Bilateral Asynchronous Displaced Olecranon Fractures in a Patient With Osteogenesis Imperfecta.

Olecranon fractures are uncommon in children. Children with osteogenesis imperfecta (OI) are at an increased risk of olecranon fractures. This is a report of a 12-year-old male patient with known osteogenesis imperfecta type 1 who sustained bilateral asynchronous olecranon metaphyseal avulsion fractures. He sustained a right olecranon avulsion fracture from a fall and underwent open reduction and internal fixation with two Steinman pins and a tension band wire. He was placed in a cast for a month. The Steinman pins were removed at three months. Six months after the first fracture, he sustained

Acute vascular compromise after avulsion of the distal phalanx with the flexor digitorum profundus tendon.

An unusual case of avulsion fracture of the insertion of the flexor digitorum profundus tendon is presented. In most such avulsion fractures a small amount of fractured bone may remain with the tendon. In this case almost the entire distal phalanx was avulsed, and vascular compromise of the digit then occurred. Prompt reduction restored vascularity.

Iliac Epiphyseal Avulsion Repair Using a Suture Bridge and Lag Screw Construct.

Apophyseal avulsion fractures in adolescents are not uncommon, particularly among young athletes participating in explosive sports. Typically, avulsion fractures around the pelvis occur associated with a forceful eccentric contraction, in which a tendon avulses its osseous origin from the underlying apophysis. An atypical or less commonly encountered avulsion fracture pattern involves the apophysis from the iliac wing extending distally to include the anterior superior iliac spine. This fracture pattern is subject to deforming forces from varying directions, which must be considered during fix

[Fractures of the Triquetrum - a Frequent Cause of Post-Traumatic Ulnar-Sided Wrist Pain].

Fractures of the triquetrum are the second most common carpal fractures after scaphoid fractures and represent a frequent cause of post-traumatic ulnar-sided wrist pain. Dorsal shear fractures account for approximately 93% of all triquetral fractures and are therefore by far the most common fracture type. Two pathomechanisms have been proposed: avulsion by the dorsal ligamentous attachments to the triquetrum and impaction of the dorsal cortex by a prominent ulnar styloid. The dorsal shear/ avulsion fracture is commonly associate with a localized tenderness to palpation over the dorsal aspect o

Artificial intelligence versus radiologist in the accuracy of fracture detection based on computed tomography images: a multi-dimensional, multi-region analysis.

BACKGROUND: Extremities fractures are a leading cause of death and disability, especially in the elderly. Avulsion fracture are also the most commonly missed diagnosis, and delayed diagnosis leads to higher litigation rates. Therefore, this study evaluates the diagnostic efficiency of the artificial intelligence (AI) model before and after optimization based on computed tomography (CT) images and then compares it with that of radiologists, especially for avulsion fractures. METHODS: The digital X-ray photography [digital radiography (DR)] and CT images of adult limb trauma in our hospital fro

Concomitant Ankle Injuries Associated With Tibial Shaft Fractures.

BACKGROUND: Ankle injuries associated with tibial shaft fractures can cause postoperative ankle pain and stiffness even when satisfactory bony union has been achieved. Although several previous studies have described these injuries, they have not been clearly defined or classified in terms of ankle injury type or need for surgical fixation. METHODS: Seventy-one consecutive patients (mean ± SD age, 48.3 ± 16.7 years; 37 men and 34 women) with tibial shaft fractures who underwent computed tomography examination were included. Data were collected including age, sex, body mass index, fracture loc

Missed Sleeve Fracture of the Superior Pole of Patella.

Patellar sleeve fracture is a form of injury in which small osseous fragments avulsed with periosteum and cartilage. 15-year-old male patient, playing in school football team, apllied to our clinic with a history of previously missed patellar superior pole sleeve avulsion fracture. Care must be taken in order not to miss the patellar superior pole sleeve fractures, which are very rare in children. Extra care must be taken in patients, whose X-ray imaging is clean but there is a problem in the extensor mechanism of the knee. INTRODUCTION Since the patella has high mobility and large cartilage s

Incidence and Risk Factors for Posterior Cruciate Ligament Avulsion during Cruciate Retaining Total Knee Arthroplasty.

