[Late radiological aspects after vagotomy and gastric drainage].

Personal experience in 87 cases of vagotomy and gastric drainage is presented. Pyloroplasty according to Heinecke-Mikulicz was performed in 50 cases and drainage according to Judd in 9. Three groups were distinguished on the basis of the presence of a "gastroduodenal canal" and "pseudodiverticula". Twenty-two patients were subjected to a Finney pyloroplasty, radiologically visible is a transparent line from the large curvature of the antrum to the lesser curvature medially. Jaboulay-Alexiu gastroduodenostomy was performed in 6 cases. Here anastomosis between the large curvature and the second

Prospective cohort study on short-term outcomes of 3D-laparoscopic pancreaticoduodenectomy with stented pancreaticogastrostomy.

BACKGROUND: Minimally invasive pancreaticoduodenectomy, either laparoscopic or robotic, is a high-risk procedure with demanding learning curve. The aim of this prospective cohort study was to evaluate short-term clinical and oncologic outcomes of 3D-laparoscopic pancreaticoduodenectomy (3dLPD) with stented pancreaticogastrostomy (sPG) and Roux-en-Y gastroenterostomy (ryGES). METHODS: Between March 2016 and July 2021, 347 consecutive patients underwent 3dLPD for confirmed or suspected pancreatic or periampullary tumors. Pancreatic duct diameter measured 3 mm or less in 221 (64%) and pancreatic…

Endoscopic biliary drainage in patients with surgically altered anatomy: the Street multicenter study.

BACKGROUND AND AIMS: Endoscopic biliary drainage (BD) in patients with surgically altered anatomy (SAA) is a challenging scenario. Different BD approaches are available but still not standardized, and they rely on local availability and expertise. The aim of this study was to explore the approach to BD in patients with SAA in different centers. METHODS: This was a retrospective multicenter cohort study. All adult patients with SAA who underwent endoscopic BD for any indication in the last 5 years were enrolled. Clinical outcomes were evaluated according to the type of surgical reconstruction,

Short- and Mid-Term Surgical Outcomes of Billroth I Versus Billroth II/Roux-en-Y Reconstruction: A Prospective Observational Cohort Study.

Background and Objectives: The best method for reconstructing the stomach after distal gastrectomy surgery in gastric cancer patients continues to be a subject of ongoing discussion. The most beneficial surgical option for patients is Billroth I (BI), yet surgeons may perform Billroth II and Roux-en-Y (BII/RY) procedures because they are easier to execute, although their impact on recovery complications and postoperative function remains unclear. This prospective observational cohort study compares the short- and mid-term surgical outcomes between BI and BII/RY reconstructions. Materials and M

Short- and long-term outcomes of Roux-en-Y and Billroth II with Braun reconstruction in total laparoscopic distal gastrectomy: a retrospective analysis.

BACKGROUND: The controversy surrounding Roux-en-Y (R-Y) and Billroth II with Braun (BII + B) reconstruction as an anti-bile reflux procedure after distal gastrectomy has persisted. Recent studies have demonstrated their efficacy, but the long-term outcomes and postoperative quality of life (QoL) among patients have yet to be evaluated. Therefore, we compared the short-term and long-term outcomes of the two procedures as well as QoL. METHODS: The clinical data of 151 patients who underwent total laparoscopic distal gastrectomy (TLDG) at the Gastrointestinal Surgery Department of the Second Hos

Functional Outcomes of Billroth I Gastroduodenostomy Using Linear Staplers in Totally Laparoscopic Distal Gastrectomy.

BACKGROUND/AIM: This study examined whether functional outcomes of linear-stapled Billroth I (LS-BI) in totally laparoscopic distal gastrectomy (TLDG) are comparable to those of circular-stapled Billroth I (CS-BI) in laparoscopy-assisted distal gastrectomy (LADG). PATIENTS AND METHODS: This was a retrospective study of patients with gastric cancer undergoing TLDG with LS-BI (n=50) or LADG with CS-BI (n=50). Postoperative endoscopic findings of the remnant stomach and nutritional status were evaluated. RESULTS: The occurrence of grade 2 or more severe remnant gastritis in the LS-BI group (46.

Stapling an extracorporeal Billroth-I anastomosis by the complete double stapling technique after laparoscopy-assisted distal gastrectomy.

