Therapy duration and improvement of ventricular function in de novo heart failure: the Heart Failure Optimization study.

BACKGROUND AND AIMS: In patients with de novo heart failure with reduced ejection fraction (HFrEF), improvement of left ventricular ejection fraction (LVEF) is expected to occur when started on guideline-recommended medical therapy. However, improvement may not be completed within 90 days. METHODS: Patients with HFrEF and LVEF ≤ 35% prescribed a wearable cardioverter-defibrillator between 2017 and 2022 from 68 sites were enrolled, starting with a registry phase for 3 months and followed by a study phase up to 1 year. The primary endpoints were LVEF improvement > 35% between Days 90 and 180 fo…

Oral and gut microbiome alterations in heart failure: Epidemiology, pathogenesis and response to advanced heart failure therapies.

Despite significant advances in therapies, heart failure (HF) remains a progressive disease that, once advanced, is associated with significant death and disability. Cardiac replacement therapies with left ventricular assist device (LVAD) and heart transplantation (HT) are the only treatment options for advanced HF, while lifesaving they can also be lifespan limiting due to the associated complications. Systemic inflammation is mechanistically important in HF pathophysiology and progression. However, directly targeting inflammation in HF has not been beneficial thus far. These failed attempts …

New-onset heart failure after acute coronary syndrome in patients without heart failure or left ventricular dysfunction.

INTRODUCTION AND OBJECTIVES: Coronary heart disease is the leading cause of heart failure (HF). The aim of this study was to assess the risk of readmission for HF in patients with acute coronary syndrome without previous HF or left ventricular dysfunction. METHODS: Prospective study of consecutive patients admitted for acute coronary syndrome in 2 institutions. Risk factors for HF were analyzed by competing risk regression, taking all-cause mortality as a competing event. RESULTS: We included 5962 patients and 567 (9.5%) experienced at least 1 hospital readmission for acute HF. Median follow…

Raised sympathetic nerve activity in heart failure and central sleep apnea is due to heart failure severity.

BACKGROUND: Congestive heart failure (CHF) patients with central sleep apnea (CHF-CSA) have elevated plasma norepinephrine (NE) compared with CHF patients without apnea (CHF-N). Patients with CHF-CSA also demonstrate higher mean pulmonary artery pressure (PAP), which is suggestive of worse cardiac function. Whether CSA contributes to chronic elevation of sympathetic nerve activity or is associated with more severe CHF remains unknown. We measured awake total body and cardiac NE spillover and related these to measurements of cardiac hemodynamics and apnea severity in CHF patients with CSA, with…

Management of heart failure: evidence versus practice. Does current prescribing provide optimal treatment for heart failure patients?

Heart failure is an increasingly common and costly chronic disorder, with a rising prevalence of at least 2% in populations over the age of 45 years, mortality rates that are as poor as common solid cancers, and very high health care utilisation costs. Despite increased evidence supporting a range of effective interventions, predominantly therapeutic, there remain significant degrees of physician underperformance in terms of heart failure diagnosis and management. Until the early 1990s, the management of heart failure was largely confined to the symptomatic relief of patients with well establi…

Management of heart failure. IV. Anticoagulation for patients with heart failure due to left ventricular systolic dysfunction.

OBJECTIVE: This article reviews the incidence of arterial thromboembolism in patients with heart failure who are not receiving anticoagulants. We also examine whether more severe ventricular dysfunction increases this incidence and the efficacy and risks of anticoagulation for patients in sinus rhythm. DATA SOURCES: English-language studies referenced in MEDLINE or EMBASE (January 1966 to September 1993) were reviewed. We used the search terms heart failure, congestive; congestive heart failure; heart failure; cardiac failure; and dilated cardiomyopathy in conjunction with the terms anticoagu…

Acute heart failure in emergency settings.

Acute heart failure (AHF) represents a major cause of emergency medical care worldwide and continues to carry a substantial burden of morbidity and mortality despite advances in chronic heart failure management. Most patients with AHF present to emergency departments (EDs), where the first hours of care are critical to determine prognosis. Of these, some have been previously evaluated and treated by prehospital emergency medical services before ED arrival. This consensus document summarizes the current evidence and provides practical advice to emergency physicians for the diagnosis, treatment,…

Remote Monitoring Devices and Heart Failure.

Remote patient monitoring (RPM) in patients with heart failure (HF) involves transmitting physiological data from devices to a health-care provider via a wireless connection with targeted interventions when values exceed the preset threshold. Devices used in telemonitoring range from weighing scales, blood pressure cuffs, and pulse oximeters to devices used to measure cardiac filling pressure and intrathoracic impedance using cardiac implantable electronic devices and wearables. Accordingly, RPM devices can potentially engage patients in their cardiovascular care and reduce the burden of HF in…

Myocardial Histopathology-Based Heart Failure Phenotyping.

BACKGROUND: Myocardial hypertrophy and interstitial fibrosis are the structural hallmarks evaluated on histopathology in heart failure (HF). Direct study of myocardial tissue has reemerged as a strategy to dissect the biology of heart failure with preserved ejection fraction (HFpEF), but whether these features, assessed together, define distinct subgroups within HF phenotypes remains unknown. OBJECTIVES: The authors assessed whether phenogroups defined by fibrosis and hypertrophy (histogroups) display distinct clinical profiles and outcomes in HFpEF vs heart failure with reduced ejection frac…

Palliative Care in Advanced Heart Failure.

PURPOSE OF REVIEW: Advanced heart failure (HF) is characterized by high symptom burden, poor quality of life (QoL), and recurrent hospitalizations. As survival improves, an increasing proportion of patients progresses to advanced stages, leading to a growing population of individuals with end-stage HF, many of whom are ineligible for heart transplantation or mechanical circulatory support due to age, comorbidities, frailty, or limited social support. This review aims to synthesize current evidence on palliative care (PC) integration in advanced HF, focusing on indications, timing of referral, …
