The foot is an ideal donor site for reconstruction of defects in the hand. Although standard toe transfer variants such as great or second toe transfer are commonly used, other variants can be designed, depending on the requirements of the defect. In this article, the authors illustrate the use of toe transfer variants to reconstruct different defects in the hand following functional and aesthetic units. Where possible, all 10 toes should be preserved. Specifically, we discuss reconstruction of pulp defects, nail complex defects, proximal interphalangeal joint defects, web space defects, compl

PURPOSE: To present a radiographic follow-up study of the use of a nonvascularized iliac crest bone graft as a treatment for modified Blauth type IIIB thumb hypoplasia.
METHODS: From January 2015 to December 2019, nonvascularized iliac crest bone grafts were used to reconstruct the first metacarpal in 23 cases with type IIIB thumbs. The average follow-up duration was 1.9 years (range 1.0-3.9 years). We evaluated the patients' serial x-rays and measured the width and length changes of the reconstructed first metacarpals.
RESULTS: Survival of the graft, judged radiographically, was achieved in

Survivors of hematopoietic cell transplantation (HCT) have been shown to exhibit both clinical and biological features of accelerated ageing. Most studies used frailty measures, comorbidities for clinical assessment and several biological assessment of premature ageing. However, these tests are less suitable for age determination of individual patients. Recently, DNA methylation has emerged as a novel test to measure cellular age. In the present study, we assessed ageing in a cohort of 26 survivors of allogeneic HCT by frailty tests comprising the handgrip and 6 min walk tests and by biologica

Background: A hypothenar perforator free flap, which is harvested from the palm, is suitable for reconstruction of a finger pulp skin defect, but this flap has rarely been reported, and most of these flaps in previous published reports were designed on the proximal zone of the hypothenar area. In the present report, hypothenar perforator free flaps on the distal zone, namely distal hypothenar perforator free flaps, including two sensate flaps, were used for four cases with pulp defects. Methods: Four distal hypothenar perforator free flaps were used for four patients with skin defects of the f

Restoration of function following mutilating injuries to the extensor surface of the hand has traditionally involved staged reconstruction with distant pedicle flaps, followed by tendon grafting. Advances in microsurgery now permit the transfer, in one operation, of vascularized composite tissues such as skin and muscle, or skin and bone. For dorsal hand reconstruction a composite flap of skin, tendons and nerves from the dorsum of the foot was transferred in one stage soon after injury, avoiding unnecessary disabling scar formation, prolonged hospitalization and resulting in the rapid restora

The World Health Organization declares in its Guiding Principles on Human Cell, Tissue and Organ Transplantation that organizations must ensure that the personal anonymity and privacy of donors and recipients are always protected. Hence, most of the European transplantation programs forbid any form of direct contact between the family of the deceased donor and the transplant recipient. The anonymity is guarded to protect both parties against potential abuse, manipulation or financial pressure. However, legislation on anonymity of organ donation has come under discussion in the last few years,

Treatment of central longitudinal deficiencies is well defined, with different techniques established for the classical clefts. However, none of these techniques is easily applied to the treatment of very deep clefts accompanied by a significant divergence of the metacarpal bones. In such cases, the results of current techniques are disappointing. We propose a new technique of "Translocation in the Radial direction of the Ulnar Finger(s)" (TRUF) by intra-carpal osteotomy. The results are illustrated in three clinical cases. The TRUF operation allowed closing of the cleft, alignment of the meta

OBJECTIVES: We evaluated the long-term results of treatment with curettage followed by an autograft or allograft application in patients with enchondroma of the hand.
METHODS: Within a 15-year period, 76 patients (41 males, 35 females; mean age 32 years; range 14 to 47 years) were operated on for enchondroma of the hand. Following curettage of the lesion, reconstruction of the defect was made either by an autograft obtained from the iliac crest (n=76) or by a dehydrated cancellous allograft (n=15). The diagnosis was histologically confirmed in all the cases. Functional and radiographic result

Nine vascularized autogenous whole joint transfers were employed in the reconstruction of traumatized joints of six male patients ages 6 to 38 years. The proximal interphalangeal joint was involved in four patients, the thumb metacarpophalangeal (MP) joint in two, and the small finger MP joint in one. In five patients, the donor joint came from the foot, and in one patient an MP joint was transferred from a digit that had sustained a more distal amputation. The mean follow-up was 24 months (range 13 to 38). Bony union as well as full radiographic preservation of the articular space has occurre

