Evaluation of Educational Activity with Guatemalan Birth Attendants About Helping Babies Breathe.

Introduction: Guatemala's Infant Mortality Rate was 20th highest in the world at 18 deaths per 1,000 live births in 2022. Part of the problem is lay midwives (LMs) who attend most births at home lack training in helping babies breathe (HBB). LMs transfer babies struggling to breathe to firefighters (FF). The aims of this mixed-methods pilot study were to evaluate a HBB educational activity, explore LM and FF's lived experience HBB, and to assess if the HBB educational activity changed participants' knowledge. Methods: Twelve LMs and six FFs (N = 18) participated in focus group discussions, com…

Ecotoxicity Evaluation of Fire-Extinguishing Water from Large-Scale Battery and Battery Electric Vehicle Fire Tests.

Electrified transport has multiple benefits but has also raised some concerns, for example, the flammable formulations used in lithium-ion batteries. Fires in traction batteries can be difficult to extinguish because the battery cells are well protected and hard to reach. To control the fire, firefighters must prolong the application of extinguishing media. In this work, extinguishing water from three vehicles and one battery pack fire test were analyzed for inorganic and organic pollutants, including particle-bound polycyclic aromatic hydrocarbons and soot content. Additionally, the acute tox…

Mortality among Fire Department of the City of New York Rescue and Recovery Workers Exposed to the World Trade Center Disaster, 2001-2017.

The World Trade Center (WTC) attacks on 9/11/2001 have consistently been associated with elevated rates of physical and mental health morbidities, while evidence about mortality has been limited. We examined mortality between 9/12/2001 and 12/31/2017 among 15,431 WTC-exposed Fire Department of the City of New York (FDNY) firefighters and emergency medical service providers (EMS), specifically assessing associations between intensity of WTC-exposure and mortality risk. Standardized mortality ratios (SMR) and 95% confidence intervals (CI) compared FDNY cohort mortality with the US general popula…

The Forgotten Responders: The Ongoing Impact of 9/11 on the Ground Zero Recovery Workers.

In the years following the September 11, 2001 terrorist attacks (9/11; New York USA), emergency first responders began experiencing a range of physical health and psychosocial impacts. Publications documenting these tended to focus on firefighters, while emerging reports are starting to focus on other first responders, including paramedics, emergency medical technicians (EMTs), and police. The objective of this research was to explore the long-term impact on another important group of 9/11 responders, the non-emergency recovery workers who responded to the World Trade Center (WTC) site of the …

SFFO-RL-intelligent environmental monitoring: smart sensing networks with artificial intelligence.

Rapid urbanization has led to increasingly severe environmental degradation, necessitating the development of intelligent, adaptable, and explainable monitoring systems for sustainable urban management. Existing modeling frameworks are limited by problems related to sensor noise, data inconsistencies, poor adaptability, and a lack of explanation of how models arrive at their predictions. This study proposes a Satin Firefighter Optimization-Reinforcement Learning (SFFO-RL) model within an Intelligent Environmental Monitoring Framework (IEMF) to enable accurate, interpretable environmental asses

Acute Toxicity of Commercial Wildfire Retardants to Two Daphniid Species (Ceriodaphnia dubia and Daphnia magna).

In the face of global climate change, there has been an increase in wildfires around the world, highlighting the need for improved firefighting techniques, such as the use of fire retardants (FRs). These products can enter aquatic systems directly or through runoff, posing potential risks to aquatic biota. In this study, the acute toxicity (24-h/48-h EC50) of three distinct FRs (N-Borate, N-Phosphate+, and N-Phosphate-) was assessed on the immobility of freshwater microcrustaceans Ceriodaphnia dubia and Daphnia magna. The toxicity of the FRs varied up to two orders of magnitude, all of which p

Fluctuation and extinction of laminar diffusion flame induced by external acoustic wave and source.

Acoustic wave can destabilize the flame and has a potential in firefighting, but the influences of the sound source and its frequency are still poorly understood. This work applies a loudspeaker to extinguish a laminar diffusion propane flame of 5-25 mm high, where the local sound frequency is 50-70 Hz and sound pressure is 0.8-3.2 Pa (92.0-104.1 dB). Results reveal a constant flame pulsating displacement at the extinction limit, independent of the sound environment used. Such a flame pulsating displacement is found to be caused by the motion of the speaker membrane (or diaphragm) and its indu

Psychological distress and post-traumatic symptoms following occupational accidents.

