Negative impact of underlying non-insulin-dependent diabetes mellitus nephropathy on long-term allograft survival in kidney transplantation: a 10-year analysis from a single center.

INTRODUCTION: We analyzed the relationship between underlying nephropathy and long-term outcomes in kidney transplant recipients. METHODS: … ONCLUSION: Underlying NIDDMN before KTx was a significant risk factor for long-term graft function. Immunologic factors and nonimmunologic factors influenced the long-term outcomes in patients with underlying NIDDMN.

Rapid progression of solitary plasmacytoma to multiple myeloma in lumbar vertebra.

The prognosis of solitary plasmacytoma varies greatly, with some patients recovering after surgical removal or local fractional radiation therapy, and others progressing to multiple myeloma years later. Primary detection of progression to multiple myeloma is important in the treatment of solitary plasmacytoma. There have been several analyses of the risk factors involved in the early progression to multiple myeloma. We describe one case of solitary plasmacytoma of the lumbar vertebra that was treated with surgical decompression with stabilization and additional radiotherapy. The patient had no

[Schistosoma mansoni in the context of the Brazilian health policy].

There are many factors involved in the determination of the emergence and reemergence of infectious diseases. In the case of Schistosomiasis biological factors are highlighted as related to the habitat, to the microorganisms and hosts adaptations and mutations, to the immunologic reply of the host and to the bio-ecology adaptations of intermediate hosts. These are added to the not less important factors related to the management politics, occupation of the environment and allocation of financial resources. Brazil congregates, today, an important echo-epidemiologic conditions for the reemergenc

Do chemokines play a role in HIV-1 heterosexual transmission? Susceptibility to HIV infection.

The probability of HIV transmission depends on the infectiousness of the infected partner, the susceptibility of the healthy partner, and th … o infection by macrophage-tropic HIV strains. Therefore, knowledge of this and of other genetic and immunologic factors that affect HIV-1 transmission is essential in understanding HIV-1 entry into cells, developing strategies for combating HIV-1 transmission and spread, and implementing new models for HIV-1 therapies and, hopefully, vaccines.

Risk factors for infantile atopic dermatitis and recurrent wheezing.

BACKGROUND: The pathogenic mechanisms of atopic dermatitis (AD) and recurrent wheezing (RW) during infancy are not fully understood. OBJECTIVE: We evaluated immunological markers associated with AD and RW during infancy. METHODS: We followed a cohort (n = 314) from birth to 14 months of age. Some of the participants underwent a physical examination and blood test at 6 and 14 months of age. Univariate and multivariate logistic regression analysis and receiver operating characteristic curve analysis were performed to find which immunological markers could be risk factors for AD and RW. RESULT

The influence of the environment and other exogenous agents on spontaneous abortion risk.

It is estimated that close to 30% of all pregnancies end in spontaneous abortion. Although about 60% of spontaneous abortions are thought to be due to genetic, infectious, hormonal, and immunological factors, the role of the environment remains poorly understood. Pregnancy involves a delicate balance of hormonal and immunological functions, which may be affected by environmental substances. Many toxic substances that are persistent in the environment and accumulate in the fatty tissues may disrupt this equilibrium. This overview addresses known risk factors for spontaneous abortions and examin

[Renal disease in diabetics. Immunological bases of tubule-interstitial fibrosis and glomeruloesclerosis. Current therapeutic approach].

Susceptibility of kidney to become target of the immunological aggression is easily recognizable when we consider the high number of diseases that, with a pathogenesis explained by autoimmunity mechanisms, have the kidney as a target organ. In diabetes mellitus, hyperglycemic state stimulates different molecular factors conditioning tubule-interstitial fibrosis and glomerulosclerosis, among them the most important are: formation of products of advanced glycation; activation of protein kinase C, activation of the angiotensin II and the activation of the nuclear factor kappaB. These factors are

Therapy for disseminated coccidioidomycosis with transfer factor from a related donor.

A 17 year old caucasian woman in whom disseminated coccidioidomycosis developed with culture positive meningitis during her third trimester of pregnancy was treated with amphotericin B and subsequently with transfer factor prepared from her father's peripheral lymphocytes. Clinical response and in vivo and in vitro immunologic data indicated that this transfer factor afforded a significant contribution to her survival whereas previous therapy with transfer factor from an unrelated donor provided only transient immunologic reactivity. This experience suggests that transfer factor prepared from

Chronic, active Epstein-Barr virus infection.

Chronic active Epstein-Barr virus (EBV) infection is an uncommon outcome of EBV infection and may present as a waxing and waning or fulminant syndrome. Unlike acute infectious mononucleosis, wherein EBV establishes lifelong infection and survives by maintaining a delicate balance with the host as a latent infection, in chronic active EBV infection the host-virus balance is disturbed. The mechanisms by which this balance becomes perturbed are likely to be heterogenous and may involve host immune factors, viral factors, or both. A number of subtle immunologic defects have been reported in patien

Neuroinflammation in kidney disease and dialysis.

