Beyond the Appendix: Unveiling Florid Lymphoid Hyperplasia.

Florid lymphoid hyperplasia (FLH) is a rare entity with an unknown etiology. It may occur anywhere in the gastrointestinal tract, from the base of the tongue to the anus, but is more common at the terminal ileum and rectum. It often presents as an acute abdominal condition. Our case illustrates how FLH can closely mimic the clinical presentation of acute appendicitis. The resemblance can be so striking that it poses a significant diagnostic challenge, highlighting the importance of careful evaluation in such scenarios.

Retroperitoneal Masson's haemangioma (intravascular papillary endothelial hyperplasia).

We present a case of an elderly female in her 70s admitted with right iliac fossa abdominal pain. MRI revealed a multiloculated retroperitoneal cystic lesion, initially suspected to be malignant but later identified as Masson's haemangioma following laparoscopic excision. This report highlights the rarity of Masson's haemangioma in the retroperitoneum and emphasises its distinction from other differential diagnoses.

[Diffuse idiopathic pulmonary neuroendocrine cell hyperplasia (DIPNECH)].

INTRODUCTION: Diffuse idiopathic pulmonary neuroendocrine cell hyperplasia (DIPNECH) is a rare, chronic condition that predominantly affects women over the age of 60. CURRENT KNOWLEDGE: DIPNECH combines non-specific clinical signs (chronic cough, dyspnea), bronchial obstruction on PFT and signs suggestive of bronchiolitis on chest CT associated with nodules and multiple micronodules. The diagnosis is most often histological, associating neuroendocrine cell hyperplasia, tumorlets and, inconsistently, carcinoid tumors and constrictive bronchiolitis. PROSPECTS: There are currently no recommenda…

Atypical Ductal Hyperplasia Presenting as Vulvar Mass.

Ectopic breast tissue (EBT) is a rare condition most frequently located in the axilla and only rarely in the vulva. It can undergo similar benign or malignant changes as normal breast tissue, including rare instances of atypical ductal hyperplasia (ADH). We report a 63-year-old woman with ectopic ADH in the right labia majora, initially misdiagnosed as a benign cyst. Histopathology confirmed EBT with ADH, and subsequent re-excision showed no residual disease. Immunohistochemistry supported the mammary origin. This case highlights the importance of histological examination for vulvar lesions su…

Ectopic Thyroid Nodule Hyperplasia: A Case Report.

BACKGROUND We present a rare case of an ectopic thyroid nodule hyperplasia, confirmed postoperatively after excision and histopathological examination of a chronic cervical mass. We discuss the different clinical and therapeutic features of this rare thyroid dysgenesis caused by a defect of migration of the gland along its normal way of descent. CASE REPORT A 48-year-old man with a history of hypertension and asthma presented with dysphagia and a progressively growing firm mass at the anterior part of his neck over the past 6 months. Ultrasound and computed tomography suggested that the mass w…

Giant Benign Prostatic Hyperplasia: A Case Report.

We present the case of a 69-year-old man experiencing lower urinary tract symptoms (LUTS), notably difficulties with urination. His total prostate-specific antigen level was measured at 3.52 ng/ml, accompanied by an International Prostate Symptom Score of 32. Transrectal ultrasound revealed a prostate volume of 268 cm3. Benign prostatic hyperplasia (BPH) is a common condition among aging men, often manifesting as LUTS. However, in rare instances, BPH can progress pathologically to giant prostatic hyperplasia, characterized by a prostate gland exceeding 500 g in weight. This report documents th…

Nodular Lymphoid Hyperplasia in Children: Recurrent Hematemesis.

Nodular lymphoid hyperplasia (NLH) is a pathology of the gastrointestinal tract that is commonly found in children. Most of its etiology is benign and associated with some underlying causes such as food hypersensitivity, viral or bacterial infection, giardiasis, Helicobacter pylori (H. pylori) infection, immunodeficiency, celiac disease, and inflammatory bowel disease. It is characterized by the growth of submucosal lymphoid tissue and a mucosal response to different types of noxious stimuli. In this report, we present the case of a child with recurrent hematemesis.

Intestinal obstruction secondary to Brunner's glands hyperplasia.

