Numerical simulation and analysis of fatigue performance for the humeral stem.

BACKGROUND AND OBJECTIVE: Fatigue failure of the humeral stem is a severe long-term failure after shoulder arthroplasty, causing harm to pat … humeral stem. RESULT: Standard testing conditions include an 8 mm diameter humeral stem, a 40-21B humeral head, an 8° tilt angle, and a 2 mm fillet radius. Further research found that the fatigue life of the humeral stem decreases with increasing patient weight, and patients should control their weight after surgery. CONCLUSIONS: The established automated f …

Femoral Head Allograft Glenoid Bone Reconstruction in Reverse Shoulder Arthroplasty.

Glenoid bone loss remains a significant challenge in both primary and revision reverse shoulder arthroplasty (RSA), particularly following infection, erosion, or component explantation. Adequate reconstruction is critical to restore the native joint line, inclination, and version while ensuring baseplate fixation and long-term stability. Although smaller defects can be addressed with augmented baseplates or humeral head autograft, massive bone loss often requires structural allografting. Femoral head allograft offers a viable and reproducible solution in complex revision cases where autograft …

Structural bone grafting with reverse shoulder arthroplasty for osteoarthritis with severe glenoid bone loss: outcomes using a long post.

BACKGROUND: Severe posterior glenoid bone loss with glenohumeral osteoarthritis with an intact rotator cuff can be managed with reverse shoulder arthroplasty but requires lateralization and version correction to avoid potential complications, such as instability, notching and implant failure. Angled bone grafting with humeral head autograft can provide durable glenoid bone stock, but results have been mixed. The purpose of this study was to evaluate patient-reported and objective outcomes as well as complication and failure rates for patients who underwent angled humeral head autografting for …

Ultrasound imaging of the humeral capitellum: a cadaveric study.

BACKGROUND: Ultrasound is suitable for routine examinations of capitellar osteochondritis dissecans because it can visualize both the subchondral bone and the overlying articular cartilage non-invasively. The radial head interferes with the sonographically visible area of the articular surface of the humeral capitellum, although the precise extent of this is currently unknown. This study aimed to investigate the visible area of the humeral capitellum using both anterior and posterior ultrasonographic scans. METHODS: Twelve elbows were used from cadavers with a mean age of 85.6 years. After ma

Shoulder kinematics with two-plane x-ray evaluation in patients with anterior instability or rotator cuff tearing.

The goals of this study were to define biplanar glenohumeral kinematics and glenohumeral-scapulothoracic motion relationships in normal patients with a two-plane radiograph series and then in patients with anterior shoulder instability or rotator cuff tear both before surgery and after surgical repair and postoperative rehabilitation. A two-plane radiographic series of x-ray films in the scapular and horizontal (axillary) planes was performed. With these films, measurements of the relationship between the centers of the humeral head and glenoid and measurements of the component contributions o…

"Champagne Fracture": A Superior Fracture-Dislocation of the Proximal Humerus: A Case Report.

A 79-year-old right-handed woman presented with an indirect trauma to her left shoulder after a fall down the stairs. X-rays and computed tomography showed a four-part glenohumeral fracture-dislocation with a subcutaneous ectopic location of the humeral head in the retroclavicular space. A reverse total shoulder arthroplasty was performed using a deltopectoral approach with direct superior extraction of the humeral head. The result at 2 years was a subjective shoulder value of 80%, an absolute Constant score of 59, and a relative Constant score of 92/100. To the best of our knowledge, this is …

Glenohumeral deformity secondary to brachial plexus birth palsy.

Ninety-four patients who had brachial plexus birth palsy were entered into a prospective study to evaluate the association between persistent palsy, age-related musculoskeletal deformity, and functional limitations. Of these patients, forty-two had either computerized tomography or magnetic resonance imaging to assess the presence and degree of incongruity of the glenohumeral joint, deformity of the humeral head, and hypoplasia of the glenoid as part of the preoperative planning for a reconstructive operation. Functional ability was rated with use of the classification of Mallet, on a scale of…

A Rare Neglected Ipsilateral Humeral Triad: Comminuted Proximal Humerus Nonunion with Avascular Necrosis, Mid-Shaft Fracture, and High Radial Nerve Palsy Managed with Staged Reconstruction.

INTRODUCTION: Ipsilateral humeral shaft and proximal humerus fractures are rare and typically an outcome of high-energy trauma. The coexistence of humeral head avascular necrosis (AVN) and high radial nerve palsy in a delayed presentation makes management particularly challenging and is rarely reported in the literature. This case is important as it highlights a unique combination of bony and neural pathology requiring a carefully staged reconstructive approach. CASE REPORT: A 39-year-old Indian man arrived 4 months after a road traffic accident with left shoulder and arm discomfort, deformit…

Magnetic resonance imaging of the shoulder in children with brachial pLexus birth palsy.

Five patients suffering from Erb-Duchenne brachial plexus birth palsy were prospectively studied with MRI. A group of 11 healthy children was used as a control to understand the MRI anatomy of the normal growing glenohumeral joint. A hypoplastic and flattened posterior part of the glenoid fossa and a blunt posterior labrum were found in all patients. Four patients had a blunt anterior labrum and a flattened humeral head. Three patients presented with a posterior subluxation of the humeral head. These results suggest that MRI provides a non-ionising and non-invasive method of demonstrating the …

Anterosuperior dislocation of the shoulder joint in an older patient with Parkinson's disease.

An 83-year-old woman, subsequently diagnosed with Parkinson's disease, fell on her right shoulder. Radiographs showed the humeral head dislocated anterosuperiorly. The supraspinatus tendon was massively disrupted and judged irreparable. The subscapularis tendon was repaired, however re-dislocation of the humeral head was confirmed after the surgery but further treatment was rejected as she was almost pain free and had a low activity level. The patient was followed up for one year after surgery and her shoulder became almost pain-free, but range of motion with active elevation was limited. In s…

Irreducible superolateral dislocation of the glenohumeral joint.

The overwhelming majority of glenohumeral dislocations are anterior dislocations that either spontaneously reduce or are reduced at the point of care without significant complications. Posterior dislocations are uncommon, and superior and inferior dislocations are even rarer. We present a case of "superolateral" shoulder dislocation in which the entire rotator cuff was torn either off its insertion or at the musculotendinous junction in combination with a large longitudinal split tear of the deltoid muscle. This allowed the humeral head to dislocate both superiorly and laterally into a subcuta…

Active and passive factors in inferior glenohumeral stabilization: a biomechanical model.

This study examines the stabilizing factors of the glenohumeral joint against inferior translation over a range of subluxations. Factors exa … ortant active stabilizers. Thus, tension in the long head of the biceps did not tend to depress the humeral head. The inferior glenohumeral ligament was an important passive stabilizer in external rotation. Understanding the effects of these factors adds insight into the underlying biomechanics of clinical shoulder instability.

High radial nerve entrapment neuropathy: an anatomical cadaver study and case report.

BACKGROUND: The pathogenetic mechanism of high radial nerve entrapment neuropathy involves the fibrous arch of the lateral head of the triceps brachii and lateral intermuscular septum. However, the details of these anatomical structures remain unknown. We described the anatomical course of the radial nerve in the upper arm and its relationship with the lateral head of the triceps brachii and lateral intermuscular septum. METHODS: Eleven freshly frozen cadavers (22 limbs, 7 females, and mean age: 87.3 years) were used. The elbow joint was placed at 90° flexion and a lateral incision was made.
