Navigating ethical and legal challenges in the HEALthy Brain and Child Development Study: Lessons learned from the ethics, law, policy working group.

The HEALthy Brain and Child Development (HBCD) Study, a multi-site prospective longitudinal cohort study, will examine human brain, cognitive, behavioral, social, and emotional development beginning prenatally and planned through early childhood. The HBCD study has faced several ethical and legal challenges due to its goal of enrolling pregnant people (including those with substance use disorder) and their newborns. Challenges not fully anticipated at the outset emerged from the rapidly changing legal landscape around reproductive rights in the United States. By embedding scholars in bioethics

Ethical Considerations in Critically Ill Neonatal and Pediatric Patients.

The care of critically ill neonates and pediatric patients can be particularly emotionally and ethically challenging. Emerging evidence suggests that we can improve the patient, family, and care team experience in the critical care setting through a better understanding and application of ethical frameworks and communication strategies. We conducted a multidisciplinary panel session at the American Academy of Pediatrics National Conference and Exhibition in the fall of 2022 wherein we explored a myriad of ethical and communication considerations in this unique patient population, with congenit

Meaningful or Marginal: An Integrative Review of Consumer and Community Involvement in Migrant Health Research.

Background/Objectives: Consumer and Community Involvement (CCI) is increasingly mandated in health research to promote ethical, relevant, and impactful research outcomes. However, in migrant health research, CCI can sometimes be tokenistic. This review aimed to critically synthesise the empirical and contextual literature on CCI in migrant health research to examine how meaningfully migrant communities are engaged across the research process. Methods: An integrative literature review was conducted. A systematic search of six databases (CINAHL, EMBASE, PubMed, PsycINFO, Scopus, Web of Science)

The self-management support needs of people diagnosed with psoriatic arthritis: a realist review protocol.

INTRODUCTION: Psoriatic arthritis (PsA) is a form of inflammatory arthritis linked to psoriasis. Previous research from the UK has found that many people feel unsupported when diagnosed with PsA and lack confidence in managing their condition. This realist review aims to understand what works and does not work for whom and in what circumstances, in relation to healthcare professionals engaging with people to support them in developing self-management skills. METHODS AND ANALYSIS: This protocol was developed by defining the scope of the review, using a brief directed literature review to suppo

Achieving universal health coverage for people with stroke in South Africa: protocol for a scoping review.

INTRODUCTION: Stroke is the second most common cause of death after HIV/AIDS and a significant health burden in South Africa. The extent to which universal health coverage (UHC) is achieved for people with stroke in South Africa is unknown. Therefore, a scoping review to explore the opportunities and challenges within the South African health system to facilitate the achievement of UHC for people with stroke is warranted. METHODS AND ANALYSIS: The scoping review will follow the approach recommended by Levac, Colquhoun and O'Brien, which includes five steps: (1) identifying the research questi

Impact of quality routine health data utilisation on health service delivery outcomes in low-income and middle-income countries: a systematic review protocol.

BACKGROUND: Evidence-based decision-making in healthcare relies heavily on routine health information. However, in many low-income and middle-income countries (LMICs), concerns persist regarding the use of quality routine health data for health service delivery. Moreover, no systematic synthesis currently exists on how the use of quality data influences health service delivery in these settings. This systematic review aims to address this gap by consolidating existing evidence on the utilisation of quality routine health data and its impact on health service delivery in LMICs. METHODS AND ANA

Congenital diaphragmatic hernia. In utero therapy and ethical considerations.

Despite advances in neonatal medicine, the mortality rate for congenital diaphragmatic hernia remains high. The results of work in animal models suggest that this anomaly may be amenable to in utero surgical correction. In this article, the natural history of congenital diaphragmatic hernia in humans is reviewed, and the development of antenatal management strategies is traced. The ethical issues surrounding the management of fetuses with diaphragmatic hernias are also discussed. The ground-breaking nature of the development of strategies for management of congenital diaphragmatic hernia under

From film to simulation: Mixed-methods study of learner-driven artificial intelligence-generated ethical dilemma scenarios and role-play to enhance ethical competence in nursing students.

AIM: We aimed to develop and implement the Film-Based, Artificial Intelligence (AI)-Generated Interactive Ethical Dilemma Role-Play Simulation program and evaluate its effects on nursing students' biomedical ethics awareness, critical thinking disposition, empathy and program satisfaction. BACKGROUND: Although AI-based educational tools enhance interactivity and realism, their application in ethics-focused education remains limited. DESIGN: A mixed-methods design was used with a quasi-experimental nonequivalent control group pretest-posttest element and a qualitative thematic analysis based

Ethical governance of public-professional commentary in global psychiatry: A cross-national analysis of Member Societies of the World Psychiatric Association.

BACKGROUND: In polarised sociopolitical and information ecosystems, public-professional commentary from psychiatrists is an increasingly contested ethical space. The World Psychiatric Association (WPA) defers to local norms in this area, positioning its Member Societies as key conduits for ethical governance; the most prominent example is the American Psychiatric Association's (APA) Goldwater Rule, which forbids professional opinions on individuals in public life without examination and consent. Beyond this, the extent to which other national-level associations have operationalised comparable

Tofacitinib in the treatment of primary Sjögren's syndrome-associated interstitial lung disease: study protocol for a prospective, randomized, controlled and open-label trial.

