OBJECTIVES: An understanding of the tumor immune microenvironment is required to improve treatment, especially the selection of immune checkpoint inhibitors (ICIs). In this study, we stratified the immunotypes of tongue squamous cell carcinoma (TSCC) based on the results of comprehensive immune profiling.
METHODS: We enrolled 87 therapy-naïve TSCC and 17 ICI-treated TSCC patients who underwent glossectomy without any other prior therapy. Comprehensive immune profile analyses employed multiplex immunofluorescence and tissue imaging.
RESULTS: Based on the hierarchies of 58 immune parameters an…

OBJECTIVES: The role of tongue reduction surgery (TRS) in preventing excessive mandibular growth and anterior open bite in children with Bec … iofacial growth pattern and dentoskeletal features in BWSp children.
CLINICAL RELEVANCE: Reductive glossectomy may not be justified for preventing or avoiding oro-facial deformities in BWSp; therefore, early monitoring of maxillofacial development of each affected child has a great clinical significance.

PURPOSE: The varied treatment options available to patients with HPV-positive oropharyngeal squamous cell carcinoma (OPSCC) can cause significant patient confusion. In particular, transoral robotic surgery (TORS) has become widely used for treatment of HPV-positive OPSCC. As patients commonly refer to the internet for additional information, we aim to evaluate the quality of online patient educational materials for TORS in comparison to other otolaryngology surgical procedures.
METHODS: The terms "transoral robotic surgery," "glossectomy," "thyroidectomy," and "neck dissection" were searched …

OBJECTIVE: To perform an evidence-based medicine review of the literature describing outcomes of hypopharyngeal surgery in obstructive sleep apnea.
DESIGN: We performed a MEDLINE search of English-language articles or abstracts using the keywords sleep and surgery in combination with any of the following terms: hypopharynx, tongue, tongue base, epiglottis, genioglossus, advancement, mortised, genioplasty, glossectomy, tongue radiofrequency, hyoepiglottoplasty, hyoid, suspension, and stabilization. Additional studies were identified from their reference lists. We reviewed abstracts to select p…

Selective tracheostomy after resection of oral tongue cancer remains contentious, particularly when reconstruction uses intraoral myomucosal flaps whose bulk and edema are not well represented in existing airway risk scores. We retrospectively reviewed 62 consecutive patients who underwent partial glossectomy with myomucosal (buccinator/FAMM-type) flap reconstruction. Primary outcome was elective tracheostomy; secondary outcomes included airway events, ICU utilization, and length of stay (LOS). We compared clinical decisions with the Cameron and Gupta/CASST scores and fitted a multivariable lo…

BACKGROUND Solid organ transplant recipients carry an elevated risk of de novo malignancies under chronic immunosuppression, including aggressive squamous cell carcinoma (SCC) of the oral cavity and oropharynx. Practical, evidence-based guidance for maxillofacial management in this population remains limited. CASE REPORT Case 1 involved a 44-year-old man who developed T1N0 SCC of the lateral tongue 23 months after orthotopic liver transplantation for alcoholic cirrhosis. He was treated with partial glossectomy and selective neck dissection, followed by re-excision for a positive deep margin; h…

BACKGROUND: Post-operative massive hemorrhage is a critical concern in oral cancer surgery, associated with severe complications and heightened morbidity and mortality rates.
CASE SUMMARY: A 46-year-old male with advanced poorly differentiated squamous cell carcinoma (ypT4aN3bN0M0) of the oral floor underwent extensive surgery, including total glossectomy, partial mandibulectomy, and free flap reconstruction. Postoperatively, he developed life-threatening hemorrhage on day 3 due to wound dehiscence. Rapid nursing interventions-prompt suture removal, pressure hemostasis, and multidisciplinary …

Standardized technique and outcome of surgical reduction of macroglossia have yet to be determined. The aim of our study is to review current literature regarding the clinical and surgical management of primary macroglossia and postoperative outcomes in the pediatric population. A systematic review was performed between the years of 1995 and 2024 according to PRISMA guidelines. Search terms included "glossectomy," "children," and "macroglossia." Studies were analyzed for level of evidence, sample and control sizes, indications for surgery, surgical reduction technique, complications, follow-up…

OBJECTIVES: The incidence of oral tongue squamous cell carcinoma is rising in young patients. This study evaluated the clinical, pathological, and prognostic characteristics of oral tongue squamous cell carcinoma in the under-30-year age group.
MATERIALS AND METHODS: The computerized database of the Department of Otolaryngology-Head and Neck Surgery of a tertiary, university-affiliated medical center was searched for all patients with oral tongue squamous cell carcinoma treated by glossectomy with curative intent in 1996-2012. Data were collected by chart review.
RESULTS: Of the 113 patients…

BACKGROUND: Cardiac tumours are typically secondary in nature, and the most common malignancies metastasizing to the heart are cancers of the lung, breast, oesophagus, melanoma, and lymphoma. We present a unique case of squamous cell carcinoma of the tongue, metastasizing to the heart and manifesting with ST elevation in the inferior-leads on electrocardiogram (ECG).
CASE SUMMARY: A 25-year-old woman was initially diagnosed with squamous cell carcinoma of the tongue at the age of 23 and treated with hemi-glossectomy with clear-margins. Sixteen months later, the tumour recurred in the orophary…

A 74-year-old man was referred to a tertiary academic otolaryngology clinic for evaluation of a left-sided neck mass with unknown primary. Nuclear imaging modalities revealed a primary cancer located at the left tongue base. Further investigation revealed the tumour to be a p16 positive squamous cell cancer with metastatic spread to cervical lymph nodes of multiple levels. The patient was found on initial investigation to have microstomia and a retrognathic mandible, which are typically considered unsuitable for robotic surgery due to difficulties obtaining adequate exposure. The patient underw

The current standard of care in oral tongue cancer surgery is complete resection with a target of 5-mm microscopic clearance at all margins on final pathologic review. While current methods of resection are often successful at determining the mucosal margins of the lesion, they may be limited when attempting to achieve an adequate deep margin. A number of previous studies suggested that ultrasound is superior to manual palpation and other imaging modalities (computed tomography, magnetic resonance imaging) at demarcating the margins of tongue lesions. Recent clinical reports of the intraoperat

PURPOSE: To define factors influencing postoperative aspiration in tongue cancer patients and to analyze the characteristics of dysphagia before and after surgery.
MATERIALS AND METHODS: A total of 112 tongue cancer patients participated in this work. Videofluoroscopic swallowing studies were performed in all patients before and after surgery. A Penetration-Aspiration Scale score of 3 or greater was defined as an aspiration risk. Qualitative data were collected on a frame-by-frame basis from each videofluoroscopic swallowing study and analyzed.
RESULTS: Smoking (58.14%, P < .01), tongue rese

Anatomical variations of the spinal accessory nerve are well known. We describe a previously unreported variant in which the nerve divided high in level II after crossing the internal jugular vein and before entering the sternomastoid muscle. Both branches were joined by a communication from the C2 cervical root. We discuss the clinical implications of this finding.