The objective of this study was to characterize the cortical activity pattern of one patient who received bilateral forearm transplants. Using fMRI we acquired motor and sensory brain activity every year after surgery and during three consecutive years while the patient underwent physical rehabilitation. The motor related cortical activity evaluated during the first year showed a sparse pattern involving several brain regions. Over time, the analysis showed a progressive delimitation of the motor-related areas that had significant activity. The results also showed continuous size reductions of

Toxoplasmosis is a widely distributed zoonosis produced by the parasite T. gondii. In Chile the seroprevalence has been estimated between 20-37% in general population. Defined risk groups acquire or reactivate the infection by T. gondii in patients undergoing SOT and HSCT are: heart transplant or heart-lung with D (+) and R (-), allogeneic HSCT with R (+), HSCT with cord cells, GVHD, history of previous clinical toxoplasmosis and use of corticosteroids for prolonged periods or in high doses. BACKGROUND: Uremic toxins have emerged as potential mediators of morbidity and mortality in patients with chronic kidney disease (CKD). Indole-3-acetic acid (IAA, a tryptophan-derived uremic toxin) might be a useful biomarker in patients with CKD. The objectives of the present study were to (i) describe IAA concentrations in a cohort of non-transplanted patients with CKD and a cohort of transplanted patients with CKD, and (ii) investigate the possible relationship between IAA levels and adverse outcomes in the two cohorts.
METHODS: Levels of free and total IAA were assayed in the two prospect

BACKGROUND: In lower root avulsion plexopathies, free muscle transfers for hand reanimation provide the only hope for the paralyzed hand, as the outcomes of hand functional restoration after primary brachial plexus reconstruction are uniformly poor. The purpose of this study was to analyze the outcomes of free gracilis muscle transfers for hand reanimation in severe brachial plexus injuries in relation to the respective motor donors.
METHODS: Since 1981, 71 free gracilis muscles have been transplanted for hand reanimation. Thirty-eight were for finger flexion and 33 were for finger extension.

The authors describe two cases of distraction of the bones of the forearm, using Ilizarov's distraction method. In the first case the author treated a patient with aplasia of the 1st metacarpal bone, a hypoplastic thumb on a dermal stalk and a hypoplastic radius on the right. After operations which were not effective (extension of the dermal stalk of the hypoplastic thumb by a Z-plasty, correction of the position of the hand by means of an osseo-cartilaginous transplant) and after pollicization of the index finger they decided for distraction of the radius by means of an original compressive-d

BACKGROUND: Due to the generally increasing number of obese patients with obesity-associated comorbidities (e.g. type 2 diabetes mellitus and nonalcoholic fatty liver disease/steatohepatitis), they are increasingly becoming transplantation candidates; however, this patient cohort is more frequently affected by intraoperative and postoperative complications and poorer transplant outcome.
OBJECTIVE: This article provides an overview of the indications, choice of procedure and outcome of bariatric surgery prior to solid organ transplantation.
MATERIAL AND METHODS: The current literature was eva

In vascularized composite allograft (VCA) for upper limb reconstruction, upper arm-level or transhumeral transplantations are performed less frequently than forearm and hand transplantations. Transplantation is technically more feasible at this proximal level, largely owing to the use of macrovascular anastomoses. Despite these challenges, the outlook for arm allotransplantation remains encouraging, and this protocol provides a standardized technique for harvesting a vascularized upper arm composite allograft, ensuring both optimal outcomes and minimal tissue trauma. In the case of upper arm t…

Solid organ transplant patients are at greatly increased risk of developing a wide variety of skin cancers, particularly epithelial skin cancers. On the other hand, it is well known that an intact immune system limits the development of benign melanocytic lesions. The eruptive nevi phenomenon, which we can observe in solid organ transplant recipients, is indicative of the relationship between melanocyte proliferation and immune system. Regression of melanocytic nevi after restoration of complete immune responsiveness is a further clinical example the role of immunosurveillance on melanocyte pr

In May, 2008, the Istanbul Declaration calling for completely against organ traffic and for self-sufficiency in organ donation was presented by a Steering Committee convened by The Transplantation Society (TTS) and the International Society of Nephrology (ISN) under the cooperation with WHO. The chief principles are to spread cadaver transplant worldwide for prevention of improper organ donation, and to protect and secure living organ donors. On the other hand, in Japan brain-dead transplantation had been legalized in 1997. Since the first Organ Transplant Law went into effect without sufficie
