Prenatal assessment of foot deformity.

Congenital talipes equinovarus (CTEV) and other skeletal abnormalities can be diagnosed prenatally by ultrasound scan. If diagnosed, clinicians should be cautiously optimistic with the parents, particularly if the talipes diagnosed is isolated and not associated with other foetal abnormalities. There is no correlation between the prenatal appearance of the foot and the state of the foot at birth, nor with the necessity for surgical intervention.

Foot malignancies: a multidisciplinary approach.

Malignant osseous and soft tissue tumors of the foot are rare and should be considered in patients who have foot-related symptoms. Most sarcomas affect patients who are older than 55 years; however, many young patients who are otherwise healthy, may present with malignant foot and ankle lesions. In addition to benign and malignant neoplasms, pseudotumorous conditions mimic neoplastic lesions and should be differentiated before any treatment is undertaken. Invasiveness is a characteristic of sarcomas. The foot is a terminal anatomic structure, with closed and tight compartments and well-vascula…

[Diabetic neuropathic osteoarthropathy (Charcot foot)].

Diabetic neuropathic osteo-arthropathy (DNOAP; Charcot's foot) is a dramatic complication of diabetic polyneuropathy. The diabetic foot is the most neglected long-term sequela of diabetes mellitus, and this is especially true for DNOAP. The increasing number of diabetics presenting in the foot ambulance with DNOAP demonstrates this general lack of knowledge and indicates that this complication is more frequent than hitherto assumed. In view of this dilemma, the pathogenesis, diagnosis and therapy of DNOAP are reviewed. Special emphasis is given to the differentiation between the neuropathic an…

Hand, foot and mouth disease.

BACKGROUND: Hand, foot and mouth disease is a common viral illness of infants and children. OBJECTIVE: This article aims to describe the cause, presentation, management and infectivity of this virus. DISCUSSION: Hand, foot and mouth disease is characterised by fever and vesicles in the mouth and on distal extremities. It is usually caused by coxsackie A virus and less commonly by coxsackie B and enterovirus 71. Hand, foot and mouth disease usually affects children under 10 years of age. Enteroviruses may also cause more serious disease such as meningoencephalitis and myocarditis. Treatment i…

Cavus foot deformity in children.

A cavus deformity of the foot is easily recognizable, but appropriate neurologic assessment can help to determine the etiology. Cavovarus, the most frequent type of cavus foot, presents with an elevated medial longitudinal arch, first ray plantarflexion, and, if rigid, a fixed heel varus. Common causes include progressive motor sensory conditions, typically Charcot-Marie-Tooth disease, and nonprogressive conditions such as cerebral palsy and poliomyelitis. A calcaneocavus foot may be seen in poliomyelitis, spinal dysraphism, and peripheral neuropathy. Initially, the cavus deformity is flexible…

Over-the-counter foot remedies.

Several effective and inexpensive over-the-counter treatments are available for minor but troubling foot problems. In most cases, one week of therapy with topical terbinafine is effective for interdigital tinea pedis. Treatment of plantar warts with 17 percent salicylic acid with lactic acid in a collodion base is as effective as cryotherapy, but treatment must be sustained for several months. Toe sleeves and toe spacers can relieve pain from hard or soft corns. Metatarsal pads can relieve the pressure associated with plantar keratoses. Heel cups often can relieve pain caused by age-related th…

Imaging of pediatric foot disorders.

Imaging pediatric foot disorders is a challenging task. Optimally exposed and well-positioned radiographs can answer many questions. Because many bones may not be completely ossified, however, CT or MR imaging is often needed to provide additional information to assist in the management of congenital and acquired lesions in the foot.

Hand-foot-and-mouth disease.

Hand-foot-and-mouth disease is a highly contagious disease most often seen in children during the summer. It is caused most commonly by the virus coxsackie A16, but other enteroviruses have been implicated. It presents with low grade fever, and a vesicular eruption on the hands, feet, and mouth. More serious manifestations are seen less commonly. Diagnosis is most often clinical and treatment is symptomatic in nature. The infection in a male adult is presented.

The Oxford club-foot programme.

We treated 63 club feet in 44 patients by a defined programme of strapping from birth followed by one of two operations performed at six weeks, either a simple calcaneal tendon lengthening or a subtalar realignment, and reviewed them prospectively. The decision as to which operation to perform was taken at four weeks after radiographic measurement of the talocalcaneal angle. All but eight patients (ten feet) were followed for a mean of 8.7 years. The overall results after calcaneal tendon lengthening were satisfactory. The re-operation rate after subtalar realignment was high (39%) due to over

[Treatment of spastic club foot].

In 38 patients with spastic cerebral palsy, treatment was carried out for talipes equinovarus. There were 12 children with spastic hemiplegi … f the patients (9 feet in 7 patients) showed poor results with over-corrections and calcaneo-valgus foot as the main problem. The best results were seen in spastic hemiplegia and the poorest in patients with severe tetraplegia and total body involvement.
