Clinical results of intraarticular fractures of the base of the fifth metacarpal treated by closed reduction and cast immobilization.

This retrospective study evaluated the results of closed reduction and cast immobilization for isolated intraarticular fractures of the base of the fifth metacarpal. Twenty-two of 37 such fractures were available for follow-up at an average of 43 months, and these had all healed at an average of 5 weeks without any cast complications. Twenty patients reported excellent or good results, and two reported fair or poor results. At follow-up, 13 had no arthrosis and nine had mild arthrosis of the carpometacarpal joint. However, outcome was not influenced by fracture type, the degree of subluxation

Clinical and Functional Outcomes of Closed Intramedullary Interlocking Nailing in Extra-articular Distal Tibial Fractures: A Prospective Observational Study.

INTRODUCTION: Extra-articular distal tibial fractures are difficult to manage because of poor soft-tissue coverage, limited vascularity, and … in 2%. CONCLUSION: In this selected cohort of adults with extra-articular distal one-third tibial fractures, closed intramedullary interlocking nailing was associated with progressive short-term functional improvement and generally satisfactory fracture healing. Because the study was small, single-center, non-comparative, and limited to 6 months of follow-up, it …

Minimally Invasive Plate Osteosynthesis for Closed Distal Tibial Fractures: Clinical and Radiographic Outcomes From a Three-Patient Case Series.

Distal tibial fractures present a therapeutic challenge due to limited soft-tissue coverage and the need to achieve stable fixation while preserving local vascularity. Minimally invasive plate osteosynthesis (MIPO) has emerged as a biologically favorable technique for managing selected extra-articular distal tibial fractures by minimizing soft-tissue disruption and maintaining fracture hematoma. This case series describes three patients with right-sided closed extra-articular distal tibial fractures classified as AO Foundation/Orthopaedic Trauma Association (AO/OTA) 43-A1.2, managed with MIPO

Outcomes of Perfused but Pulseless Versus Well-Perfused Pediatric Supracondylar Humerus Fractures Treated With Closed Reduction and Percutaneous Pinning.

BACKGROUND: The management of pediatric supracondylar humerus fractures (SHF) with a pink pulseless hand (PPH) remains controversial. Traditional teaching advocated urgent vascular exploration, but many centres now favour closed reduction and percutaneous pinning (CRPP), followed by observation if the hand stays warm and well-perfused. OBJECTIVE: This study aimed to compare clinical and radiological outcomes of displaced pediatric SHF in two groups: those presenting with a PPH versus those with a well-perfused hand and palpable pulse. All patients were managed with CRPP. METHODS: We retrospe

Bilateral Distal Tibial Fractures with Preexisting Long-Stem Knee Prostheses Treated with Closed Retrograde Intramedullary Nails: A Case Report.

Recently there has been expanded interest in the management of fractures of the ankle in the elderly. This case presentation demonstrates an alternative treatment choice for bilateral distal tibial fractures in a comorbid patient. This is a novel case of interest because low-energy bilateral distal tibial fracture in the elderly and comorbid population is a relatively infrequent occurrence. Several case series have demonstrated the efficacy and safety of closed retrograde nailing of distal tibial fractures. The patient was an 80-year-old woman with a medical history that included hypertension,

There is no difference in prevalence of rotation malalignment following intramedullary nailing of open versus closed tibial shaft fractures.

INTRODUCTION: Tibial shaft fractures are the most prevalent long-bone fracture and often associated with high energy mechanisms. As a result, 12-15% are seen to been open fractures making up 45% of open fractures. Open fractures are commonly associated with higher complication rates such as infection, and non-union. Intramedullary nails (IMN) are an accepted treatment modality open tibial shaft fractures; however, IMNs are known to be associated with potential rotational malalignment. We hypothesis that an open tibial shaft fracture may in fact be protective against post operative rotational m

Secondary Procedures after Closed Reduction Percutaneous Pinning versus Open Reduction Internal Fixation of Phalanx Fractures: A Large Database Analysis.

Background: While functional and patient-reported outcomes are similar for closed reduction percutaneous pinning (CRPP) and open reduction internal fixation (ORIF) of phalanx fractures, controversy exists regarding optimal fixation choice. In this study, we tested the null hypotheses that there is no difference in (1) percentage of patients undergoing secondary procedures within 2 years, (2) therapy utilisation and (3) total costs following CRPP and ORIF of phalanx fractures. Methods: Using an administrative claims database, we identified patients undergoing CRPP or ORIF of phalanx fractures f

Early Postoperative Infections After Closed Reduction and Percutaneous Pinning in Type II and Type III Pediatric Supracondylar Humerus Fractures.

BACKGROUND: Supracondylar humerus fractures (SCHF) are the most common elbow fracture type in children, and one of the most common pediatric fracture types overall. Excellent outcomes are generally reported with closed reduction and pinning (CRPP), but the technique involves leaving the pins outside the skin. External pins can act as a nidus for infection. We characterize the infection complications from SCHF treatment at a single-centre tertiary children's hospital over 10 years. This is the largest series on infectious outcomes after CRPP of SCHF reported to date. METHODS: Pediatric patient

Volar Locking Plate Fixation Versus Closed Reduction for Distal Radial Fractures in Adults: A Systematic Review and Meta-Analysis.

BACKGROUND: Distal radial fractures in adults are common, representing a substantial burden to patients and health systems. The 2 main treatments are closed reduction and cast immobilization (CR) and volar locking plate (VLP) fixation. Our primary aim was to determine if VLP fixation leads to better patient-reported pain and function at 12 months compared with CR. METHODS: We searched systematically for randomized controlled trials (RCTs) comparing outcomes of VLP fixation with CR for the treatment of distal radial fractures in adults. The Cochrane Collaboration risk-of-bias tool was used to
