patients with uremia are prone to infection; however, end-stage renal disease (ESRD) as a risk factor for acute epiglottitis warrants study. We investigated the risk of severe epiglottitis requiring hospitalisation in patients with ESRD. SETTING: We conducted a retrospective matched cohort study by using the claims data of Taiwan's National Health Insurance Research Database. PARTICIPANTS: We identified an ESRD cohort with 87 908 patients newly diagnosed in 2000-2013 and underwent dialysis. The non-ESRD cohort comprised patients who had not received a diagnosis of ESRD, and they …
