Too Much of a Good Thing: Updated Current Management and Perinatal Outcomes of Polyhydramnios.

Amniotic fluid assessment is crucial in prenatal ultrasound to monitor fetal conditions, with polyhydramnios, characterized by excessive amniotic fluid, affecting 1%-2% of pregnancies. Polyhydramnios is linked to complications such as placental abruption, preterm labor, congenital anomalies, and postpartum hemorrhage, emphasizing the need for early detection and management. While idiopathic causes account for 60%-70% of cases, other causes include impaired fetal swallowing and increased urine production due to maternal, fetal, and placental conditions. Accurate amniotic fluid volume (AFV) asse

Fetal extrasystole may predict poor neonatal outcome.

Extrasystoles particularly premature atrial contractions noted during labour on the fetal heart rate monitoring strip are usually thought to be benign. In pregnancies complicated by fetal infection and/or the fetal inflammatory response syndrome, there are some data that extrasystoles noted during the intrapartum period may be related to neonatal sepsis and eventual poor neonatal outcome including death or neonatal encephalopathy. Additional observations are needed to substantiate this hypothesis.

Intrauterine rupture of the umbilical cord during delivery.

Two cases of bleeding from ruptured umbilical vein during delivery are reported. In one case the rupture was spontaneous, leading to fetal distress. In the second case the rupture was probably iatrogenic, and was caused by forceps delivery. Bleeding from ruptured umbilical vessel should be considered when a combination of variable decelerations during fetal heart rate monitoring, blood-stained amniotic fluid and fetal distress are detected during delivery.

Intrapartum fetal heart rate patterns preceding terminal bradycardia in infants (>34 weeks) with poor neurological outcome: A regional population-based study in Japan.

AIM: Intrapartum fetal bradycardia necessitates immediate operative delivery. Our aim was to investigate the hypothesis that some non-reassuring fetal heart rate (FHR) patterns were present before the onset of terminal bradycardia in infants who developed subsequent brain damage. MATERIAL AND METHODS: From a population-based study of 65,197 deliveries, 190 stillbirths, 115 neonatal deaths, and 136 neurologically high-risk infants were registered by the Miyazaki Perinatal Conference. There were 15 cases of neurologically high-risk infants born at >34 weeks of gestation exhibiting intrapartum t

Midwives' and obstetricians' knowledge and management of women presenting with decreased fetal movements.

BACKGROUND: Maternal perception of decreased fetal movements (DFM) affects 5-15% of pregnancies. DFM is associated with intra-uterine fetal death (IUFD) and intra-uterine growth restriction (IUGR). It has been proposed that maternal perception of DFM may be used as a screening tool for IUFD or IUGR. However, this proposal is complicated by variations in definitions and management of DFM. HYPOTHESIS: We hypothesised that uncertainties in the definition and management of women presenting with DFM leads to variation in clinical practice. METHODS: A postal questionnaire was sent to midwives and

The role of antepartum surveillance in the management of gastroschisis.

OBJECTIVE: To evaluate the perinatal morbidity and mortality of fetuses diagnosed with gastroschisis at our Fetal Diagnosis and Treatment Center. METHODS: A retrospective review of a regional prenatal diagnostic center. Twenty-nine cases of gastroschisis which were diagnosed, managed, delivered and had corrective surgeries through the Fetal Diagnosis and Treatment Center were identified from 1985 to 1994. Perinatal morbidity and mortality were reviewed. Antepartum testing schemes were reviewed when available to determine whether morbidity or mortality could have potentially been prevented. R

Critical care obstetrics: management of the fetus.

Whenever a critically ill gravida presents for care, consideration should be given to consulting an obstetrician or a maternal-fetal medicine specialist. The technological advances of the past two decades permit these critical care obstetrical specialists to immediately assess fetal health with ultrasonography or a fetal monitor. By so doing, maternal and fetal care can be optimized through a team approach.

Fetal arrhythmias: Surveillance and management.

Fetal arrhythmias warrant sophisticated surveillance and management, especially for the high-risk pregnancies. Clinically, fetal arrhythmias can be categorized into 3 types: premature contractions, tachyarrhythmias, and bradyarrhythmias. Fetal arrhythmias include electrocardiography, cardiotocography, echocardiography and magnetocardiography. Oxygen saturation monitoring can be an effective way of fetal surveillance for congenital complete AV block or SVT during labor. Genetic surveillance of fetal arrhythmias may facilitate the understanding of the mechanisms of the arrhythmias and provide th

Antepartum fetal heart rate pattern associated with major congenital malformations.

