Designing trustworthy, low-threshold care coordination: co-creating a hybrid CareCompass with rural informal caregivers.

INTRODUCTION: As health systems across Europe face the dual pressures of environmental change and demographic aging, the need for a socio-ecological transformation that integrates health, equity, and sustainability has never been more pressing. Informal caregivers are central to long-term care systems, yet they often face time poverty, fragmented support, and unequal access to resources. These are conditions that can also constrain participation in health-promoting and climate-friendly everyday practices. This study reports a participatory co-design process that developed and iteratively refin

Technical and management coaching for government institutions: Lessons learned and health systems transformations across 8 countries in sub-Saharan Africa and India.

Traditional engagement with local governments often relies on financial and human resources from international or local partners, leading to direct implementation by organizations, which can hinder sustainability. While some organizations include sustainability indicators, few focus on transferring technical and financial ownership to governments. The Challenge Initiative (TCI) uses a phased coaching model-lead, assist, observe, and monitor-to build local government capacity for scaling family planning (FP) and adolescent and youth sexual and reproductive health (AYSRH) programs. TCI ensures l

The longitudinal primary care of transgender and gender diverse people.

BACKGROUND: With more transgender and gender diverse (TGD) people affirming their genders, often with gender‑affirming hormone therapy (GAHT) - which involves using hormones to help align physical characteristics with an individual's gender identity - there is a growing need for general practitioners who are confident and skilled in providing affirming, ongoing care. This includes support related to not only gender affirmation itself, but also broader health needs such as cardiovascular disease risk management, mental health, bone health, sexual health and wellbeing, and lifestyle factors incl

Later-Life Friendship in Advance Care Planning: Variation by Marital Status and Gender.

Friendship is an understudied social context in research on advance care planning (ACP). Using data from the Health and Retirement Study (2018-2020), we examine the associations between the quality and quantity of friendships and older adults' engagement in advance directives (AD) and end-of-life (EOL) discussions. Multivariable logistic regressions show that having any friends is associated with greater odds of AD and EOL discussions. Number of close friends and emotional support are positively associated with AD only. Marital status and gender, both independently and jointly, moderate the as

Hurricane preparedness: Current procedures at Blue Cross Blue Shield of Florida.

This paper discusses experience, methodology and recommendations for successful business continuity and disaster recovery planning for health care organisations. Hurricanes, tornadoes and other natural disasters are a regular occurrence in Florida. Low-lying coastal areas are at increased risk, with populations in inland areas as far as 200-300 miles with potential to suffer heavy damage. This case study shows how one institution, Blue Cross Blue Shield of Florida, provides and maintains emergency response plans for critical functions, services or processes before, during and after a disaster,

Social Aspects of Dementia Prevention from a Worldwide to National Perspective: A Review on the International Situation and the Example of Italy.

At the moment, dementia is affecting around 47 million people worldwide, with a forecast amount of 135 million affected people in 2050. Dementia is a growing health concern worldwide with no treatment currently available, but only symptomatic medication. Effective interventions in the prevention and management of dementia are urgently needed to contain direct and indirect costs of this disease. Indeed, the economic impact of dementia is a vast and continually growing figure, but it is still difficult to quantify. Due to an increase in both the disease spreading and its direct and indirect cost

Developing a mental health care plan in a low resource setting: the theory of change approach.

BACKGROUND: Scaling up mental healthcare through integration into primary care remains the main strategy to address the extensive unmet mental health need in low-income countries. For integrated care to achieve its goal, a clear understanding of the organisational processes that can promote and hinder the integration and delivery of mental health care is essential. Theory of Change (ToC), a method employed in the planning, implementation and evaluation of complex community initiatives, is an innovative approach that has the potential to assist in the development of a comprehensive mental healt

Mental Health and Placenta Accreta Spectrum.

Nearly half of women describe childbirth as traumatic in some way, making them more vulnerable to perinatal psychiatric illness. Patients with high risk pregnancies, such as abnormal placentation, are even more susceptible to childbirth related mental health sequelae. There are no formal recommendations for mental health intervention in women with placenta accreta spectrum (PAS). In many institutions, the Edinburgh Postpartum Depression Scale is used to assess for depressive and anxiety symptoms during pregnancy and postpartum. Women with PAS should be screened at time of diagnosis, monthly un

A home care wound care challenge: its rewards, inspiration, and positive outcomes.

Quite often, home health team members are presented with a challenge to provide management for a patient who has undergone a complex gastrointestinal surgery. An appropriate plan of care includes the incision, wound, and colostomy care, as well as the psychosocial support, time management, and cost issues. Many times this requires ingenuity and flexibility on the part of the home care team. Comorbidities often increase the acuity of the nursing care as well. In any case and no matter how difficult the circumstances, the goal for the patient's plan of care is to achieve an optimal outcome that

Transition Planning For After Polio Eradication.

The Global Polio Eradication Initiative (GPEI) has been in operation since 1988, now spends $1 billion annually, and operates through thousands of staff and millions of volunteers in dozens of countries. It has brought polio to the brink of eradication. After eradication is achieved, what should happen to the substantial assets, capabilities, and lessons of the GPEI? To answer this question, an extensive process of transition planning is underway. There is an absolute need to maintain and mainstream some of the functions, to keep the world polio-free. There is also considerable risk-and, if se