The complex relationship between chronic kidney disease (CKD) and neuroinflammation shows how important immunological processes are in mediating cognitive dysfunction and psychiatric symptoms in this disease. Proinflammatory cytokines and chemokines, such as IL-1β and IL-6, are capable of crossing the blood-brain barrier, and, consequently, may contribute to neuropsychiatric symptoms including anxiety, depression, and cognitive impairment in CKD patients. The peptides of the renin-angiotensin system (RAS), with their dual functions in inflammation and neuroprotection, also highlight the intric

Alloimmunization in sickle cell disease: Addressing an ongoing clinical challenge.

Transfusion therapy remains a cornerstone in the management of sickle cell disease (SCD). Yet, repeated exposure to donor red blood cells poses a substantial risk for alloimmunization against red blood cell antigens. Patients with SCD exhibit a disproportionately high incidence of alloimmunization and often experience more severe clinical consequences than other transfusion-dependent populations. This review explores the multifactorial mechanisms underlying this heightened immunogenicity, the clinical and immunologic sequelae that result, and the factors contributing to the under-recognition o

Recent developments in the pathogenesis of pruritus in bullous pemphigoid.

Bullous pemphigoid (BP) is a common autoimmune bullous disease which mainly affects the elderly. The incidence of BP is gradually increasing and associated with high mortality. This disease is clinically characterized by intensely pruritic and widespread bullous lesions. Alternative therapy options for pruritus in patients with BP are limited primarily because pathophysiological mechanisms of itching in BP are still unclear. This review aims to explain crucial concepts of the pathogenesis of pruritus in BP. Vital findings in recent years will be summarized, and cofactors of the pathogenesis of

Lung Cancer Staging.

Anatomic staging is a critical step in evaluation of patients with lung cancer. Accurate identification of stage based on features of primary tumor (T), regional nodes (N), and metastatic disease (M) is fundamental to determining appropriate care. In this article, the TNM components of the anatomic staging system and a framework for description of lung cancer with multiple pulmonary sites of involvement are discussed. TNM combinations are grouped according to prognosis, with patient-level, tumor-level, and environment-level factors also influencing survival outcomes. Although the staging syste

Behçet's disease: minireview with emphasis on oral aspects.

Adamatiades-Behçet disease (BD) is a primary systemic vasculitis of unknown origin, that may involve blood vessels of all sizes. The clinical hall-mark of the disease are recurrent oral aphthous ulcers, painful, sharply margined and often associated with recurrent genital ulcers. A variety of skin lesions, such as erythema nodosum-like nodules and purpuric lesions are frequently observed. Less common manifestations of BD include thrombophlebitis, central nervous system and ocular inflammation which, however, may impact significant morbidity and mortality. The etiology is unknown, although gene

[Risk factors for occurrence of ophthalmopathy in the course of Graves-Basedow disease].

Ophthalmopathy is a serious complication of Graves-Basedow disease. It is a common and the most significant symptom of this illness and usually persists even when other symptoms associated with abnormal thyroid function disappear. Despite of well-known auto-immunologic patho-genesis of ophthalmopathy it is still hard to predict when this severe complication will occur. This condition is thought to be associated with diagnosis and treatment errors. We studied the patients suffering from Graves-Basedow disease with and without ophthalmopathy. We concluded that the risk factors for ophthalmopathy

Sensitization versus elicitation in allergic contact dermatitis: potential differences at cellular and molecular levels.

The induction and elicitation phases of allergic contact dermatitis are very complicated processes that differ in many respects. Many issues remain to be investigated, especially in the elicitation phase of this disease. Several key molecules involved in the development and maintenance of allergic contact dermatitis have already been defined, including a number of different cytokines and adhesion molecules. Attempts are being made to develop new, more specific pharmacologic and immunologic inhibitors of their activity. Because patients who see a doctor are already sensitized, further elucidati

Delayed graft function: immediate and late impact.

Delayed graft function remains a frequent problem after renal transplantation, which is often associated with subsequent graft failure. The major risks for delayed graft function are incurred during organ procurement, preservation, and transplantation, and are predominated by ischemic injury. The cofactors associated with delayed graft function that lead to subsequent poor outcome include early acute rejection as well as immunologic risk factors for rejection, such as presensitization, human leukocyte antigen mismatch, and previous loss of graft. Numerous diagnostic and therapeutic approaches

Inflammatory myopathies: issues in diagnosis and management.

The techniques of magnetic resonance imaging and spectroscopy have been shown to have utility in the diagnosis and management of inflammatory muscle diseases. But perhaps more important have been the new insights into the pathophysiology of these diseases which MR studies, along with new immunologic data on autoantibodies and cellular infiltrates, have afforded. Pathologic subsets of inflammatory muscle disorders have been identified, suggesting, for example, that PM and DM are distinct disorders, thereby challenging the idea that these are relatively homogeneous syndromes. Further insights in

A localized absence of interleukin-4 triggers periodontal disease activity: a novel hypothesis.

Pathogenic bacteria constitute the primary extrinsic agent in the etiology of Adult periodontitis. In addition to direct toxic effects, bacteria induce destructive immunologic and other inflammatory reactions in the host, leading to the observed pathologic alteration in the tissue. The risk to develop periodontal disease is not equal for all individuals, suggesting host factors are important in determining an individuals disease susceptibility. Regulation of immune response is important in maintaining the equilibrium between periodontal health and disease. We hypothesize that, in the case of A
