Dollars and dilemmas: lenacapavir's pricing, patents, and the path to affordability.

Lenacapavir (Yeztugo), the world's first twice-yearly human immunodeficiency virus (HIV) prevention injection, offers transformative potential but faces a critical challenge: affordability. While its production cost is estimated at just $25 per person annually, projections place its market price at over $25,000 in high-income settings, a 1000-fold markup that could restrict access in low- and middle-income countries (LMICs). Gilead's licensing agreements with six generic manufacturers, covering 120 low-income countries, mark a step forward. However, upper middle-income countries with significa

Community-level barriers and facilitators of indoor residual spraying for malaria control: a systematic review of qualitative evidence from sub-Saharan Africa.

INTRODUCTION: Indoor residual spraying is a key strategy in the control against malaria, which still remains a devastating public health challenge in Sub-Saharan Africa. It was recommended by the World Health Organization, yet its effectiveness is still being compromised by low community uptake. Unlike other malaria interventions, indoor residual spraying is intrusive and requires entry into private household spaces and substantial domestic preparation. Household refusal and locked structures still remain primary threats to the success of the campaign. Although qualitative studies have examine

Advancing Women's Performance in Fitness and Sports: An Exploratory Field Study on Hormonal Monitoring and Menstrual Cycle-Tailored Training Strategies.

BACKGROUND: Extensive research confirms that hormonal fluctuations during the menstrual cycle significantly influence female athletic performance, with profound implications for public health, including promoting equitable access to sports and enhancing women's overall physical and mental well-being. Numerous scientifically validated methods are available to monitor hormonal status and menstrual cycle phases. However, our prior investigations revealed that these insights are rarely applied in practice due to the complexity and invasiveness of existing methods. This study examines the effects o

Historical redlining and breast cancer screening in a regional spatial analysis.

BACKGROUND: Redlining, introduced in 1933 by the Homeowners' Loan Corporation, created enduring structural inequities in housing, health care access, and neighborhood investment. These historical patterns may contribute to persistent disparities in breast cancer screening. This study examines how historical redlining, alongside contemporary socioeconomic factors, shapes screening disparities across the University of Kansas Cancer Center (KUCC) catchment area. METHODS: We conducted a census tract-level spatial analysis across 123 counties (Kansas and western Missouri) by linking CDC PLACES bre

Intersectional Prevalence of Suicide Ideation, Plan, and Attempt Based on Gender, Sexual Orientation, Race and Ethnicity, and Rurality.

IMPORTANCE: Suicidal thoughts and behaviors (STBs) are major public health problems, and some social groups experience disproportionate STB burden. Studies assessing STB inequities for single identities (eg, gender or sexual orientation) cannot evaluate intersectional differences and do not reflect that the causes of inequities are due to structural-level (vs individual-level) processes. OBJECTIVE: To examine differences in STB prevalence at the intersection of gender, sexual orientation, race and ethnicity, and rurality. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study used adu

Non-communicable disease prevalence among asylum seekers and Swiss nationals: a comparative cross-sectional study in emergency care.

BACKGROUND: Non-communicable diseases (NCDs) are the leading causes of death worldwide. Ongoing global migration increases the proportion of recently arrived migrants in many countries. Yet data on NCD prevalence and care among asylum seekers (AS), a marginalised population with barriers to healthcare access, remain scarce. Emergency departments (EDs) often serve as healthcare access points for vulnerable populations, providing valuable data to identify unmet needs and health inequities. The primary objective of this study was to assess NCD prevalence, along with care, patterns and ED consulta

Borderland Hearing Health: Insights From Community-Based Screenings.

IMPORTANCE: Underserved populations, especially Hispanics along the U.S.-Mexico border, are systematically excluded from early hearing loss detection efforts. Without targeted, culturally informed approaches, existing health disparities will only be exacerbated. This study addresses a critical gap in equitable access to hearing health care for Hispanic individuals in the United States. OBJECTIVE: The aim of this study was to examine the outcomes of community-based hearing screenings embedded within a mobile health outreach program in El Paso, Texas, and identify demographic predictors of scre

Global, regional, and national trends in 34 cancers through a novel burden-quality-of-care framework: a cross-sectional study.

BACKGROUND: Current unidimensional cancer evaluation systems limit precise cancer control. This study establishes a novel burden-quality framework that integrates disease burden and quality-of-care index (QCI) to better assess cancer management effectiveness globally. METHODS: The QCI was derived by principal component analysis of six standardized indicators using the Global Burden of Disease data (1990-2021). A four-quadrant framework integrated QCI with disease burden to classify 34 cancers across 204 countries/regions. The sociodemographic index (SDI) and gender disparity ratios (GDR) were

The Price of Absence: Productivity Costs and Sickness-Absence Episodes Attributable to Mental and Behavioral Disorders in the Brazilian Federal Workforce - A Nationwide Time-Series Study (2012-2024).

BACKGROUND: Mental and behavioral disorders (MDs) represent a leading cause of sickness absence among Brazilian public servants, generating substantial social and economic burdens. Despite their impact, comprehensive nationwide assessments of productivity costs attributable to MDs over extended periods remain scarce. OBJECTIVES: The aim of this study was to estimate the productivity costs associated with MDs (ICD-10 Chapter V) among Brazilian federal civil servants from 2012 to 2024 compared with the four other most prevalent non-mental causes of absence. METHODS: We conducted a retrospectiv

Introducing the intruder paradox: "It's not the imposter syndrome, it's you don't want me in the field".

INTRODUCTION: Imposter phenomenon (IP) or syndrome is typically framed as a personal problem rooted in low self-esteem or anxiety that an individual must overcome to succeed. Critics argue that this framing overlooks external forces, such as discrimination, that engender feelings of inadequacy. Since women in medicine disproportionately experience both IP and workplace harms, our purpose was to examine the link between gender-based discrimination and IP from their point of view. METHODS: Women training and practicing across Canada were invited to participate via Twitter/X. Forty consented (n