INTRODUCTION: Tofacitinib, a selective inhibitor of JAK1 and/or JAK3, is considered to alleviate the pulmonary condition of primary Sjögren's syndrome (pSS)-associated interstitial lung disease (ILD) through its anti-inflammatory and antifibrotic effects. METHODS AND ANALYSIS: This is a single-center, prospective, randomized, open-label trial. The trial will compare a 52-week course of oral tofacitinib with traditional therapy cyclophosphamide (CYC) combined with azathioprine (AZA) in the treatment of pSS-ILD. A total of 120 patients will be randomly assigned into two treatment groups with a

Ethical considerations of maternal-fetal intervention in a twin pregnancy discordant for anomalies.

BACKGROUND: Recent evidence suggests prenatal fetoscopic tracheal occlusion (FETO) may improve the survival and long-term morbidity of neonates with congenital diaphragmatic hernia, yet little guidance exists in the medical literature as to the ethical permissibility of performing a maternal-fetal surgical intervention in a twin pregnancy discordant for a structural abnormality. CASE: Here, we present a case of a twin pregnancy with an unaffected twin (Twin A) and a twin diagnosed with severe congenital diaphragmatic hernia (Twin B). A proposed fetoscopic tracheal occlusion (FETO) procedure m

Treating difficult or disruptive dialysis patients: practical strategies based on ethical principles.

For more than a decade, dialysis units have had to contend with an increasing number of difficult or disruptive dialysis patients. These individuals present a spectrum of behaviors, ranging from those that harm only themselves to those that physically endanger dialysis staff. Such behaviors can interfere with the ability of the dialysis staff to care for the patient in question and for other patients; in addition, threats or actual physical abuse jeopardize the health and safety of both patients and staff. In this Review, we discuss how the application of ethical principles can assist dialysis

Challenging behaviour by patients or close contacts, as experienced by healthcare workers in adult general and university hospital care: a scoping review protocol.

INTRODUCTION: Healthcare professionals in general and academic hospitals increasingly face challenging behaviour from patients and their close contacts, particularly in acute care settings. Ranging from aggression to socially inappropriate actions, such behaviour is understood as interactional and context-dependent. Yet research largely focuses on verbal and physical aggression, leaving other forms and hospital context underexplored, despite concerns about staff safety, workforce sustainability and care quality. METHODS AND ANALYSIS: This scoping review aims to map literature on challenging b

Progression from uncomplicated to severe malaria among children in settings receiving different malaria control interventions in sub-Saharan Africa: a systematic review protocol.

BACKGROUND: Different malaria control measures are deployed simultaneously in endemic settings globally, with varying impacts on malaria burden. In sub-Saharan Africa, which bears the greatest burden of malaria, evidence on the impact of implementing various control interventions on malaria immunity remains unknown. This systematic review seeks to collate evidence on the extent of progression from uncomplicated to severe malaria among populations in sub-Saharan Africa settings receiving concurrent deployment of various malaria control measures. METHODS: The review will use a priori criteria c

Randomised controlled trial comparing antibiotic cement bead pouch versus negative pressure wound therapy for the management of severe open tibia fracture wounds: Beads versus VAC (BvV) protocol.

INTRODUCTION: Early open fracture management aims to minimise the risk of complications. For the most severe open fracture wounds, multiple irrigation and debridement surgeries are required to overcome severe wound contamination, to reassess the evolving tissue injury or to temporise and plan further surgery. When multiple irrigation and debridement surgeries are needed, uncertainty remains about how the open fracture wound should be managed to best minimise complications. The primary aim of this trial is to compare the antibiotic cement bead pouch vs negative pressure wound therapy in the man

Key Stakeholders' Perceptions of Motivators for Research Participation Among Individuals Who Are Incarcerated.

Understanding motivations of research participants is crucial for developing ethical research protocols, especially for research with vulnerable populations. Through interviews with 92 institutional review board members, prison administrators, research ethicists, and researchers, we explored key stakeholders' perceptions of what motivates incarcerated individuals to participate in research. Primary motivators identified were a desire to contribute to society, gaining knowledge and health care, acquiring incentives, and obtaining social support. The potential for undue influence or coercion wer

Teaching Responsible Innovation in Endodontics: A Pedagogical Framework for Long-Term Clinical Success: A Narrative Review.

This review explores the responsible innovation concept in endodontics, emphasising its educational implications, biological accountability in clinical outcomes and a framework to guide everyday practice. Responsible innovation in endodontics must transcend mere technical advancement; it demands a comprehensive critical appraisal of practices, ethical awareness and a stewardship of long-term patient outcomes. Educational frameworks that prioritise reflective practice and encourage practitioners to continuously assess and refine their methodologies are essential. Additionally, a focus on biolog

Firmly in the saddle? A scoping review of the role of consultation-liaison clinicians as gatekeepers.

This scoping review examines psychiatric gatekeeping (PG) in somatic care, its prescribed settings and perceived role. Considered were English peer-reviewed articles published between January 1994 and January 2025 with the terms "psychiatric gatekeeper-s" and/or "gatekeeping" in the title, abstract, or full text, addressing the adult population. The initial search retrieved 4398 publications of which 34 were included. Citation searching yielded an additional 12 articles. Most articles discussed PG in gender-affirming medical care (GAMC; N = 31), followed by medical assistance in dying (MAD; N

From guillotine to EEG: Revisiting the neurological controversies of decapitation through modern neurophysiology.

The question of whether consciousness persists after decapitation has fascinated scientists, philosophers, and the public for over two centuries, from 18th-century debates over the guillotine to modern neurophysiological studies of cerebral ischemia. This review synthesizes historical controversies-such as the anecdotal reports of facial movements in severed heads and early galvanism experiments-with modern neurophysiological evidence to reinterpret the mechanisms underlying the loss of consciousness. We trace the evolution of hypotheses from initial philosophical speculations to electroenceph