During cruciate retaining (CR) total knee arthroplasty (TKA), the posterior cruciate ligament (PCL) may avulse at its insertion. The incidence of PCL avulsion fracture has not been previously studied. The aim of this study is to report on the incidence and clinical significance of intraoperative PCL avulsion during primary CR TKA and to identify potential risk factors. Our institutional joint registry was retrospectively reviewed for PCL avulsion occurring during CR TKA implanted between April 2008 and April 2016. Patient demographics, preoperative range of motion (ROM), complications, and rev

Osseous Injury Associated With Ligamentous Tear of the Knee.

One of the most common knee injuries is ligament tear, which may initially manifest as an osseous injury in radiographs. Radiologists should therefore be able to recognize ligament tears of the knee as osseous abnormalities in images. This review focuses on the imaging features of knee ligament injuries and their related osseous injuries: anterior cruciate ligament (ACL) tear with Segond fracture; associated marrow contusion; ACL avulsion fracture; posterior cruciate ligament (PCL) tear with osseous avulsion of the ligament including arcuate sign; reverse Segond fracture; PCL avulsion fracture

Medial and lateral segond fractures in a skeletally immature patient: a radiographic marker for the multiply injured knee.

Marginal fractures of the medial tibial plateau have been reported in the literature as a secondary type of Segond fracture. Some reports described this entity in the setting of combined injuries such as root avulsions of the medial meniscus, complete disruption of the posterior cruciate ligament (PCL), partial tear of the anterior cruciate ligament (ACL), and tears of the medial meniscus and medial collateral ligament. It has been postulated that medial marginal fractures are secondary to compression of the medial aspect of the femoral condyle and tibial plateau with a corresponding posterola

Single-Stage Fixation of Bilateral Open Medial Hoffa Fractures Associated with Bilateral Patella Fracture and Bilateral Distal Radius Fractures: A Case Report.

INTRODUCTION: Posterior cruciate ligament (PCL) avulsion fractures, lateral collateral ligament (LCL)-biceps femoris complex avulsion injuries are relatively uncommon injuries, and associated diaphyseal tibia fracture further is the rarest combination. It is challenging to diagnose such complex injuries, and missed injuries, particularly at the knee can lead to significant instability, increasing patients' morbidity. CASE REPORT: A 25-year-old male patient was diagnosed with PCL avulsion fracture, LCL - biceps femoris complex injury at the fibular side with Type I open mid-shaft both-bone leg

The effect of peroneus brevis tendon anatomy on the stability of fractures at the fifth metatarsal base.

BACKGROUND: Nonunion of classic Jones fractures has typically been attributed to the precarious vascular anatomy of the proximal fifth metatarsal. Despite this theory, the operative treatment of these fractures utilizes biomechanical solutions. The purpose of the present study was to evaluate the influence of the peroneus brevis (PB) tendon on the stability of fractures of the proximal fifth metatarsal. METHODS: We utilized 5 matched pairs (10 specimens) of fresh-frozen cadaveric specimens. We used 2 loading conditions: (1) a simulated fracture distal to the PB insertion (Jones equivalent) an

What is the frequency of anterior syndesmosis injury in posterior malleolus fracture?

BACKGROUND: Posterior malleolus fractures may be fixed to restore syndesmosis stability. However, these fractures are often accompanied by ruptures of other ligaments that stabilize the syndesmosis. This study investigates the frequency of anterior syndesmosis injury in posterior malleolus fractures and its effect on rotational stability. METHODS: Seventy-five consecutive patients who underwent surgery for ankle fractures involving posterior malleolus were prospectively included. Preoperative computed tomography scans in two planes (transverse and sagittal) were used to assess the presence an

Small flake, big problem: an unreported cause of extensor pollicis longus tendon rupture.

