Investigating Autism-Related Symptoms in Children with Prader-Willi Syndrome: A Case Study.

Prader-Willi syndrome (PWS), a rare genetic disorder caused by the lack of expression of paternal genes from chromosome 15q11-13, has been investigated for autism spectrum disorder (ASD) symptomatology in various studies. However, previous findings have been variable, and no studies investigating ASD symptomatology in PWS have exclusively studied children. We aimed to characterize social communication functioning and other ASD-related symptoms in children with PWS, and assessed agreement across measures and rates of ASD diagnosis. Measures included the Autism Diagnostic Observation Schedule-2

Fatty Acid Amide Hydrolase Inhibitor Treatment in Men With Chronic Prostatitis/Chronic Pelvic Pain Syndrome: An Adaptive Double-blind, Randomized Controlled Trial.

OBJECTIVE: To examine the effect of a peripherally active fatty acid amide hydrolase (FAAH) inhibitor ASP3652 on safety and efficacy outcomes in chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS). Inhibition of FAAH is hypothesized to reduce the excitability of urinary tract afferents including nociceptors. MATERIALS AND METHODS: In this adaptive, randomized, double-blind, placebo-controlled study, adult male patients with moderate to severe CP/CPPS were treated for 12 weeks with an oral dose of ASP3652 (25, 75, 150, or 300 mg twice daily, or 300 mg once daily), or placebo. A Bayesian

Idiopathic scoliosis and the central nervous system: a motor control problem. The Harrington lecture, 1983. Scoliosis Research Society.

An etiologic concept linking an impaired axial motor control system to the structural deformity of idiopathic scoliosis (ISc) is proposed. Postural studies reveal that during quiet stance, adaptation is marked in conditions associated with visual control of sway, particularly of lateral sway; during imposed perturbations of the body, destabilized postural reactions are pronounced in tests requiring visual-vestibular coupling. Observations of visual and/or vestibular generated eye movements indicate ocular instability among a high proportion of the ISc. Previously, the authors argued that a dir

Antithrombotic Therapy in Patients With Atrial Fibrillation With Prior Complex PCI: A Secondary Analysis of the Randomized ADAPT AF-DES Trial.

BACKGROUND: In patients with atrial fibrillation and chronic coronary syndrome beyond 12 months after percutaneous coronary intervention (PCI), oral anticoagulant monotherapy is recommended by the guideline; however, its efficacy and safety in patients with complex PCI remain uncertain. METHODS: We conducted a post hoc analysis of the randomized ADAPT AF-DES trial (Appropriate Duration of Antiplatelet and Thrombotic Strategy After 12 Months in Patients With Atrial Fibrillation Treated With Drug-Eluting Stents), which compared non-vitamin K antagonist oral anticoagulant (NOAC) monotherapy with

CRPS: A contingent hypothesis with prostaglandins as crucial conversion factor.

UNLABELLED: CRPS is an acute pain disease expressed as chronic pain with a severe loss of tissue and function. CRPS usually occurs after minor injuries and then progresses in a way that is scarcely controllable, or completely uncontrollable. This article addresses the functional control mechanism of a biological organism, a comparison of techniques, and the way the negative feedback mechanisms fail in regulated feedback systems. The measurement and regulation system is controlled at the local, regional, and central levels in a biological system. Locally generated substances such as prostagland

[Intellectual and motor development of extremely low birth weight (≤1000 g) children in the 7th year of life; a multicenter, cross-sectional study of children born in the Malopolska voivodship between 2002 and 2004].

INTRODUCTION: A better understanding of the developmental problems in extremely low birth weight (ELBW) preterm infants may enhance their chances of proper adaptation to their environment and make it possible to retrospectively assess perinatal and neonatal methods of treatment. The aim of the study was to evaluate the cognitive and motor development of ELBW children born from 2002 to 2004 in the 7th year of life. Based on these results and perinatal mortality data, it was established what chance the children have to live free of severe complications. MATERIAL AND METHODS: Two hundred and fou

Lung protection in acute respiratory distress syndrome: moving from protective ventilation to precision respiratory support.

PURPOSE OF REVIEW: Lung-protective ventilation focuses on limiting tidal volume and airway pressure to prevent ventilator-induced lung injury (VILI) in acute respiratory distress syndrome (ARDS). Nevertheless, patients exposed to similar ventilatory strategies often develop markedly different degrees of lung injury, indicating that conventional ventilatory targets alone do not fully account for VILI development. This review examines the evolving concept of lung protection, emphasizing load distribution, regional lung mechanics, respiratory effort, and host susceptibility. RECENT FINDINGS: Mec

Feasibility study of microbubble enhanced echocardiography in normal horses.

UNLABELLED: Intrapulmonary shunting, a contributor to hypoxemia in both anesthetized and spontaneously breathing horses, may occur in association with pulmonary disease, general inhalation anesthesia, or systemic inflammation. In human medicine, transthoracic microbubble-enhanced echocardiography is a widely used method to detect intrapulmonary right-to-left shunting, especially in hepatopulmonary syndrome. This study aimed to adapt and evaluate the feasibility of this technique in horses, laying the groundwork for future clinical applications. Twelve healthy horses underwent two treatments: (

Reproductive health of the sow during puerperium.