Brunner's gland hyperplasia constitutes 10.6% of benign tumors of the duodenum, with an incidence of 0.008%. It is usually an incidental finding during endoscopy or imaging tests as they are small and asymptomatic. In the case of symptomatic tumors, resection of the lesion is indicated. In lesions ≤2 cm, endoscopic resection can be chosen, reserving surgery for larger lesions or endoscopically inaccessible ones. We present the case of a patient with a history of vomiting and hyporexia of months of evolution who presented peptic ulcer perforation and underwent surgery. During follow-up, she pre…

An Excimer Laser-Induced Eruptive Sebaceous Hyperplasia.

Sebaceous hyperplasia (SH) is a benign proliferation of the sebaceous glands. Ultraviolet radiation is known to be the cofactor for the pathogenesis of SH. Frequently reported adverse events of 308-nm excimer laser are erythema, burning or pain, and itching sensation. However, the role of excimer laser as an aetiological factor of SH is rarely reported in the literature. A 59-year-old female presented with several elevated, umbilicated papules on the face. Papular eruption appeared abruptly after 11 weeks of treatment for vitiligo which includes 308-nm excimer laser therapy. The distribution o…

Angiosarcoma in Long-Standing Nodular Regenerative Hyperplasia.

Nodular regenerative hyperplasia (NRH) is an uncommon chronic liver disease associated with noncirrhotic portal hypertension. A 29-year-old man with X-linked agammaglobulinemia and NRH complicated by noncirrhotic portal hypertension was followed. Laboratory test results showed pancytopenia and elevated transaminases. Magnetic resonance imaging showed innumerable hepatic lesions. Liver biopsy showed angiosarcoma (hepatic angiosarcoma). He was not a candidate for medical or surgical intervention because of extensive disease and died. Liver histology at autopsy showed infiltrating foci of angiosa…

Long-Term Outcomes of Congenital Adrenal Hyperplasia.

A plethora of negative long-term outcomes have been associated with congenital adrenal hyperplasia (CAH). The causes are multiple and involve supra-physiological gluco- and mineralocorticoid replacement, excess adrenal androgens both intrauterine and postnatal, elevated steroid precursor and adrenocorticotropic hormone levels, living with a congenital condition as well as the proximity of the cytochrome P450 family 21 subfamily A member 2 (CYP21A2) gene to other genes. This review aims to discuss the different long-term outcomes of CAH.

Congenital Adrenal Hyperplasia and Ehlers-Danlos Syndrome.

Congenital adrenal hyperplasia (CAH) secondary to 21-hydroxylase deficiency is an autosomal recessive disorder. The 21-hydroxylase enzyme P450c21 is encoded by the CYP21A2 gene located on chromosome 6p21.33 within the HLA major histocompatibility complex. This locus also contains the CYP21A1P, a non-functional pseudogene, that is highly homologous to the CYP21A2 gene. Other duplicated genes are C4A and C4B, that encode two isoforms of complement factor C4, the RP1 gene that encodes a serine/threonine protein kinase, and the TNXB gene that, encodes the extracellular matrix glycoprotein tenascin…

[Long-term morbidity in congenital adrenal hyperplasia].

Congenital adrenal hyperplasia (CAH) is one of the most common autosomal recessive disorders and is characterized by cortisol deficiency. The most common cause of CAH is a mutation in the CYP21A2 gene, resulting in 21-hydroxylase deficiency in the adrenal cortex. The lack of cortisol causes an increase in adrenocorticotropic hormone (ACTH), which in turn results in an excess of adrenal androgens. Aldosterone synthesis may also be impaired. The clinical manifestation of CAH depends on the residual activity of 21-hydroxylase and the subsequent lack of cortisol and adrenal androgen excess. While …

Modified-Release Hydrocortisone in Congenital Adrenal Hyperplasia.

CONTEXT: Standard glucocorticoid therapy in congenital adrenal hyperplasia (CAH) regularly fails to control androgen excess, causing glucocorticoid overexposure and poor health outcomes. OBJECTIVE: We investigated whether modified-release hydrocortisone (MR-HC), which mimics physiologic cortisol secretion, could improve disease control. METHODS: A 6-month, randomized, phase 3 study was conducted of MR-HC vs standard glucocorticoid, followed by a single-arm MR-HC extension study. Primary outcomes were change in 24-hour SD score (SDS) of androgen precursor 17-hydroxyprogesterone (17OHP) for ph…