Fracture of the base of the third metacarpal with associated avulsion of the extensor carpi radialis brevis tendon is a rare injury. We report such a fracture and the unusual resulting complication of division of the extensor pollicis longus tendon by the avulsed bony fragment. Careful monitoring using lateral radiographs is needed to make the diagnosis and displacement of the avulsed fragment warrants open reduction and internal fixation.

Skier's thumb--the significance of bony injuries.

In a retrospective study to determine the anatomic nature of injuries in thumbs that were treated surgically for either fracture or instability, we reviewed 63 consecutive patients with acute skier's thumb injury. Of the 63 thumbs, 25 (40%) had a fracture. Surgical exploration showed 2 fracture types: a fragment that was attached to the ulnar collateral ligament, and a fragment that was not attached to the ulnar collateral ligament. The 1st type, corresponding to true avulsion fracture of the ulnar collaternal ligament, was found in 8 cases; the same fracture type was seen in another 7 cases,

[Medical assessment in the field of Social Act VII: pre-existing damage and causality of apophyseal injuries and detachments].

The medical assessment of affections of the apophyses frequently presents the assessor with problems in causality testing and estimation of functional impairment. Apophyses are the insertion of tendons and are therefore at the center of an acting parallelogram of forces. They fuse via an apophyseal plate in the second decade of life. Depending on the age there is a reduced mechanical load-bearing capacity of the apophyseal plates in the last 1-2 years before complete mineralization of the joint. Two types of injury must be distinguished in an expert opinion: apophyseal detachment as a result o

Fracture Line Morphology of Greater Tuberosity Fragments of Neer Three- and Four-Part Proximal Humerus Fractures.

OBJECTIVE: In complicated Neer three- and four-part proximal humerus fracture (PHF), greater tuberosity (GT) fragments are often comminuted, and the currently widely used locking plate may not fix GT fragments effectively. A further understanding of morphological characteristics of the GT fragments may help explore new fixation devices. This study aimed to determine the fracture line morphology of the GT fragment of Neer three- or four-part PHF and analyze the location relationship between the locking plate and the GT fragment. METHODS: Seventy-one three-dimensional computed tomography scans

Partial Avulsion Patterns in the Pediatric Humeral Medial Epicondyle.

BACKGROUND: The anterior oblique bundle of the medial ulnar collateral ligament (UCL) inserts on the anteroinferior aspect of the humeral medial epicondyle, while the flexor pronator mass (FPM) originates superficial and proximal to the UCL. With valgus stress, these distinct footprints may produce injury patterns that affect only focal areas of the medial epicondyle. HYPOTHESIS: The proximal UCL can act on the medial epicondyle either in isolation or in conjunction with the FPM to form partial avulsion fracture patterns within the pediatric medial epicondyle, and the predominant pattern invo

Fracture of the carpal scaphoid in children. A clinical and roentgenological study of 108 cases.

A series of 108 carpal scaphoid fractures in children was analysed with respect to the site, type and treatment of the fracture. The patients were followed until radiological fusion occurred. All fractures united and healed completely with conservative treatment. Ninety-four fractures, 41 of which were avulsions, were located in the distal third of the scaphoid. There were 13 waist fractures, and one avulsion fracture was located in the proximal pole of the scaphoid. The great number of avulsion fractures and the location of the fractures in the distal third of the scaphoid is very typical in

Atypical tibial tuberosity fracture in an adolescent.

Avulsion fractures of the tibial tuberosity are typically sustained by adolescent males during sporting activities. Tibial tuberosity avulsions with simultaneous proximal tibial epiphyseal fractures are rare injuries. We present an unusual case of Ogden type IIIA avulsion fracture of tibial tuberosity with a Salter Harris type IV posterior fracture of proximal tibial epiphysis in a 13-year-old boy. We believe that the patient sustained the tibial tuberosity avulsion during the take-off phase of a jump while playing basketball due to sudden violent contraction of the quadriceps as the knee was
