The impact of donor transition on continuity of maternal and newborn health service delivery in Rwenzori sub-region of Uganda: a qualitative country case study analysis.

BACKGROUND: The transition of donor-supported health programmes to country ownership is gaining increasing attention due to reduced development assistance for health globally. It is further accelerated by the ineligibility of previously Low-Income Countries' elevation into Middle-income status. Despite the increased attention, little is known about the long-term impact of this transition on the continuity of maternal and child health service provision. Hence, we conducted this study to explore the impact of donor transition on the continuity of maternal and newborn health service provision at

Global health 2050: the path to halving premature death by mid-century.

Global health 2050 (GH2050), a new report from the Lancet Commission on Investing in Health, finds that dramatic improvements in human welfare are achievable by mid-century with focused health investments. By 2050, countries that choose to do so can halve their probability of premature death (PPD)—the probability of dying before age 70—from their pre-pandemic level in 2019. We call this goal “50 by 50”: a 50% reduction in PPD by 2050. The interventions for achieving “50 by 50” will also reduce morbidity and disability at all ages. Historical experience and continued scientific advance indicate

Impact of Socioeconomic Status, Ethnicity, and Urbanization on Risk Factor Profiles of Cardiovascular Disease in Africa.

Africa is a continent characterized by marked ethnic, sociodemographic, and economic diversity, with profound changes in many regions over the past 2 decades. This diversity has an impact on cardiovascular disease presentation and outcomes. Within Africa and within the individual countries, one can find regions having predominantly communicable diseases such as rheumatic heart disease, tuberculous pericarditis, or cardiomyopathy and others having a marked increase in noncommunicable disease such as hypertension and hypertensive heart disease. Ischemic heart disease remains rare in most countri

Developing a national HIV/AIDS Prevention Program through state health departments.

The Centers for Disease Control (CDC) shaped the basic development and direction of the HIV/AIDS Prevention Program through technical support and financial assistance for State and local health departments and other organizations. Through this provision of support, CDC has responded to the course of the human immunodeficiency virus (HIV)-acquired immunodeficiency syndrome (AIDS) epidemic by creating programs to preserve the safety of the blood supply, by developing counseling and testing centers, by promoting "safer sex," by promoting health education and risk reduction, by evaluating existing

Polio transition in the African Region: progress, challenges and prospects for health systems integration.

BACKGROUND: The Global Polio Eradication Initiative (GPEI) has made substantial progress towards global eradication of polio. As regions, including Africa, approach the final stages of this goal, countries must ensure that essential polio functions are sustained and effectively integrated into national health systems. To support this transition, the World Health Organization(WHO), through its regional offices, has guided countries using the Polio Transition Global Vision and its Monitoring and Evaluation Framework. This paper reviews progress made in transition planning across 15 polio transit

Cost of health systems strengthening for small and sick newborn care in four sub-Saharan African countries implemented with NEST360: incremental cost analyses.

BACKGROUND: Newborn survival requires high-quality small and sick newborn care (SSNC). Domestic and donor financing for SSNC is low, yet real-world cost data to inform investment are scarce. We analysed primary financial data for SSNC health system improvements nationally in Malawi and subnationally in Kenya, Nigeria, and Tanzania. METHODS: We analysed incremental health system perspective costs using hospital-specific assessments, inpatient data (n=93 942 admissions per year), and government and NEST360 costs. We compared annual hospital-specific costs post-implementation (June, 2022, to Jun

Preventing Dental Caries in Indonesia: A Scoping Review of Policies and National Initiatives.

BACKGROUND: Dental caries remains a significant public health issue in Indonesia, with high prevalence across age groups and ongoing disparities in access to preventive care. While national initiatives such as the 'Caries-Free Indonesia by 2030' goal signal increasing political attention, the effectiveness and equity of current oral health strategies remain uncertain. OBJECTIVE: This scoping review aims to assess the policy direction and documented effectiveness of Indonesia's national-level caries prevention programmes through a review of government policy documents and relevant literature.

