In this study, we report four cases of Histoplasma capsulatum infection in eight biologists who made an expedition to determine the prevalence of this fungus in a cave localized in the northwest of Minas Gerais state, Brazil. This case report demonstrates the importance of evaluating the H. capsulatum presence in Brazilian caves before opening to public visitations.

OBJECTIVES: Dysvascular patients account for >80% of major amputations in the US. We sought to determine if early mobilization and discharge disposition decreased post-operative hospital length of stay (PO-LOS) and expedited independent ambulation.
METHODS: A retrospective review of dysvascular patients undergoing major amputations was performed. Primary outcomes included PO-LOS, discharge disposition, and days to ambulation.

Histoplasmosis is a rare disease in the Cape Province. The diagnosis of acute disseminated histoplasmosis in a patient who presented at Groote Schuur Hospital 3 weeks after a caving expedition, led to the identification of acute benign pulmonary histoplasmosis in 9 other people who had explored the same cave. Possible explanations for this outbreak are discussed.

We evaluated the use of the T2Bacteria panel for diagnosis of bloodstream infection in pediatric HCT/CT patients. Overall, the panel expedited recognition of bloodstream infections, providing results sooner than Gram stain and final blood culture identification. The T2Bacteria panel contributed to a change to appropriate therapy in 23% of patients with concordant positive results.