The modern hyperprolific sow is susceptible to metabolic disease and chronic inflammation. The most sensitive phase is parturition, when the sow experiences systemic inflammation and stress, and major changes in metabolism and endocrinology. Resolution of inflammation and stress needs to happen quickly to ensure good reproductive health during puerperium. If the sow fails to adapt to these changes, puerperal disease may occur. The economically most important puerperal disease complex is the postpartum dysgalactia syndrome (PPDS). Other puerperal diseases include infections of the urogenital tr

COVseq is a cost-effective workflow for mass-scale SARS-CoV-2 genomic surveillance.

While mass-scale vaccination campaigns are ongoing worldwide, genomic surveillance of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is critical to monitor the emergence and global spread of viral variants of concern (VOC). Here, we present a streamlined workflow-COVseq-which can be used to generate highly multiplexed sequencing libraries compatible with Illumina platforms from hundreds of SARS-CoV-2 samples in parallel, in a rapid and cost-effective manner. We benchmark COVseq against a standard library preparation method (NEBNext) on 29 SARS-CoV-2 positive samples, reaching 95.

Kleefstra syndrome: Recurrence in siblings due to a paternal mosaic mutation.

Kleefstra syndrome (KS) is a rare autosomic dominant genetic disorder caused by euchromatic histone methyltransferase 1 (EHMT1) alterations. Patients mainly present with moderate to severe intellectual disability, a severe delay in/or absence of speech, autism spectrum disorder, childhood hypotonia, neuropsychiatric anomalies, and distinctive dysmorphic features. Here, we report the cases of a male and a female, two younger siblings of three, with asymptomatic parents. An EHMT1 new mutation was identified. Both presented with a typical core phenotype. Some specific features were noted, such as

Targeted Transgene Expression in Cholinergic Interneurons in the Monkey Striatum Using Canine Adenovirus Serotype 2 Vectors.

The striatum, the main input structure of the basal ganglia, is critical for action selection and adaptive motor control. To understand the neuronal mechanisms underlying these functions, an analysis of microcircuits that compose the striatum is necessary. Among these, cholinergic interneurons (ChIs) provide intrinsic striatal innervation whose dysfunction is implicated in neuropsychiatric diseases, such as Parkinson's disease and Tourette syndrome. The ability to experimentally manipulate the activity of ChIs is critical to gain insights into their contribution to the normal function of the s

The Vagus Nerve at the Interface of the Microbiota-Gut-Brain Axis.

The microbiota, the gut, and the brain communicate through the microbiota-gut-brain axis in a bidirectional way that involves the autonomic nervous system. The vagus nerve (VN), the principal component of the parasympathetic nervous system, is a mixed nerve composed of 80% afferent and 20% efferent fibers. The VN, because of its role in interoceptive awareness, is able to sense the microbiota metabolites through its afferents, to transfer this gut information to the central nervous system where it is integrated in the central autonomic network, and then to generate an adapted or inappropriate

Tryptophan Biochemistry: Structural, Nutritional, Metabolic, and Medical Aspects in Humans.

L-Tryptophan is the unique protein amino acid (AA) bearing an indole ring: its biotransformation in living organisms contributes either to keeping this chemical group in cells and tissues or to breaking it, by generating in both cases a variety of bioactive molecules. Investigations on the biology of Trp highlight the pleiotropic effects of its small derivatives on homeostasis processes. In addition to protein turn-over, in humans the pathways of Trp indole derivatives cover the synthesis of the neurotransmitter/hormone serotonin (5-HT), the pineal gland melatonin (MLT), and the trace amine tr

Identification of small FRAXA premutations.

BACKGROUND: Diagnosis of fragile X syndrome in mentally retarded individuals is satisfactorily achieved using a Southern blot test that detects the typical triplet repeat expansion (>200 repeats) within the FMR1 gene. All such individuals inherit the mutation from a carrier, who usually shows a lower triplet repeat number and may be asymptomatic. Having identified a fragile X proband, it is necessary to identify related carriers of this familial X-linked dominant mutation to provide family counseling and testing. METHODS AND RESULTS: For one family in which a fragile XA repeat expansion occur

[Mechanical complications at implantation sites].

Mechanical complications of implanted venous access devices are more common than suggested by the literature. Among them, the most severe is catheter embolism, which is due primarily to costoclavicular pinch-off syndrome (POS). POS occurs mainly after infraclavicular approach of the subclavian vein, the incidence being 8/1000 in our experience. Clinical and radiological findings suggestive of rupture should be well known since they require removal of the device. Other access sites (internal jugular vein, cephalic vein, subclavian vein by the supraclavicular approach) seem preferable for long-t

Neuropsychological deficits in adolescents with fetal alcohol syndrome: clinical findings.

Understanding the nature of cognitive deficits among adolescent patients with fetal alcohol syndrome (FAS) can direct future research on assessment and intervention. In an exploratory study, nine nonretarded teenagers with FAS were administered tests of IQ and adaptive behavior, and neuropsychological tests presumed sensitive to alcohol effects. Their performance was compared with psychometric norms and to data from a sample of 174 adolescents with minimal or no prenatal alcohol exposure. These nonretarded FAS patients commonly showed behavior problems, decreased social competence, and poor sc

Fundamental concepts of convergent (parallel) evolution in human-pathogenic viruses and their implications for global health.

Various environmental conditions force viruses to continuously evolve to survive. Evolving viruses with improved fitness are subject to positive selection and will pass on their genetic information to the next generation. If virus populations experience similar environmental pressure, they may undergo a dynamic process of molecular adaptation, which is known as convergent or parallel evolution (parallelism). Noteworthy, these phenomena are among the underlying mechanisms of cross-species transmission and emergence of novel viruses in the human population with a significant impact on global hea
