[Irritable bowel syndrome--irritable bowel or irritable mind?].

Irritable bowel syndrome (IBS) is one of the most common disorders of gastrointestinal tract, affecting significant number of western population. The characteristic features are chronic and(or) recurrent abdominal pain and discomfort associated with changes in the bowel habit with or without bloating. According to Rome Criteria III there is an improved focus on the intestinal transit time which allows for better diagnosis and characterization of patients either with diarrhea, constipation predominant or mixed type IBS. Patients with IBS also frequently manifest various symptoms related to cent

[Recurrent hemorrhage from a duodenal ulcer in a 5-year-old healthy boy. A case report].

Peptic ulcers in infants are rare. We report a 5-year-old boy who was admitted with recurrent bleeding from a huge duodenal ulcer. There were no concomitant disease and no preceding symptoms discovered. The only clinical symptom was bloody stool of light red color what led to diagnostic problems. Upper gastrointestinal bleeding was not considered initially. Short transit time through the gut may suggest a bleeding source within the lower intestine. Gastroscopy was performed delayed. Injection therapy of the ulcer once using fibrin sealant was followed by definite cessation of bleeding. Helicob

Gastroparesis in systemic sclerosis: a detailed analysis using whole-gut scintigraphy.

OBJECTIVES: Gastroparesis is a common complication of SSc. We sought to determine the degree of overlap between gastroparesis and dysmotility in other areas of the gut, correlate our findings with gastrointestinal (GI) symptoms, and examine associations between gastroparesis and SSc features. METHODS: Whole-gut scintigraphy was performed on SSc patients who were enrolled in the Johns Hopkins Scleroderma Cohort, for whom detailed longitudinal clinical and serologic data are collected. A subset of patients completed the University of California Los Angeles Scleroderma Clinical Trial Consortium

Regional gastrointestinal pH profile is altered in patients with type 1 diabetes and peripheral neuropathy.

BACKGROUND: Gastrointestinal (GI) symptoms, such as nausea and bloating, are common in people with type 1 diabetes (T1DM). Autonomic dysfunction can lead to changes in the GI secreto-motor function which can be associated with GI symptom development. We hypothesized that regional pH profiles in T1DM differs from health and would be associated with objective physiological/clinical markers. METHODS: Forty-seven T1DM with confirmed diabetic sensory peripheral neuropathy and 41 healthy age-matched subjects underwent standardized wireless motility capsule testing. T1DM completed the gastroparesis

L-β-Galactoglucan alleviates loperamide-induced constipation in mice and regulates gut microbiota and metabolism.

Constipation is a common gastrointestinal disorder that affects millions of individuals worldwide. This study investigated the modulatory effects of L-β-galactoglucan, a novel exopolysaccharide derived from Agrobacterium sp. FN01, in a loperamide-induced murine constipation model. Mice were orally administered loperamide (10 mg/kg body weight) to induce constipation and subsequently received L-β-galactoglucan at doses of 0, 200, 400, or 800 mg/kg body weight for 14 days. The modulatory effects of L-β-galactoglucan were elucidated through an integrated approach combining histological, molecular

Biomarkers to distinguish functional constipation from irritable bowel syndrome with constipation.

Treatments for functional constipation (FC) and irritable bowel syndrome with constipation (IBS-C) differ, but symptom criteria do not reliably distinguish between them; some regard FC and IBS-C as parts of a single constipation spectrum. Our goal was to review studies comparing FC and IBS-C to identify possible biomarkers that separate them. A systematic review identified 15 studies that compared physiologic tests in FC vs IBS-C. Pain thresholds were lower in IBS-C than FC for 3/5 studies and not different in 2/5. Colonic motility was decreased more in FC than IBS-C for 3/3 studies, and whole

Potential benefits and hazards of physical activity and exercise on the gastrointestinal tract.

This review describes the current state of knowledge on the hazards of exercise and the potential benefits of physical activity on the gastrointestinal tract. In particular, acute strenuous exercise may provoke gastrointestinal symptoms such as heartburn or diarrhoea. A substantial part (20-50%) of endurance athletes are hampered by these symptoms which may deter them from participation in training and competitive events. Nevertheless, these acute symptoms are transient and do not hamper the athlete's health in the long term. The only exception is repeated gastrointestinal bleeding during trai

Inhibition of calcium-sensing receptors improves intestinal motility in obese mice through enteric neurons and vagal center.

