The value of ulnar somatosensory evoked potentials (SEPs) in cervical myelopathy.

In 57 patients with clinical signs and surgical documentation of compressive myelopathy, ulnar nerve somatosensory evoked potentials (SEPs) were more sensitive (with 74% abnormal) than either median or tibial nerve SEPs. The most frequent abnormalities were reduced or absent neck evoked responses and prolonged central conduction time. All subjects who had an SEP abnormality were identified by combined tibial and ulnar SEPs. Median nerve SEP added no additional information. Normal ulnar and tibial nerve SEPs were also able to exclude major cord damage in patients with cervical radiculopathy but…

Brain evoked potentials to noxious sural nerve stimulation in sciatalgic patients.

In sciatalgic patients and before any treatment, the goal of this work was to compare the amplitude of the late component (N150-P220) of the brain evoked potential (BEP) between resting pain-free conditions and a neurological induced pain produced by the Lasègue manoeuvre. The study was carried out with 8 inpatients affected with a unilateral sciatica resulting from an X-ray identified dorsal root compression from discal origin. The sural nerve was electrically stimulated at the ankle level while BEPs were recorded monopolarly from the vertex. The stimulus intensity eliciting a liminal nocicep

[Somatosensory evoked potentials of the trigeminal nerve in chronic trichloroethylene poisoning].

Industrial intoxication by trichloroethylene can produce neurological disturbance of the trigeminal nerve. Therefore, among 105 workers exposed to this toxin, we recorded cortical responses after stimulation of the trigeminal nerve (TSEP) and we compared our results with a control group of 52 subjects. We considered the physical examination exploring, the facial sensitivity and reflexes, the levels of urinary metabolites of trichloroethylene and TSEP. We can describe 4 groups of workers: group A, 13 subjects with perturbations of clinical and evoked responses; group B, 27 subjects with normal

Critical neuromonitoring at spinal and brainstem levels by somatosensory evoked potentials.

Electronic averaging makes it possible to analyze somatosensory evoked potentials (SEP) recorded noninvasively from the body surface in man. With noncephalic reference recording, the SEP discloses a series of components that are volume-conducted from distinct open-field generators with a geometry adequate to produce external potential gradients over the head. Farfields are brief positive dips with widespread distribution that present stationary onset and peak latencies all over. They reflect the propagated afferent volley in axons bundles, thus in brachial plexus (P9), dorsal column (P11), and…

Triplicate post-traumatic sciatic nerve palsy: evoked potentials in the diagnosis.

Three episodes of sciatic nerve palsy occurred after open reduction and internal fixation of a fracture-dislocation of the left hip and pelv … prolonged disability and hospitalization were suspected as a possible cause of weakness. Therefore evoked potentials obtained by stimulation of the peroneal nerves were used to aid diagnosis. There was no response from lumbar and cerebral recording sites on stimulation of the peroneal nerve on the affected side. Diagnosis of a conduction block in the sciatic nerve …

Effects of fentanyl and morphine on intraoperative somatosensory cortical-evoked potentials.

We compared the effects of morphine and fentanyl in the presence of 60% N2O on somatosensory cortical-evoked potentials (SCEP). Both drugs were administered by intravenous bolus (n = 12) and infusion (n = 20) techniques. SCEPs were recorded preoperatively and intraoperatively in 32 patients undergoing corrective surgery for scoliosis. Records were taken at the contralateral cerebral cortex by individual stimulation of the posterior tibial nerves at the ankle. Both drugs increased the latencies of the N1, P2, and N2 peaks and affected the peak-to-peak amplitudes of the primary complex. Intraven…

Cortical evoked potentials obtained after stimulation of the lower urinary tract.

Cortical evoked potentials following stimulation of the lower urinary tract have been recorded in humans. Of 26 patients investigated 9 had no urinary signs or symptoms except genuine stress incontinence, while the remaining 17 had various neurogenic bladder disorders. Monopolar and bipolar stimulation of the bladder and urethra was performed using conventional and newly designed suction-type electrodes. While no cortical evoked potentials could be obtained from patients with complete transverse spinal cord lesions polyphasic cortical evoked potentials (N45, P60, N80 and P100 msec.) were recor…

[Effect of cryosurgery of the thalamic nuclei on somatosensory evoked potentials].

Somatosensory evoked potentials were recorded in two groups of patients with extrapyramidal syndromes including: 1) parkinsonian syndromes, 2) spasmodic torticollis. In the first group stereotactic lesions were produced at the borderline between VOp and Vim nuclei, and in the second group they were situated in VOa and VOi thalamic nuclei. Short-latency components of the potentials were analysed assuming that they were the specific parts of the somatosensory potential and their value was doubtless, in contrast to the late latency components. It was found that stereotaxic lesions the VOa and VOp…

Cortical evoked potentials in response to trigeminus nerve stimulation in humans.

