[Amyloidosis presenting as melaena].

Amyloidosis is a disease characterized by abnormal extracellular deposits of protein. The disease may affect the stomach, however, symptoms are rare in this case. Furthermore, the rare symptoms are diffuse and unspecific and the diagnosis relies on biopsy. We report the case of a 79-year-old female presenting with melaena and no history of amyloidosis. Gastroscopy raised suspicion of a malignant process in the stomach, but biopsy revealed gastric amyloidosis. The treatment of gastric involvement is primarily symptomatic, and causal treatment is reserved for the few.

Hematemesis from esophageal varices associated with esophageal perforation: sclerotherapy and endoscopic clipping.

A 46-year-old man was referred to our Unit for hematemesis. The medical history of the patient revealed an HCV-related cirrhosis, a human immunodeficiency virus (HIV) infection and recent and persistent episodes of emesis. An urgent gastroscopy disclosed evidence of active bleeding from varices of the lower third of the esophagus and a concomitant laceration of the esophageal wall due to the emesis. These two conditions have been endoscopically diagnosed and successfully treated by sclerotherapy and endoscopic clipping.

[Our experience with the surgical treatment of complicated duodenal ulcer using truncal vagotomy].

Personal ten-year-long surgical and clinical experience with the application of truncal vagotomy in 51 patients with complicated duodenal ulcers-23 with hemorrhage, 19 with perforation and nine with stenosis, is shared. Emphasis is laid on the superiorities and comparatively low lethality (5.8 per cent) of the application of this type of vagotomy during the surgical management of complicated duodenal ulcers, as well as on the great diagnostic relevance of fiber gastroscopy applied on the apex of hemorrhage in case of bleeding duodenal ulcers.

Gastric antral vascular ectasia. A case report.

A case of vascular lesion of angiodysplastic type in the pyloric part of the stomach has been reported in a patient with chronic viral hepatitis. The diagnosis was made from clinical symptoms, endoscopic and histopathologic findings in the specimens obtained during gastroscopy. Many authors emphasize a correlation between the incidence of "gastric antral vascular ectasia" and liver diseases, especially cirrhosis with portal hypertension. A review of bibliography with particular consideration of its etiology has been presented.

Endolumenal endoscopic full-thickness resection of muscularis propria-originating gastric submucosal tumors.

BACKGROUND: This study retrospectively reviewed 48 cases of gastric submucosal tumors (SMTs) treated by endolumenal endoscopic full-thicknes … as 1.59 cm (range, 0.50-4.80 cm; standard deviation, 1.01 cm). During the EFR process, dual-channel gastroscopy was applied in 20 cases of SMTs, and paracentesis during the EFR process was applied in 9 cases. EFR for larger SMTs and gastric corpus-originating SMTs had longer operative times. Pathological diagnosis included 43 gastrointestinal stromal tumors, …

Successful Endoscopic Management of Gastric Volvulus: A Case Report Highlighting Effective Treatment Strategies.

Gastric volvulus is a rare entity which lacks evidence in the literature, characterized by the abnormal twisting or rotation of the stomach on itself of at least 180 degrees along its transverse or longitudinal axis. This rotation can lead to the obstruction of the stomach and interfere with its blood supply, causing a variety of symptoms and potentially serious complications. Some patients may be managed temporarily or definitively with conservative or endoscopic treatment, the latest including reduction and percutaneous gastropexy, but most patients benefit from surgery. The authors report t

Giant gastric bezoar removal from the stomach using combined dual knife-electric snare treatment: a case report.

Gastric bezoars are foreign bodies that occur when certain drugs or foods aggregate in the stomach. They cannot be digested and most are unable to pass through the pylorus. Here, we describe a giant hard gastric bezoar that was removed by using combined dual knife-electric snare treatment. This strategy has rarely been reported and can eliminate the need for surgery. The patient exhibited a giant hard gastric bezoar and reported a habit of eating persimmons. A giant persimmon bezoar was treated using a combined dual knife-electric snare approach, without any complications. Follow-up endoscopy

Endoscopic guided intubation of long nasointestinal decompression tube.

Surgeons have long been faced with the problem of intubating long nasointestinal tubes through the pylorus of the stomach when intestinal obstructions are present. The procedure described herein provides a direct view of intubation, endoscopically, rather than a blind approach fluoroscopically. This is achieved in every instance through the use of a guided wire inserted through an endoscope with a sleeve type, long nasogastrointestinal decompression tube. The procedure has proved to be advantageous because of the time saved by providing for rapid passage and prompt decompression which also eli

An evaluation of the gastroduodenal fibrescope.

