[Classification and osteosynthesis technique of calcaneus fractures. External fixator as temporary distractor].

In the treatment of fractures of the calcaneus, the particularly intricate local anatomy, complicated fracture forms and associated soft tissue damage often prejudice operative, anatomical reconstruction. We propose a simplified classification that is based on the Regazzoni classification of 1993 and has six grades of severity. It can be helpful in the selection of operative treatment and, above all, make it possible to recognize whether operative reconstruction is possible and appropriate. When operative reconstruction is indicated we find the secondary operation important; it is also importa

Trapezio-lunate external fixation for scaphoid fractures. An experimental and clinical study.

Trapezio-lunate external fixation for scaphoid fractures has been investigated experimentally and clinically. Six below-elbow fresh amputation specimens in which proximal, waist and distal scaphoid fractures were created, displaced, reduced and externally fixed were used for an experimental study and after 10,000 extension-flexion movements of the wrist, no displacement was observed. A strain-gauge study confirmed the strains at the fracture site. Seven patients were treated in this way. No immobilization was necessary and all patients returned to their regular employment within 3 days and all

Post-traumatic osteoporosis and algodystrophy after external fixation of tibial fractures.

The incidence of two forms of post-traumatic osteoporosis and algodystrophy in the ankle region of 20 patients with displaced diaphyseal fractures of the tibia treated with external fixation were studied. Acute localised osteoporosis (ALO) occurred in 14 (70 per cent) and 5 (25 per cent) developed algodystrophy. ALO was significantly less frequent in matched patients treated by internal fixation or by external plaster-of-Paris (POP) cast. Regional osteoporosis (RO) was most common in POP patients. The presence of ALO may indicate the development of algodystrophy following tibial fracture. Our

[External fixation of distal radius fractures: indication, technique and functional results].

A series of 56 consecutive patients with 59 fractures of the distal end of the radius, treated with an unilateral external fixator of Hoffmann between 1980 and 1988, is presented, 47 of these patients were examined clinically at home in the last months of 1989. We can distinguish two different groups of patients: the older females with an average of 59.2 years and the younger males with an average of 40.1 years. The fractures of the females were caused by a lower energy trauma than those of the males (p less than 0.05). The males had significantly more complex fractures (Frykman type VII or VI

External fixation of Smith's fracture. 16 patients followed for 2 years.

External fixation of 16 Smith's fractures was evaluated after 22 (6-42) months. In 15 patients, the radiographic results were satisfactory. There was a definite correlation between the radiographic and clinical end results. However, 6 patients had residual signs of an upper limb dystrophy. Although external fixation of Smith's fractures results in good function, this method should be used with caution because it cannot prevent reflex dystrophy; indeed, it may even initiate it.

[Special utilization potentials of the external fixator in severe complex hand injuries].

Complex injuries to the hand may cause considerable problems in the aftertreatment. Two possibilities are described, which may facilitate and optimize the postoperative care of the severely injured hand: 1. In case of an inguinal pedicle flap, the temporary stabilization of the hand against the pelvis by means of external fixation is advocated. 2. After multiple extensor tendon repair, the temporary immobilization of the wrist, leaving the fingers free to move, by means of a small fixateur externe is suggested.

Comminuted fractures of the base of the first metacarpal treated by distraction-external fixation.

A new technique is presented for the treatment of comminuted intraarticular fractures of the base of the thumb metacarpal. A triangular Hoffmann external radius and first- and second-metacarpal fixator is used. The reduction is maintained by prolonged distraction and abduction of the first metacarpal bone. The application of the technique in three cases is reported.

[Treatment of intra-articular metacarpal and phalangeal fractures with an external fixation minidevice].

Open reduction of intraarticular fractures involving the metacarpals and phalanges and fixation with the small external fixation apparatus is a promising new method which fulfills a need. Full return of function in seven patients and a small limitation of function in three patients can be regarded as a satisfactory result. All patients returned to work after a short time without any complications. The external mini-fixation device which has already been useful in the treatment of open and closed fractures, infected nonunions and in performing arthrodeses has now found another application in th

Soft-tissue reconstruction of the leg associated with the use of the Hoffmann external fixator.

Soft-tissue injuries and defects associated with severe fractures of the tibia treated with the Hoffmann external fixator were reviewed. Of 84 cases of open tibia fractures (classified as Gustilo's Type II and III), 54 patients with 55 injured limbs required soft-tissue reconstruction other than skin grafts or small rotational flaps. These 55 cases were constructed with 37 local musculocutaneous flaps (20 gastrocnemius, 17 soleus and other smaller muscle flaps), 15 free musculocutaneous flaps (14 latissimus dorsi, and one tensor fascia lata), and three medial gastrocnemius cross leg flaps. [Possibility of fixation of a compound fracture of the 1st metacarpal bone with the minifixateur externe].

Comminuted fractures of the base of the first metacarpal are difficult to stabilize with internal fixation or with plaster casts. Use of the external mini-fixator can provide good results. Complications such as pin-infections were not seen after the use of short duration of this apparatus in closed fractures.

External fixation is not superior to K-wire fixation in pediatric patients with high-level extension-type supracondylar humeral fractures.

OBJECTIVES: Supracondylar humerus fractures (SHFs) are among the most common fractures in pediatric patients. The following two surgical options are available for high-level extension-type pediatric supracondylar humeral fractures: Kirschner wire (K-wire) fixation and lateral external fixation. Evidence comparing the clinical outcomes of these two procedures remains insufficient. This study aimed to compare functional and radiological outcomes between the two surgical approaches. METHODS: This retrospective comparative study included 52 pediatric patients (15 treated with external fixation an

Clinical and Radiological Outcomes of Schatzker Type V and VI Tibial Plateau Fractures Treated With Internal Fixation and External Fixation.

INTRODUCTION: Tibial plateau fractures are complex injuries involving the articular surface of the proximal tibia and often accompanied by soft-tissue damage. These fractures are challenging due to their potential to disrupt knee joint stability and function. Schatzker classification is widely used to categorize these fractures globally. Several different treatment modalities, focused on stabilizing the knee joint while restoring pain-free motion, are used for treatment. Schatzker type V and VI tibial plateau fractures are treated with two different approaches: open reduction and internal fixa

Circular External Fixator Assisted Open Reduction Combined With Locking Plate Fixation for Intra-articular Comminuted Fractures of the Calcaneus.

Comminuted fractures of the calcaneus are relatively common and generally require surgical treatment. The quality of fracture reduction is crucial. The extended lateral approach (ELA) can better expose the fracture end and facilitate the reduction of the fracture, while it has a higher risk of postoperative skin complications. In this study, the ELA was adopted, and the calcaneal comminuted fractures were treated with circular external fixator assisted reduction to achieve the purpose of good reduction of the fracture and fewer skin complications. During 64 months, a total of 61 cases of unila