Antenatal fetal heart rate monitoring of 20 fetuses with major congenital malformations revealed loss of long-term variability in 11 (55%) and an isolated, abrupt-onset fetal heart rate deceleration in 13 (65%). In ten (50%), loss of variability coexisted with periodic fetal heart rate decelerations. These fetal heart rate changes were significantly more prevalent in the malformed group than in a control population. There was also a significantly increased incidence of fetal distress in labor and in the requirement for primary cesarean section delivery. Perinatal mortality was 75%, reflecting

Cardiac arrhythmias in the human fetus.

Fetal cardiac arrhythmias have been recognized with increasing frequency during the past several years. Most fetal arrythmias are intermittent extrasystoles, often presenting as irregular pauses of rhythm. These are significant only when they occur with appropriate timing to initiate sustained tachycardia, mediated by anatomic bypass pathways. The most common important fetal arrhythmias are: 1) supraventricular tachycardias, and 2) severe bradyarrhythmias, associated with complete heart block. Symptomatic fetal tachycardias are usually supraventricular in origin, and may be associated with the

Asphyxia.

The prevention of fetal asphyxia or hypoxia starts with prepregnancy counseling and continues with careful antenatal care and intrapartum fetal surveillance. Further progress in eliminating antepartum and intrapartum deaths will only be made when it is accepted that, even with intense investigation by detailed autopsy, the cause of many deaths remains unknown. Many of these deaths may be ascribed to hypoxia. In the future, with more detailed non-invasive probing with CAT scanning and magnetic resonance imaging, other causes may be determined. The mother at risk of hypoxia requires specialized

[Experiences in the assessment of the subpartal cardiotocogram (author's transl)].

750 cardiotocographic curves of the last 30 minutes prior to vaginal delivery were assessed retrospectively according to the CTG score after Hammacher et al. (6). The aim of this study was to examine the prognostic value of the CTG score with regard to the foetal acid-base stains. The results obtained showed a distinct interdependence between the CTG score and the risk of foetal acidosis. Correct interpretation of the CTG can be ensured only if all three different CTG parameters are considered. The most frequent and reliable pointers to the existence of foetal acidosis are the occurrence of se

Use of computers in the labor and delivery suite: an overview.

Some simple computers for fetal monitoring are already commercially available. These systems are designed to alarm when abnormalities reflected by the fetal heart are detected during labor. Unfortunately, the problems of fetal heart rate/intrauterine pressure data analysis have been oversimplified. An "ideal" obstetric computer-based data management system would begin, as the clinician should, with the assessment of risk from the patient's past history, pregnancy course, and labor progress as a basis for analyzing the condition of the fetus during labor. Systems available now are of limited sc…

Fetal thyrotoxicosis: a case report and recommendations for prediction, diagnosis, and treatment.

A maternal history of Graves' disease places the fetus at risk for thyrotoxicosis in utero via the placental transfer of thyroid-stimulating immunoglobulins. Methods for prediction of fetal hyperthyroidism are available, but are not widely used. Clinical assessment of fetal thyroid status by monitoring of fetal heart rate and growth may be inaccurate. This raises some uncertainty in the initial diagnosis of fetal thyrotoxicosis and complicates the assessment of fetal response to maternal propylthiouracil therapy. A case illustrating these pitfalls in the diagnosis and management of fetal hyper

The utility of fetal heart rate deceleration's descending slope in searching for a non-National Institute of Child Health and Human Development parameter for the detection of fetal acidosis.

OBJECTIVE: To identify new parameters predicting fetal acidemia. METHODS: A retrospective case-control study in a cohort of deliveries from a tertiary referral hospital-based cohort deliveries in Zaragoza, Spain between 2018 and 2021 was performed. To predict fetal acidemia, the NICHD categorizations and non-NICHD parameters were analyzed in the electronic fetal monitoring (EFM). Those included total reperfusion time, total deceleration area and the slope of the descending limb of the fetal heart rate of the last deceleration curve. The accuracy of the parameters was evaluated using the speci…

[Value and significance of obstetric monitoring methods].

In a retrospective study on 224 births, 200 continuous tracings recorded during intrapartum cardiotokography were analysed for the presence of alarm signals and the incidence of fetal acidosis assessed by fetal blood sampling. Moreover, we classified the 200 monitored parturitions according to the presence of risk at the beginning and/or the appearance of alarm signals during labour into 4 groups. In 17% of the parturitions without initial risk alarm signals were subsequently recorded on CTG, whereas alarm signals occurred in 35% of high-risk births. In both groups fetal distress, as assessed
