We report two cases of severe upper airway obstruction caused by supraglottic oedema secondary to adult epiglottitis and Ludwig's angina. In the former case, attempts to intubate with a direct laryngoscope failed but were successful once percutaneous transtracheal jet ventilation (PTJV) had been instituted. In the case with Ludwig's angina, PTJV was employed as a pre-emptive measure and the subsequent tracheal intubation with a direct laryngoscope was performed with unexpected ease. In both cases recognition of the glottic aperture was made feasible with PTJV by virtue of the fact that the hig

The aim of this paper is to review the role of extracorporeal membrane oxygenation (ECMO) in neonates with severe acute hypoxemic respiratory failure secondary to congenital diaphragmatic hernia (CDH). The difficulties in identifying patients with fatal lung hypoplasia are highlighted and the role of adjunctive therapies on ECMO (surfactant, inhaled nitric oxide, high-frequency ventilation and liquid lung distension) as well as the timing of surgical repair is discussed. Survivors of severe CDH who have been supported on ECMO have significant late mortality and morbidity. There remains a need …

OBJECTIVE: To (a) describe the experience with high-frequency oscillation (HFO) in children with acute respiratory distress syndrome (ARDS) unresponsive to conventional ventilation; (b) compare observed survival to that predicted by pediatric mortality scores and (c) determine if oxygenation index changes during HFO can predict survival.
DESIGN: Retrospective, observational study.
SETTING: A university hospital pediatric intensive care unit.
PATIENTS: Nineteen children with ARDS (PaO2/FIO2<200) unresponsive to conventional ventilation treated with HFO from January 1995 to September 1996.

UNLABELLED: Patients with critical upper airway stenosis require a tracheotomy for corrective surgery. We describe a new transtracheal device that permits safe ventilation of these patients without tracheotomy. It is based on a coaxial bicannular design that allows "push-pull" ventilation by jetting gas through the inner cannula and applying suction through the outer cannula. It further allows monitoring of airway pressure, tidal volume, and end-tidal CO2. The device was placed in the "trachea" of an artificial lung, and the preparation was made airtight by sealing the proximal end of the trac

The aim of the present study was to assess whether nasal continuous positive airway pressure (nCPAP) treatment, applied for only a few hours at the beginning of the night, has any residual effect on sleep and breathing during the ensuing hours of unassisted sleep in patients with obstructive sleep apnoea syndrome (OSAS). In 27 patients with newly-diagnosed OSAS, effective nCPAP was applied during the first part of the night and then withdrawn. Polysomnographic parameters after nCPAP withdrawal were compared with those of the corresponding part of the diagnostic polysomnography performed a few

Approximately 2-5% of admissions to pediatric intensive care units are due to diseases which develop into the adult respiratory distress syndrome (ARDS). About 8% of intensive care patient days are involved in treating patients with ARDS. ARDS is associated with approximately a third of deaths in the pediatric intensive care unit. Overall mortality is 50-70%. Frustratingly, little improvement in outcome has occurred over the past decade. The outcome predictors which have been developed to date have been unreliable for individual patients who may have been treated with alternative respiratory s

The usual anterior and lateral routes used for superior laryngeal (SL) nerve blocks are not often used because too difficult to carry out. A simpler and easier technique is described, which blocks the SL nerve at the level of the greater cornu of the hyoid bone. Because the nerve is not located accurately with this technique, a larger volume of local anaesthetic is required. This technique was tested in 31 patients scheduled for endoscopies with rigid tubes. The mean duration of endoscopy was 30 +/- 10 min. The use of 8 ml 1% lidocaine for the block 30 min surgical anaesthesia. In 2 patients,

BACKGROUND: Supraglottic jet ventilation (SJV) is used during flexible bronchoscopy to improve oxygenation, but its impact on overall lung ventilation is unclear. Thoracic electrical impedance tomography (EIT) offers real-time monitoring of respiratory function.
METHODS: This prospective pilot study enrolled patients undergoing flexible bronchoscopy. End-expiratory lung impedance (EELI) changes from baseline (ΔEELI) were measured at baseline (T0), oxygenation with Wei nasal jet (WNJ) (T1), SJV (T2), during bronchoscopy (T3), and post-bronchoscopy (T4). The primary outcome was ΔEELI at T2 vs.

BACKGROUND: High-frequency pressure oscillations created by gas bubbling through an underwater seal during bubble CPAP may enhance ventilation and aid in lung recruitment in premature infants. We hypothesized that there are no differences in the magnitude of oscillations in lung volume (ΔV) in a preterm neonatal lung model when different bubble CPAP systems are used.
METHODS: An anatomically realistic replica of an infant nasal airway model was attached to a Silastic test lung sealed within a calibrated plethysmograph. Nasal prongs were affixed to the simulated neonate and supported using bub

OBJECTIVE: To determine the effect of frequency on the accuracy of volume and pressure displays of high frequency oscillators.
METHODS: The effect of frequency on the displayed volume of the Stephanie, Dräger Babylog 8000 Plus, and SLE 5000 oscillators was assessed. A sine wave pump delivered a constant tidal volume at frequencies of 5-15 Hz to the patient manifold of the oscillators. The displayed volumes at each frequency were compared with the delivered volume. The effect of frequency on displayed pressure was assessed by connecting the oscillator's patient manifold to a lung model; three

An 18-month-old was transferred (intubated and ventilated) to our hospital with staphylococcal tracheitis, which progressed to a necrotizing pneumonitis, complicated by surgical emphysema and pneumomediastinum. Maximum conventional ventilation on a Servo 300 failed. Treatment with high frequency oscillatory ventilation (for 10 days) with a permissive hypercarbia and hypoxaemia strategy to limit mean airway pressure facilitated recovery in our patient.

We examined the effects of high-frequency (30-Hz) low-pressure oscillations on respiration in nine patients with central sleep apnea. All patients were studied during sleep and wore a nasal mask through which the oscillations were applied. All tests were performed during periods of repetitive central apneas. Respiratory efforts were monitored from the airflow and calibrated Respitrace signals. After several cycles of apnea were monitored, the oscillatory pressures were applied for brief periods (less than 5 s) at the midpoint of the central apneas. All trials in which arousal occurred were dis

BACKGROUND: Pneumothorax is common complication in newborns; its incidence is increased by intubation, particularly when high pressure ventilation is necessary. Systemic air embolism is a rare and usually fatal form of "displaced air".
CASE REPORT: A preterm newborn, 27 weeks of gestational age, weighing 940 g, with respiratory distress syndrome was treated by intubation at 1 min of life and high pressure ventilation (Peak 36 cmH2O, PEP 6 cmH2O, FiO2 = 1). Bilateral pulmonary interstitial emphysema was seen on X-rays at 4 hr of life. Acute cyanosis and bradycardia occurred at 7 hr of life; la
