Looking into the future: a machine learning powered prediction model for oocyte return rates after cryopreservation.

RESEARCH QUESTION: Could a predictive model, using data from all US fertility clinics reporting to the Society for Assisted Reproductive Technology, estimate the likelihood of patients using their stored oocytes? DESIGN: Multiple learner algorithms, including penalized regressions, random forests, gradient boosting machine, linear discriminant analysis and bootstrap aggregating decision trees were used. Data were split into training and test datasets. Patient demographics, medical and fertility diagnoses, partner information and geographic locations were analysed. RESULTS: A total of 77,631

Systematic review of health-related quality of life in patients undergoing pelvic exenteration.

BACKGROUND: Pelvic exenteration is a radical operative treatment reserved for the management of a number of advanced primary and recurrent pelvic malignancies, including, rectal, gynaecological and urological. The advances in radiological staging, surgical techniques and greater use of chemotherapeutic agents haves translated clinically into improvements in the overall survival of this cohort of patients, irrespective of underlying disease pathology. Consequently, a greater proportion of the surviving population will present to healthcare professionals with a range of physical and psychologica

Integrated care programmes for severe mental illnesses and comorbid physical health disorders: the EU-MIND umbrella review to inform future actions in Europe.

BACKGROUND: People with severe mental illness (SMI) represent up to 6% of the population and experience a high burden of physical comorbidity and premature mortality. Despite longstanding international calls to develop integrated care, the optimal organisational approaches for this population remain poorly defined, and health systems lack guidance on how to redesign care pathways. This review aimed to characterise the core components, reported impacts, and implementation challenges of existing integrated care programmes for individuals with SMI and physical health conditions. METHODS: This um

The scoping review aimed to map strategies for the integrated management of HIV and TB services in West Africa and identify barriers and enablers influencing implementation. METHODS: The Arksey and O'Malley scoping review design was used for the study. The PRISMA extension for scoping review guidelines

Economic Evaluation of Pneumococcal Vaccination in Egypt: Cost-Effectiveness, Budget Impact, and Domestic Manufacturing Potential.

Background/Objectives: Streptococcus pneumoniae remains a major cause of morbidity and mortality in Egypt, yet pneumococcal conjugate vaccines (PCVs) are not included in the national immunization program. Recent commitments to domestic vaccine manufacturing and temporary Gavi support create a timely decision context for policymakers to assess whether PCV introduction is cost-effective, affordable, and sustainable within Egypt's health financing constraints. This study evaluates the cost-effectiveness, budget impact, and return on investment (ROI) of PCV introduction in Egypt. Methods: A determ

Integration of tuberculosis and diabetes mellitus care in health systems: a cross-sectional analysis of healthcare facilities in Lagos, Nigeria.

BACKGROUND: Despite WHO recommendations for integrated care since 2011, implementation of tuberculosis (TB) and diabetes mellitus (DM) care integration remains poorly documented in sub-Saharan Africa. We assessed integration and identified implementation barriers across healthcare facilities in Lagos, Nigeria. METHODS: Cross-sectional survey of 100 facilities providing TB or diabetes services in Lagos State, January to June 2019, drawn by stratified random sampling from a frame of 237 facilities. Structured questionnaires based on WHO guidelines assessed integration measures, and twelve focus

From campaign to continuity: stakeholders' recommendations for integrating HPV vaccination into Nigeria's healthcare system.

BACKGROUND: For Human Papillomavirus (HPV) vaccination to be delivered effectively, countries must transition from campaign-based introduction to integration within the routine immunization schedule. While existing literature provides insights into the effectiveness of HPV campaigns and barriers to initial vaccine acceptance, there is a dearth of stakeholder-informed implementation research around the transition from campaign to routine delivery. This study aimed to explore the experiences and perspectives of key stakeholders involved in the HPV vaccine campaign in Nigeria to generate stakehol

BACKGROUND: Early-onset colorectal cancer is now the leading cause of cancer death among adults under 50, with delays in diagnosis cited as … ng. LIMITATIONS: Findings may not be generalizable outside the United States, given differences in healthcare financing, access, and screening practices. Using late-stage diagnosis as a proxy for delays is also debated and could confound stage-based inferences about delay. Few studies capture the subjective experiences of younger patients or the clinical reasoning an …

Challenges and countermeasures of the "Xinjiang Model" in tuberculosis prevention and control: a qualitative study based on health specialists' perspectives.

BACKGROUND: Tuberculosis (TB) remains a significant public health threat, particularly in Xinjiang Uygur Autonomous Region of China, showing a high incidence. The "Xinjiang Model" is a TB service delivery system integrating universal chest X-ray screening, hospitalization for infectious patients, community-based follow-up for non-infectious cases, and nutritional support. Since 2018, Xinjiang has implemented the "Xinjiang model" for TB prevention and control to improve early screening and cure rates, but it continues to face challenges in long-term implementation. OBJECTIVE: This qualitative

Universal Health Service Coverage under threat? A study of the effects of PEPFAR HIV funding on UHC.

Introduction: While the effects of the US government's sudden pause of foreign aid on the global HIV response have been well documented, little is known about the likely impact on universal health coverage (UHC) in recipient countries. This study assesses the association between PEPFAR funding and the universal health service coverage index. Methods: Using data on PEPFAR investments in 24 countries between 2004 and 2020, the study applied fixed and time effects panel data regression on the main UHC index and its sub-indices for non-communicable diseases (NCDs), reproductive, maternal, newborn a

Wading through Molasses: A qualitative examination of the experiences, perceptions, attitudes, and knowledge of Australian medical practitioners regarding medical billing.

BACKGROUND: Medical billing errors and fraud have been described as one of the last "great unreduced healthcare costs," with some commentators suggesting measurable average losses from this phenomenon are 7% of total health expenditure. In Australia, it has been estimated that leakage from Medicare caused by non-compliant medical billing may be 10-15% of the scheme's total cost. Despite a growing body of international research, mostly from the U.S, suggesting that rather than deliberately abusing the health financing systems they operate within, medical practitioners may be struggling to under

The integration of the global HIV/AIDS response into universal health coverage: desirable, perhaps possible, but far from easy.

BACKGROUND: The international community's health focus is shifting from achieving disease-specific targets towards aiming for universal health coverage. Integrating the global HIV/AIDS response into universal health coverage may be inevitable to secure its achievements in the long run, and for expanding these achievements beyond addressing a single disease. However, this integration comes at a time when international financial support for the global HIV/AIDS response is declining, while political support for universal health coverage is not translated into financial support. To assess the risk