One hundred and sixty patients have been examined with the flexible gastroduodenoscope. Passage of this instrument is easier than of the conventional gastroscope, and the stomach can be examined down to the pyloric canal and pylorus. Duodenoscopy is sometimes possible but some modification is required in order to guarantee success. Examination of a gastrojejunal stoma is practically always possible and is superior to that obtained with the gastroscope. The fibrescope can be used in patients with haematemesis within a few hours of their admission to hospital, and with it definitive early diagno

Dysphagia secondary to neuroendocrine carcinoma: a rare esophageal tumor.

We present two clinical cases, a 68-year-old male and a 55-year-old female, who are being studied for progressive solid food dysphagia. The initial gastroscopy revealed a polypoid lesion in the first case and a partially stenosing lesion in the second case. Both lesions were located in the mid-esophagus and presented a normal mucosa. Histological diagnosis confirmed an esophageal neuroendocrine carcinoma (NEC) of large cell and small cell types, respectively.

Endoscopic management of portal hypertension.

Cirrhosis is the leading cause of portal hypertension worldwide, with the development of bleeding gastroesophageal varices being one of the most life-threatening consequences. Endoscopy plays an indispensible role in the diagnosis, staging, and prophylactic or active management of varices. With the expected future refinements in endoscopic technology, capsule endoscopy may one day replace traditional gastroscopy as a diagnostic modality, whereas endoscopic ultrasound may more precisely guide interventional therapy for gastric varices.

Gastric perforation of a left lobe amoebic liver abscess.

Liver abscess is the most common extra-intestinal manifestation of invasive amoebiasis. Perforation of the abscess is a potential life-threatening complication. We report a case where perforation into the stomach was successfully managed conservatively. The initial diagnosis in this case was made by gastroscopy and biopsy. To our knowledge, only five cases of gastric perforation of an amoebic liver abscess have been reported in the English literature. In none of these cases was the diagnosis established by histology of gastric biopsy specimens.

Tubular esophageal duplication associated with esophageal stenosis, pericardial aplasia, diaphragmatic hernia, ramification anomaly of lower lobe bronchus and partial pancreas anulare.

Alimentary tract duplications are rare anomalies, which are often associated with other congenital malformations. We report on a newborn with the combination of tubular esophageal duplication, congenital esophageal stenosis, ramification anomaly of lower left lobe bronchus, complete pericardial aplasia and malrotation of the bowel with a partial pancreas anulare. The diagnoses are based on esophagogram, bronchogram, bronchoscopy, gastroscopy, explorative thoracotomy and laparotomy. The treatment was a resection of the duplication per thoracotomy, esophageal myotomy with fundoplicatio.

[Diagnostic approach in tumor patients with non-specific upper abdominal complaints].

The authors demonstrate on the basis of several observations of perigastric space-occupying growths in malignoma patients that x-ray examination of the stomach via mucosa and double-contrast imaging yields results of greatest diagnostic value in non-specific upper abdominal complaints. Supplementary diagnostic tools--but by no means competing methods--are often supplied by computed tomography and sonography. In case of extensive infiltration of the gastric wall, gastroscopy is indicated as an additional means to obtain confirmation by biopsy.

Gastric Fundus Splenosis Mimicking Malignancy on FDG PET/CT.

A 17-year-old man presented with dull pain in the left upper abdomen for 1 month. Initial CT and gastroscopy revealed a mass in the gastric fundas, protruding into the lumen. Based on findings of a fine-needle biopsy, an inflammatory myofibroblastic tumor was suspected. Subsequent PET/CT showed increased FDG uptake in the gastric fundas as well as hepatogastric ligament, para-aortic region. Eventually, he underwent surgical resection, and histopathologic findings confirmed the diagnosis of splenosis.

Hemosuccus pancreaticus secondary to pseudoaneurysm of the splenic artery.

A 41-years-old male who presents abdominal pain and rectal bleeding with acute anemization. Both the gastroscopy and the colonoscopy dont identify any potential cause of bleeding. The computed tomography (CT) shows a pseudoaneurysm of the splenic artery. Gastrointestinal bleeding was attributed to a hemosuccus pancreaticus secondary to the pseudoaneurysm of the splenic artery. An arteriography was performed to embolize it. Pseudoaneurysm of the splenic artery is a rare cause of gastrointestinal bleeding. Given the risk of a rupture, the treatment is mandatory irrespective of the size or sympto…
