Primary hyperparathyroidism is rarely found in the pediatric population. Delays in diagnosis due to vague symptoms can lead to severe disease manifestations such as osteitis fibrosa cystica. We report a rare case of primary hyperparathyroidism in a 13-year-old girl who presented with a pathological femur fracture and numerous brown tumors.

BACKGROUND: Diagnosing primary hyperparathyroidism (PHP) in patients with osteoporosis is important because of the benefits of surgery. Screening patients with osteoporosis for PHP with only total serum calcium level will fail to diagnose PHP in patients with intermittent or no elevation of the total calcium level.

Primary hyperparathyroidism (PHPT) is a common condition which may have few symptoms. One of the principal concerns in the older person with minimally symptomatic PHPT is restoration of bone mineral density and prevention of fracture. Other important considerations are cardiovascular risk and quality of life. Surgery, the traditional treatment of choice, may not always correct these factors. We present a review of the literature and advice of medical therapies which should be of benefit, with emphasis on a multifaceted approach in protecting the patient from PHPT.

We report herein a case of familial primary hyperparathyroidism diagnosed in a 23-year-old woman who presented with hypercalcemia and urolithiasis. The parathyroid gland was removed, and pathological examination revealed chief cell adenoma. The proband's younger sister had undergone surgery for parathyroid adenoma at the age of 19, and her aunt had a history of urolithiasis with a high level of serum parathyroid hormone. We have not yet found evidence of any other endocrine disorders suggesting multiple endocrine neoplasia (MEN) type 1 in this pedigree.

OBJECTIVE: To study the diagnosis and treatment of hyperparathyroidism.
METHOD: 37 patients with primary hyperparathyroidism (PHPT) were confirmed pathologically. The course of the disease varied, the longest one being 14 years and the shortest one 3 months.

Except in unusual circumstances patients with documented primary hyperparathyroidism are probably best served by early operation performed by an experienced parathyroid surgeon.