A standard technique for recording trigeminal somatosensory evoked potentials (TEP) in response to stimulation of the second branch of the trigeminus nerve is described. Thirty-six nerves of 18 apparently healthy subjects have been investigated. A characteristic polyphasic wave was repeatedly observed. It consisted mainly of a P20, N30, P40, N50 sequence of deflections. Intraindividual variations were minimal. The response pattern varied according to the positions of the recording and stimulating electrodes. The method is useful in detecting functional integrity of the afferent pathways of the…

Pattern electroretinograms and visual-evoked potentials in glaucoma and multiple sclerosis.

Steady-state visual-evoked potentials and electroretinograms were simultaneously recorded in four patients with glaucoma and in five patients with multiple sclerosis. The stimuli included a homogenous field and a 2.3 cycles per degree sinusoidal grating that were counter-phase modulated at the rate of 7.5 Hz. We used narrow bandwidth spectral analysis to measure the response amplitudes and signal-to-noise ratios. Transient pattern visual-evoked potentials (1 Hz) were also measured for latency in each eye. We found abnormal pattern electroretinograms, based on the absence of a significant secon…

Far-field somatosensory evoked potentials after stimulation of the tibial nerve.

In 21 normal subjects, far-field somatosensory potentials were recorded from the scalp after stimulation of the tibial nerve at the ankle (tibial SEP). With the use of a knee reference contralateral to the side of stimulation, the tibial SEP consisted of three major positive peaks, P17, P24, and P31, and three additional but inconsistent components, P11, P21, and P27. Presumable generator sources of the tibial SEP are the popliteal fossa for P11, entry to the sacral plexus for P17, the cauda equina for P21, entry to the conus medullaris for P24, the rostral spinal cord for P27, and the brainst

Multimodality evoked potentials and electrically elicited blink reflex in optic neuritis.

Pattern shift visual evoked potentials, brain stem auditory evoked potentials, spinal and scalp recorded somatosensory evoked potentials, and electrically elicited blink reflexes were investigated in 32 patients with isolated optic neuritis. Eleven patients were shown to have one additional lesion in the central nervous system outside the optic nerve. Therefore, cases with optic neuritis of unknown origin should be considered as possible cases of multiple sclerosis.

[Monitoring of the spinal cord function using evoked spinal cord potentials].

Monitoring of the spinal cord function by evoked spinal cord potentials (ESP) has come to be used widely with the establishment of a safe technique using an epidural electrode and the great improvement in medical electronic devices. According to other investigations ESP has been recorded through an electrode placed in the posterior epidural space, it is well known that residual ESP can be actually recorded after transection of the posterior half of the spinal cord and that ESP shows no change after a small transection of the anterior column. If ESP originating from the anterior half of the spi

Abnormal brainstem auditory evoked potentials and neuropathology in "locked-in" syndrome.

Brainstem auditory evoked potentials (BAEPs) were studied in a "locked-in" syndrome resulting from multiple occlusions of paramedian pontine arteries. Abnormality of BAEPs (first recorded 10 days after infarction) indicated that brainstem damage extended beyond the basis pontis, where, typically, interruption of corticospinal pathways results in the "locked-in" syndrome. Later, partial BAEP recovery suggested that the technique can be used to distinguish between permanent and transient brainstem involvement. Autopsy examination revealed close correspondence between permanent brainstem damage a…

Somatosensory evoked potentials in diagnostics of cervical spondylosis and herniated disc.

Somatosensory evoked potentials (SEPs) were recorded in 16 healthy controls and 12 patients suffering symptoms typical of cervical spondylosis and/or herniated disc and showing abnormal findings both in clinical and myelographic examinations. SEP recordings were performed at 3 different levels of the nervous system, at Erb's point, over the 7th cervical spine and at the inion, by stimulating the median nerve separately at both wrists. Nine of the 12 patients were chosen for surgical treatment, while 3 patients received physiotherapy. Six out of the 12 patients showed abnormal findings in SEP r…

Visual evoked potentials in normal subjects and patients with multiple sclerosis.

Visual evoked potentials (VEPs) by checkerboard pattern-reversal stimulation were recorded in 70 subjects aged 10--69 years and in 100 patients with definite, probable or possible multiple sclerosis (MS). Longer latencies and smaller amplitudes of the major positive component were found in male subjects, in old subjects and when the amplifier's band-pass was narrowed. Subjects 10--14 years old had longer latencies and higher amplitudes than mature adults. Based on findings in the normal material, the following three criteria were used in evaluating the recordings from patients: the latency, th…

[Using brainstem auditory evoked potentials in cerebellopontine angle tumors (author's transl)].

The brainstem electric responses on acoustic stimulation are recorded on 37 patients with verified cerebellopontine angle tumors. These responses are compared with those of 101 normal hearing young adults and those of 47 patients with cochlear hearing loss. The Interval between the latency of the first wawe (acoustic nerve) and the fifth (midbrain) or I-V delay is related to the lesion location. The I-V delay is increased in retro-cochlear hearing loss and not different of the normal in cochlear hearing loss. The delay variation is best seen by comparing the I-V delay for the two ears. The int