Health workforce development and retention in Guinea: a policy analysis post-Ebola.

BACKGROUND: The state of the Guinean health workforce is one of the country's bottlenecks in advancing health outcomes. The impact of the 2014-2015 Ebola virus disease outbreak and resulting international attention has provided a policy window to invest in the workforce and reform the health system. This research constitutes a baseline study on the health workforce situation, professional education, and retention policies in Guinea. The study was conducted to inform capacity development as part of a scientific collaboration between Belgian and Guinean health institutes aiming to strengthen pub

User fees and maternity services in Ethiopia.

OBJECTIVES: To examine user fees for maternity services and how they relate to provision, quality, and use of maternity services in Ethiopia. METHODS: The national assessment of emergency obstetric and newborn care (EmONC) examined user fees for maternity services in 751 health facilities that provided childbirth services in 2008. RESULTS: Overall, only about 6.6% of women gave birth in health facilities. Among facilities that provided delivery care, 68% charged a fee in cash or kind for normal delivery. Health centers should be providing maternity services free of charge (the healthcare fin

Impact of government subsidy reforms on primary health care efficiency in rural eastern China: an interrupted time series analysis.

BACKGROUND: Despite rising government subsidies, inefficiencies persist in China’s primary health-care system. We evaluated an eastern rural pilot that shifted subsidies from passive grants to strategic purchasing via fixed salaries plus activity-based payments using resource-based relative value units (RVUs). METHODS: Interrupted time series (January 2015 - December 2022) assess the instantaneous and long-term effects of the reform (implemented January 2017) on monthly RVUs per employee across all primary health institutions (PHIs) and three sub-categories (central town/street PHIs, other PHI

Comparing National Institutes of Health funding for cancer survivorship: a spotlight on breast and gynecologic cancer.

BACKGROUND: Cancer survivorship in the United States is rapidly rising, with a projected 50% increase by 2050, driven by improvements in treatment and increased incidence of cancers such as endometrial cancer. Along with this growth, long-term side effects create significant burdens, yet survivorship research, particularly for female-organ cancers, remains limited and imbalanced - with breast cancer studies far outnumber those for gynecologic cancers. To understand these disparities, funding patterns for survivorship research between disease sites, while accounting for differences in disease p

Allergy and immunology in Africa: Challenges and unmet needs.

The tremendous increase in allergy in the African continent cannot simply be explained by the change in public hygiene. There are many "prehygiene" communities with sewage-contaminated water supplies, helminth infestations, bare footedness, and poor housing, and still there is a high prevalence of allergic disease. Africans can be exposed to many risk factors facilitating severe asthma and wheezing, including airborne viruses, smoke, indoor dampness, cockroaches, and poor access to health care. Although the reporting on food allergy is inadequate to perform systematic reviews or meta-analyses,

Toward the prevention of childhood undernutrition: diet diversity strategies using locally produced food can overcome gaps in nutrient supply.

BACKGROUND: Chronic undernutrition is prevalent in Mozambique, where children suffer from stunting, vitamin A deficiency, anemia, and other nutrition-related disorders. Complete diet formulation products (CDFPs) are increasingly promoted to prevent chronic undernutrition. OBJECTIVE: Using linear programming, to investigate whether diet diversification using local foods should be prioritized in order to reduce the prevalence of chronic undernutrition. METHODS: Market prices of local foods were collected in Tete City, Mozambique. Linear programming was applied to calculate the cheapest possibl

Pushing the employment agenda: case study research of high performing States in integrated employment.

Organizational variables, including policies, practices, collaborations, and funding mechanisms resulting in high performance in integrated employment, were described through case study research in 3 states. Findings address how contextual factors, system-level strategies, and goals of the system are related as well as how they sustain systems change. Strategies such as flexibility in funding and practices; communication of values through data, rewards, and funding incentives; and innovation diffusion through relationships and training were most successful when they were embedded within the co
