Assessing environmental contamination in Dutch and Belgian nursing homes: a cross-border ATP-based study.

BACKGROUND: To reduce the spread of micro-organisms in nursing homes adequate measurement of cleaning is necessary. ATP measurements can be a helpful tool in quantifying contamination. AIM: To objectively measure environmental contamination within multiple nursing homes, such that cleaning can be focused on the most contaminated fomites. METHODS: Standardized ATP measurements were conducted in 28 nursing homes on multiple wards. Fifteen pre-defined surfaces per ward were measured with the 3M Clean Trace NG luminometer. Results are displayed in relative light units (RLU). RLU >1000 was consid

Usefulness of the Palliative Prognostic Index in Predicting Prognosis when Considering the Transition from Hospital to Home Care in Patients with Terminal Stage Cancer.

BACKGROUND: No accurate prognostic tool is available for patients with cancer who spend their final days at home. In this study, we examined whether performance status (PS) and the palliative prognostic index (PPI), a well-known prognostic tool in palliative care units, could be used to predict prognosis in the home care setting at the time of intervention by home physicians. SUBJECTS AND METHODS: Using medical records, we conducted a retrospective analysis of 132 patients who were referred to the Home Clinic Naginoki for home care for terminal stages of carcinoma in situ. Based on the status

Home-based physical therapy intervention with adherence-enhancing strategies versus clinic-based management for patients with ankle sprains.

BACKGROUND AND PURPOSE: To some extent, favorable treatment outcomes for physical therapy intervention programs depend on patients attending their clinic appointments and adhering to the program requirements. Previous studies have found less-than-optimal levels of clinic attendance, and a viable option might be physical therapy intervention programs with a large component of home treatment. This study investigated the effects of a standard physical therapy intervention program--delivered primarily at either the clinic or home--on ankle function, rehabilitation adherence, and motivation in pati

Using simulation modeling to inform intervention and implementation selection in a rapid stakeholder-engaged hybrid effectiveness-implementation randomized trial.

BACKGROUND: Implementation research generally assumes established evidence-based practices and prior piloting of implementation strategies, which may not be feasible during a public health emergency. We describe the use of a simulation model of the effectiveness of COVID-19 mitigation strategies to inform a stakeholder-engaged process of rapidly designing a tailored intervention and implementation strategy for individuals with serious mental illness (SMI) and intellectual/developmental disabilities (ID/DD) in group homes in a hybrid effectiveness-implementation randomized trial. METHODS: We u…

Morbidity and mortality patterns of ventilator-dependent children in a home care program.

We evaluated the effectiveness of the Home Care Program of Children's Hospital of Winnipeg for ventilator-dependent children by retrospectively examining morbidity and mortality from February 1, 1979, to July 31, 1992. For the 22 study subjects, the cause of chronic respiratory failure was neurologic disorders for 14 (64%) (group A) and pulmonary disorders for eight (36%) (group B). There were no significant differences between groups A and B in the average number of hospital days, readmission rate, or length of stay per admission. Eleven patients have remained ventilator-dependent at home, fo

Adopting assistive technologies for the support and care of people with mild dementia living at home-Understanding the decision-making process: a qualitative study.

BACKGROUND: Assistive technologies may play a crucial role in addressing needs of people with dementia. While technical feasibility often drives development, the decision-making process that might lead to their adoption (or rejection) is not fully understood. We aim to explore contextual factors influencing this process and "necessities" technology should fulfil from the user-perspective, so that the use of assistive technologies can be better targeted- potentially fostering a more supportive home environment. METHODS: In this qualitative study, interviews (8 people with dementia and 7 partne

Enablers and barriers of domiciliary midwifery practice using the socio-ecological model: a qualitative study exploring the perspectives of students and lecturers in Uganda.

BACKGROUND: Early postnatal discharge is increasingly common worldwide, yet the World Health Organization’s recommended 3-3-6 schedule for postnatal visits and assessments is often not adhered to by mothers and healthcare workers. The early neonatal period is critical for both mother and baby, accounting for a significant proportion of maternal and neonatal deaths, especially in developing countries. According to the recently released Midwifery Accelerator, achieving universal coverage of care provided by midwives can avert two-thirds of maternal and newborn deaths, including stillbirths. Domi

Structural violence in long-term, residential care for older people: comparing Canada and Scandinavia.

Canadian frontline careworkers are six times more likely to experience daily physical violence than their Scandinavian counterparts. This paper draws on a comparative survey of residential careworkers serving older people across three Canadian provinces (Manitoba, Nova Scotia, Ontario) and four countries that follow a Scandinavian model of social care (Denmark, Finland, Norway, Sweden) conducted between 2005 and 2006. Ninety percent of Canadian frontline careworkers experienced physical violence from residents or their relatives and 43 percent reported physical violence on a daily basis. Canad

Characteristics of patient and staff victims of assaults in community residences by previously nonviolent psychiatric inpatients.

There currently exists a large body of empirical research examining patient assaults in inpatient psychiatric facilities. These studies have focused primarily on staff and have found younger, male mental health workers with lower levels of formal education and experience as well as nurses involved in restraint procedures to be most at risk. However, despite the increased utilization of community-based services, little attention has been directed toward patient assaults on patients and staff in community settings, particularly residential services. This study began to respond to this need by ex

Staff stress on an Alzheimer's unit.

Nurses working with the elderly encounter many complex and potentially stressful care situations. Nowhere is this more true than for nursing home staff who work in highly demanding, labor- and client-intensive jobs. A number of factors contribute to the high levels of stress found among nurses who work with the elderly, including continual exposure to physical and emotional pathology and death, and conflict with families, co-workers, supervisors, and representatives from other departments or professions (Goldin, 1985; Klus, 1980). When the stress level in a long-term care facility becomes over

Interventions with care givers of dementia patients: comparison of two approaches.

We compared the effect of two approaches, family counseling and support groups, for relieving the stress and burden experienced by care givers of dementia patients. Both treatments are designed to implement features of a stress-management model that have been identified in prior research with this population: providing information about the patient's disease and its effects on behavior, teaching behavioral problem solving for managing difficult behavior, and identifying potential support for care givers. Subjects were primary care givers of dementia patients living in the community; they inclu