The calcium-sensing receptor (CaSR) is expressed in the gastrointestinal tract and plays a key role in regulating gastrointestinal functions, including motility. However, the effect of CaSR on the gastrointestinal motility of obese mice remains unclear. Our findings revealed that CaSR expression was elevated in the enteric nervous system (ENS) of high-fat diet (HFD)-induced obese mice, and its expression was reduced by the CaSR antagonist NPS-2143. While NPS-2143 had no significant effect on gastric emptying, it notably shortened intestinal transit time in obese mice. Mechanistically, NPS-2143

Dietary Fibers in Healthy Children and in Pediatric Gastrointestinal Disorders: A Practical Guide.

Dietary fibers include non-digestible plant carbohydrates, lignin and resistant starch. Dietary fibers provide immune, cardiovascular, metabolic and intestinal beneficial effects in humans. Fibers naturally present in foods (fruits, vegetables, legumes, cereals) or used as supplements have different physical, chemical and functional profiles. This narrative review provides an update to the knowledge on the effects of dietary fibers in healthy subjects and in children with gastrointestinal disorders. Soluble fibers are digested by gut bacteria, producing short-chain fatty acids and energy for c

Gastrocolic Reflex Is Delayed and Diminished in Adults with Type 1 Diabetes.

BACKGROUND: Constipation is a prevalent gastrointestinal complication in diabetes. The pathophysiology may include neural dysfunction and impaired gastrocolic reflex; however, investigation of the latter has been limited in diabetes. Pathophysiology as a basis for understanding symptom complexes and therapeutic targets.

Sensorimotor disorders of the stomach, small intestine and colon have a limited repertoire of clinical manifestations, and there is the potential for more than one mechanism to lead to symptoms. In many recent clinical trial programs of novel agents in neurogastroenterology, the emphasis has been primarily on symptom assessment of broad groups of patients identified by the Rome criteria. Drugs of potential value have fallen by the wayside with this approach. We propose the current paradigm is partly to blame; physiological testing should provide the basis for identifying more homogeneous popul

Orally deliverable Perilla frutescens-derived nanovesicles as natural bioactive nanocarriers for colon-targeted colitis therapy via microenvironment reprogramming.

Effective oral therapy for inflammatory bowel disease (IBD) requires overcoming gastrointestinal barriers to modulate the dysregulated mucosal niche. Here, we present edible nanovesicles derived from Perilla frutescens (PLENs) as an intrinsically stable, bioactive nanotherapeutic. Multi-omics profiling defined a robust lipid-bilayer architecture encapsulating a synergistic cargo of proteins, miRNAs, and antioxidant metabolites. This structural integrity enabled PLENs to survive gastrointestinal transit and exhibit preferential fluorescence localization with prolonged retention in the inflamed …

Pathophysiology of irritable bowel syndrome.

Despite the continued absence of a definitive biomarker for irritable bowel syndrome (IBS), research over the last three decades has identified a wide range of underlying pathophysiological abnormalities. Peripheral mechanisms include gastrointestinal infection, changes in the gut microbiome, visceral hypersensitivity, increased intestinal permeability, low-grade mucosal inflammation and altered immune function, abnormal gastrointestinal motility, and the role of serotonin, bile acid metabolism, and carbohydrate metabolism. Central mechanisms include psychological health and altered central pa

Dysphagia and gastrointestinal disorders in Parkinson's disease: towards a diagnostic standard operating procedure for the interprofessional neurological outpatient care team.

Gastrointestinal Disorders in people with Parkinson's disease (PwP) are able to cause widespread clinical symptoms which can be challenging to classify in day-to-day clinical practice. In the upper gastrointestinal tract (GIT) oropharyngeal dysphagia, esophageal motility disorders as well as delayed gastric emptying may cause nutrition problems and reduce the efficacy of oral pharmacotherapy. In the lower GIT, bacterial overgrowth and prolonged colorectal transit times induce different stages of constipation leading to reduced quality of life and GIT comorbidities as well. Most of these sympto

A Novel Approach for Reducing Enteropathogens Using LAB-Fermented Whey Solution and UV-C Light-Activated Gallic Acid.

This study evaluates the combined antibacterial activity (AA) of lactic acid bacteria (LAB) and UV-C light-activated gallic acid (AGA) against two common enteropathogens (Escherichia coli and S. Typhimurium) in terms of microbial growth and survival during gastrointestinal simulation (GS). The LAB were selected based on their capacity to ferment soy and whey concentrates in monoculture and co-culture. The resulting LAB-fermented medium (LABF) was evaluated for its antibacterial properties against enteropathogens and combined with AGA to determine its effects during the GS. Results indicated th