INTRODUCTION: Laparoscopy-assisted distal gastrectomy is one of the major treatments for early stage gastric cancer, particularly in the East Asia. In this method, extracorporeal anastomosis is performed via a small laparotomy wound, but excessive tissue traction may be encountered during the anastomotic procedure. Therefore, we developed an original procedure for extracorporeal Billroth-I reconstruction: end-to-end stapling gastroduodenostomy with complete double stapling technique. This procedure aims to reduce the problems related to maneuvers through a small laparotomy. METHODS: An end-to

Surgical Strategy and Outcome in Patients Undergoing Pancreaticoduodenectomy After Gastric Resection: A Three-Center Experience with 39 Patients.

BACKGROUND: Stomach cancer is the second most commonly diagnosed cancer in Korea. Although the long-term survival outcome has improved, secondary primary tumors from periampullary regions are increasing inevitably and pancreaticoduodenectomy (PD) following gastrectomy is challenging. This study evaluates the surgical outcomes of PD following gastrectomy and suggests the optimum method for reconstruction. METHODS: Patients who underwent curative PD with a history of gastric resection between 2005 and 2015 were assessed retrospectively. PD was performed according to the standard fashion, with t

[Use of plasma scalpel and one-layer suture in resection of the stomach].

110 cases of resection of the stomach with transection of the stomach and the duodenum by plasmic scalpel and suturing of curvatura minor of the stump and anastomosis with the use of original one-layer sero-muscular-submucosal suture have been analysed. Changes in gastric wound at the site of plasma transection were evaluated microscopically. It was established that plasma transection of the stomach is aseptic, hemostatic, thermal trauma is minimal and does not exceed the level of 3-4 mm from transection line. Favourable conditions for applications of one-layer suture contribute much to a decr

Highly selective vagotomy plus Jaboulay: a possible alternative in patients with benign stenosis secondary to duodenal ulceration.

BACKGROUND: The treatment of patients with duodenal ulcers has undergone radical changes in recent years. Symptomatic stenotic obstruction of the gastric outlet, however, has remained a specific indication for elective operation, with gastric resection (Billroth I or II) and vagotomy often used as options for intervention. STUDY DESIGN: The present report describes the results of highly selective vagotomy (HSV) in combination with lateral Jaboulay gastroduodenostomy in the treatment of patients with benign stenosis secondary to duodenal ulceration. Functionality of results and patient satisfa

Plasma gastrin in congenitial hypertrophic pyloric stenosis. A hypothesis disproved.

Fasting plasma gastrin levels were measured in babies with pyloric stenosis and in normal babies of similar age. There was no difference in gastrin levels either before or after operation between the babies with pyloric stenosis and normal babies. Similarly, neither the fasting blood glucose nor the fasting gastric pH of the babies with pyloric stenosis differed significantly from the values obtained in normal babies. Our findings do not support the hypothesis that gastrin in the fasting baby has an aetiological role in the development of hypertrophic pyloric stenosis of infancy. An alternativ

Effects of anastomotic techniques in laparoscopic distal gastrectomy with Billroth I method on remnant stomach motor functions and postoperative quality of life.

Laparoscopic distal gastrectomy with Billroth I reconstruction (LDGBI) is a standard procedure for early-stage gastric cancer. Although both Delta-shaped (Delta) and circular stapler (CS) techniques are commonly used for anastomosis, their long-term effects on remnant gastric motility and postoperative quality of life (QOL) remain insufficiently defined. This retrospective study analyzed 43 patients who underwent LDGBI at least one year prior, divided into CS (n=22) and Delta (n=21) groups. Gastric emptying was evaluated using the 13C-acetate breath test, focusing on residual reservoir capacit

Billroth-I Reconstruction with Overlap Anastomosis Using an EndoWrist Linear Stapler After Robotic Distal Gastrectomy.

Introduction: Robotic distal gastrectomy (RDG) is now thought to be less invasive than conventional laparoscopic distal gastrectomy (LDG) for gastric cancer. Although the delta-shaped anastomosis is an established, widely performed procedure for intracorporeal Billroth-I (B-I) gastroduodenostomy after LDG, it has some difficulties and is performed in the ischemic region of the duodenum. We therefore developed a novel overlap B-I gastroduodenostomy after RDG. Materials and Methods: We started using the da Vinci Surgical System (Intuitive Surgical, Sunnyvale, CA) for RDG in May 2017. The robotic