Depression and post-traumatic stress disorder frequently occur as a consequence of occupational accidents. To date, research has been primarily focused on high-risk workers, such as police officers or firefighters, and has rarely considered individuals whose occupational environment involves the risk of severe, but not necessarily life-threatening, injury. Therefore, the present study was aimed at assessing the psychological consequences of accidents occurring in several occupational settings (e.g., construction and industry). Thirty-eight victims of occupational accidents (injured workers) an…

Discovery of Previously Unreported Per- and Polyfluoroalkyl Substances (PFAS) in Pooled Australian Human Serum Using Nontarget Analysis.

Humans may be exposed to thousands of per- and polyfluoroalkyl substances (PFAS), yet only a small fraction of these are regularly monitored, potentially underestimating the associated risks. This study employed high-resolution mass spectrometry (HRMS)-based nontarget analysis to investigate previously unreported PFAS in pooled serum from Australians. Samples collected between 2002 and 2021 from men aged > 30 years in the general population (GP, n = 27) and firefighters (FF, n = 18), categorized into three age groups (31-45, 46-60, and ≥ 60), were analyzed. Fourteen PFAS were identified or str…

Experiences/needs of US first responders, family caregivers, and children after TBI/catastrophic injury.

PURPOSE: First responders (law enforcement officers, firefighters, and emergency medical personnel) are at high risk for traumatic brain injury (TBI) and other catastrophic injury (CI) requiring long-term caregiver support. The aim of this pilot survey was to investigate, via caregiver perception, the lived experiences and needs of: (1) US first responders who experienced TBI/CI, (2) the caregivers themselves, and (3) children living in the home. MATERIALS AND METHODS: Caregiver participants (n = 27) completed an online survey including closed and open-ended questions. RESULTS: Results show …

Traumatic Events, Psychopathology, and Post-Traumatic Stress Disorder in the General Community and First Responders: Presence of Symptoms and Associated Factors.

Most individuals experience at least one traumatic event during their lifetime, which can lead to the development of psychopathological symptoms and Post-Traumatic Stress Disorder (PTSD). First responders (e.g., police officers, firefighters, emergency medical professionals) are exposed to traumatic events daily, making them particularly vulnerable to developing such symptoms. Using an online questionnaire, this study aimed to compare self-reported exposure to traumatic events and the presence of psychopathological and PTSD symptoms between a sample from the general community (n = 137) and fir…

Helping Frontline Workers in Texas-A Framework for Resource Development.

First responders disproportionately experience occupational stress when compared to the general population, and COVID-19 has exacerbated this stress. The nature of their duties as law enforcement officers, firefighters, and medics exposes them to repeated trauma, increasing their risk of developing a broad array of mental health issues, including post-traumatic stress disorder (PTSD), substance use disorder (SUD), and compassion fatigue. This paper describes the need for resources for frontline workers and provides a framework for creating and implementing resources. A team of interdisciplinar…

Exogenous Ketones as Therapeutic Signaling Molecules in High-Stress Occupations: Implications for Mitigating Oxidative Stress and Mitochondrial Dysfunction in Future Research.

High-stress occupations (ie, firefighters, military personnel, police officers, etc.) are often plagued by cardiometabolic diseases induced by exposure to chronic stressors. Interrupted sleep cycles, poor dietary patterns, lack of physical activity, and smoke exposure along with simultaneous psychological stressors promote chronic low-grade inflammation and excessive oxidative stress. Collectively, these data suggest that practical interventions which might mitigate the underlying pathologies of these cardiometabolic diseases are warranted. Ketones, specifically R-βHB, modulates intracellular …

Pilot survey of urinary porphyrins from persons transiently exposed to a PCB transformer fire.

In rats and humans chronically exposed to large amounts of PCB (polychlorinated biphenyls) and PCDF (polychlorinated dibenzofurans), the urinary excretion of uroporphyrin and coproporphyrin are altered. However, porphyrin excretion in humans after acute transient low level exposure has not been evaluated. Following such an exposure in which bystanders and firefighters were in contact with smoke from a PCB transformer fire, we surveyed 90 self-referred individuals by questionnaire and by determining single 24 hour urinary excretion of uroporphyrin and coproporphyrin 2-4 weeks after the fire. Qu…

We Should Celebrate, Not Censor, Learning From Epidemiologic History.

The controversy over whether repeated head impact (RHI)-a feature of occupations including professional contact sports, military service, firefighting, and logging-can cause the neurodegenerative disease now known as CTE (chronic traumatic encephalopathy) has thrust many positive epidemiologic studies into the spotlight. Various skeptics who dispute that the relationship is strong and causal continue to raise objections to these studies and their interpretation. The arguments these skeptics use remind other observers of many past sagas of "manufactured doubt," particularly the history of attem
